Risk can only be controlled in different scenarios but not absolutely removed. A defect generally transforms to a risk.

Defect and Risk Interrelation

Let’s imagine, for example, you are developing an accounting application and you have done the incorrect tax calculation. There is a great probability that this will result to the risk of the organization to incur losses.

But if this defect is controlled then you can eliminate this risk absolutely or at least mini-mize it.
