District and province should be mention.

Applicant have to provide address proof .(NIC / Driving license /Birth Certificate / Electricity bill /water bill, rent agreement / Gramsewaka certificate or acceptable proof)

Mention the contactable correct contact numbers.

Father, mother and spouse name should fill as full name as far as possible.

If anyone mention only one name the visa application will return.

Port of arrival and exit should mention as “ALL PORT”

Expected date of journey should be mention after 5 days from applying date.
