Hospital Aim: Reduce All Cause All Payor ReadmissionsFrom 13% to 11% by December 31, 2018.

The Project ACHIEVE* study on readmissions suggests that patients and their caregivers need to have these goals met to feel ready for discharge:

• To feel prepared and capable when they leave• To know what to do and who to contact if help is needed• To feel cared for and about during their hospitalization and

discharge planning

We will evaluate if peer rounding will:1) Determine if these goals were met upon last discharge2) Assist in meeting these goals before current discharge
