Steps to Achieve ServicesSteps to Achieve Services

1. Contractor/case manager family joint visits within one week of the referral date.

• Goal: identify immediate psychological and social risk that contribute to crisis and risk of child maltreatment.

2. Contractor advises case manager on weekly basis, regarding the family’s status.

3. Contractor advises County DFCS office within five days if is unable or unwilling to service a particular family and states the reason for this decision.

4. Contractor submits written docs of all activity to include

– Visitation documentation forms– Monthly activity reports

documenting family’s progress toward goals and status.

– Reports attached to Annex A (Invoice) on a monthly basis, for month in which activity is billed.

5. Contractor agrees to work according to Progress Payment Schedule.

6. Contractor provides planned program of ongoing training and staff development in the areas of Parenting and Child Development including regular staff meetings for all paraprofessional staff employed by the contractor.
