Worth Crescent is one of two services providing Replacement Care (or Res-pite) to adults with a Learning Disability. Worth Crescent has 10 bedrooms, 2 of which are on the Ground Floor. There is no lift at the property, meaning most people who attend are physically mobile.
The CQC (Care Quality Commission inspection report included: -
"At our last inspection we rated the service good. At this inspection we found the evidence continued to support the rating of good and there was no evi-dence or information from our inspection and on-going monitoring that demonstrated serious risks or concerns." "Worth Crescent has been developed and designed in line with the values that underpin Registering the Right Support and other best practice guid-ance. These values include choice, promotion of independence and inclusion. People with learning disabilities and autism using the service can live as ordi-nary a life as any Citizen." "The registered manager had identified potential risks to people and had put plans in place to support staff. Staff were consistent with management of risk while also promoting people's independence." Pershore Short Breaks is the second of two services providing Replacement Care to Adults with a Learning Disability. There are 4 bedrooms, all of which are on the Ground Floor. The service shares the building with the Area Social Work Team, and is also utilised by Learning and Development and Urgent Promoting Independence.

“At this inspection we found the quality of care had been maintained and people continued to receive a service that was well led, providing safe, caring, effective and responsive care and support that met their needs." Exmoor Drive
Exmoor Drive Exmoor Drive is a 12 bed Residential home providing long term care to adults with a Learning Disability. For some time the beds were restricted to 8 in order to ensure the staffing was suitable for those people living there. Investment was made to the staffing budget in 2017 and the remaining beds were opened, and the service is being developed to provide care to those people with a Learning Disability who also develop dementia.

"People received care from staff who knew how to keep them safe. Staff knew what they would do to protect a person from the risk of harm and how to report any concerns." "People got the assistance they asked for or staff ensured they were available to help them when needed." "People's decisions about their care and treatment had been recorded and staff showed they listened and responded to people's choice to choose or decline care." Connect LD Long Term
DESCRIPTION:
There are several Connect services operating across the County. These
services are based in Bromsgrove (covering Bromsgrove and Droitwich),
Redditch, Worcester, Malvern and Evesham. The service in Wyre Forest
closed in March 2019 following a Cabinet decision in November 2018.
The services operate Monday to Friday and they provide mainly community
based day services to adults with a Learning Disability. Some of these services
are based in building owned by Worcestershire County Council while others
rent space from other buildings.
FEEDBACK:
Recent engagement with the people using the services and their families
prior to changes generated positive feedback:
• Carers reported that adults were very happy in the services and that
staff are providing a high quality service; the stability and reliability
of the services are highly valued by carers; people using the services
have long-established friendship groups; carers feel that their family
members are safe in the services and have a high degree of trust for
the staff.
• The variety of activities and amount of individual choice on a day-to-
day basis are valued by carers; the services are person-centred and
staff successfully meet everyone's needs and individual choices; the
community based activities are plentiful and varied and enable
people to contribute to their communities.
• Carers feel that the Connect services successfully increase people's
Independence; the separation of Connect from Resource Centres as
part of the previous reorganisation has enabled people in Connects
to increase their independence and confidence; the Connects
services encourage independence, for example through independent
travel training and voluntary work opportunities; there was a feeling
that people using the service have "blossomed" under the new
model.
• The service is of value to carers to help them maintain their caring
role. Lots of people attending Connect also attend other externally-
provided day services; however, Connect is a valued part of the day
service offer and it was felt that Connect offers a service which is
different from most other day service offers, with the opportunities
to access varied activities out in the community.

Resource Centres DESCRIPTION:
There are 4 Resource Centres in the county; Wyre Forest, Bromsgrove,
Pershore and Worcester.
The Resource Centres operate Monday to Friday and provide a variety of ac-
tivities to people with a Learning Disability. Many of these activities are
building based with some community based activities planned according to
individual needs and preferences.
FEEDBACK:
As with the Connect services, recent engagement with people using the ser-
vices and their carers gave positive feedback:
• The Resource Centres provide a quality service; carers feel that eve-
ryone is well looked after in a very positive way; carers often com-
pared the Council-provided services favourably to other services; the
services enable people to engage with their peer group and they are
"one big family"; there is effective partnership working between the
Resource Centres and other services.
• The experience and quality of the staff in the services is highly val-
ued by carers and there is a relationship of trust; people feel safe and
secure in the services and stability of the service, including the conti-
nuity of staff, is highly valued .
• The services offer a wide variety of activities, although some carers
felt that activities could be developed even further with the addition
of new types of activities.
• It was particularly felt that the services should be promoted to
younger people transitioning to adulthood.
LEISURE LINK:
Leisure Link is an extension of the Council's Day Services offer, providing 3
social clubs in the evenings in the Wyre Forest area, although these are at-
tended by people from across the county. There is a disco held every fort-
night, a music group held weekly and a sports club held fortnightly. The ser-
vice is co-ordinated by a single person working part time and uses sessional
workers to support this. The office is located at Worth Crescent.

Domiciliary Care Services DESCRIPTION:
There are a number of services that fall under the umbrella of Domiciliary Care. These
services operate from Wildwood.
URGENT PROMOTING INDEPENDENCE TEAM (UPI – sometimes referred to as Pathway 1
This service is commissioned by the CCGs and is designed to provide support to peo-
ple discharging from hospital. The target is to achieve 70 discharges per week from
Acute hospitals, Community hospitals and outlying hospitals. The service provides
short term support, usually around 4 weeks, and aims to reduce the need for long
term care by promoting people's independence after a period of illness. The service
regularly outperforms its targets. The most recent report on outcomes showed that in
the 12 months April 2018-March 2019 the service received 3742 referrals. Of the people
progressing to a service (2193) 94% had outcomes met or partially met. The main rea-
son for outcomes not being met or only partially met was medical limitations (217 out
of 289). The number of care hours per week saved as a cumulative total for the year is
5869.5. This service is also supplemented by a waking Night Service which operates
countywide along with a Night Sitting Service which provides urgent short term
support to prevent admission.
SIDE BY SIDE:
This is a specialist service providing domiciliary care to people living with dementia.
There are currently 25 people in receipt of the service and 35 staff. This group of staff
also provide social care in the two Prisons in the county, the additional support for
people living at The Woodlands (Supported Living) are also used as a resource to
provide a Service of Last Resort to those people whose care provider is unable to
continue or where a provider cannot be found in the market.
The Domiciliary Care service is sup-
ported by a Homecare Call Centre,
which takes calls from people using
the services, their families and carers
and staff employed in the services. It
also provides a point of access out of
normal office hours for other
professionals such as NHS staff
wanting to refer to the UPI service.
The Call Centre is operated from
06:30 to 22:00 7 days per week and is
staffed by people from existing teams
within their budgets.
RISK TEAM: In addition there is also a
Risk Team which supports people
with complex needs and challenging
behaviour. The Risk Team provides
advice and practical support to the
other Domiciliary Care Teams in
order to maximise the range of needs
that the services can meet. The Risk
Team has also been used in the past
to support external services and to
support reviews of care.

of Better Care Funding – for UPI, Night
Service and Night Sitting)

Shared Lives
Shared Lives operates from Wildwood. Shared Lives is a service which provides accommodation
with support, but in a domestic family setting rather than traditional Supported Living. Shared
Lives has 85 placements supporting 117 individuals. Placements are provided by self-employed
Shared Lives Providers, of which there are currently 125 approved providers. Most placements
are for people with Learning Disabilities, though the service does support people with physical
disabilities and Older People. Shared Lives has recently been promoted with the aim of recruit-
ing more Shared Lives Providers. There is a very good awareness of Shared Lives, particularly
amongst the Learning Disability Social Work Teams.
Shared Lives is a member of Shared Lives Plus – this is the UK network for shared living, and
offer guidance, information regarding best practice, legal advice and insurance.

The following are key findings from the inspection (please note – references to Shared Lives Carers and
"staff" means those people who provide the care, and not the staff employed by the Council – these are
referred to in the CQC report as "Shared Lives Workers"):
“All people we spoke with felt safe from harm. One person told us '[Carers name] has got my best Interests at heart'. "Shared Lives carers told us how the training around maintaining people's safety and continued sup-port from the shared lives workers, helped them to support people in a way which kept them safe but promoted their independence." "Staff told us that people were encouraged to be independent and this was the ethos of the shared lives scheme. One person told us, 'I have my phone with me, so if I need to ring for help I know I can call, [shared lives carer and workers names.'" "The carers told us they supported people who they had been matched with and were able to support them appropriately." "We saw there were robust checks in place to ensure the carers were safe and had the right values to support people." "People we spoke with did not have any concerns about how their medication was managed. We spoke with carers who admin-istered medication and they told us they had training and support to do this safely." "A further shared lives worker told us how the registered manager encouraged them to develop their knowledge and said, 'They give you the opportunity to progress in your career'" "People felt their views and wishes were respected and that staff sought their consent first." "One person told us how they had been supported by their carer to eat a healthy balanced diet and was proud to tell us of their steady weight loss and how this had benefitted their overall fitness."
