Curtis was overweight and flirting with high blood pressure. “I was always tired and didn’t have much energy,” he recalls. “I had stopped doing some of the things I enjoyed.”

Plan Design Whether you offer a PPO plan or Consumer Driven Health Plan (CDHP), your plan design should align with your company’s healthcare strategy. We design Value-Based Health Plans that create incentives for employees to make personal health care choices, use high performance providers and engage in healthier lifestyles. Plan Administration One of the reasons employers love working with us is our partnership with MedCost, a third-party administrator (TPA) based in North Carolina. This established company is known for its excellent customer service. They process claims, issue ID cards and handle your employees’ questions, attending to the details of administration so you can focus on your business.

Self-funded plans are an essential component of our approach. Because you eliminate the third-party carrier, you keep savings generated when costs are lowered. This offers an opportunity to reduce your expenditures over time, while providing a tremendous incentive to keep your people healthy. Most businesses can expect to save 8% to 12% annually compared to the cost of traditional insurance, and you control risk through stop-loss insurance. Furthermore, you’ll change the dynamic with your employees; you are now positioned to support them in their journey to wellness.

Provider Network In healthcare, quality matters. With a Healthy State plan, employees can choose between two outstanding network options: Mission Health Partners® (MHP) or MHP plus MedCost. MHP is Healthy State’s signature network and includes nearly 1,100 physicians, ten hospitals and thousands of other healthcare practitioners currently serving nearly 90,000 individuals. This critically acclaimed clinically integrated network has the professionals, infrastructure and programs in place to ensure coordinated care, seamless communication and an optimal customer experience by placing people’s needs first. Employees also can choose the MedCost network with a broader geographical reach and access to national networks.

HOW IT WORKS

Population Health: The Approach that Empowers Employees.Our approach to healthcare encourages each member to reach their own unique potential through engagement, education and empowerment. We address behavior and social factors that contribute – or create barriers – to an individual’s ability to achieve healthy outcomes. In addition, we supply you with monthly reports and dashboards to help you support your employees.

Primary Prevention: We work with employees and your company to help position your employees for success with a wellness account manager, a worksite assessment, an online portal for employee health and our award-winning wellness programming.

Early Intervention: We address issues early on the curve before disease and acute conditions develop.

Chronic Condition Management Program: We provide support and care for individuals diagnosed with chronic conditions, including diabetes, depression, high blood pressure, high cholesterol, COPD and asthma.

Post-Acute Care Management: After a health event like a stroke, heart attack or surgery occurs, we provide a smooth transition to the care and services people need to return to optimal health.
