To Our Community:

In a world where people can order, view, or receive almost anything “on demand,” often with same-day access, why has healthcare – needed by everyone – been left out?

We’ve engaged this question over years, and – excitingly in 2017 – we made great leaps.

Via innovation and enhanced partnership with the State, CN Guidance & Counseling Services vastly expanded on-the-spot appointments for people struggling with mental health and substance use issues — regardless of their ability to pay.

Declared a “Certified Community Behavioral Health Clinic” (CCBHC) in July by state and federal offices, CN Guidance and our new Integrated Counseling & Recovery Services program have met rigorous requirements to serve as one of only 13 providers across NY state and 67 nationwide taking healthcare access and quality to a new level. We are part of a 2-year demonstration project that providers and leaders around the country are watching closely.

Thanks to CCBHC reimbursement rates and our own vision, CN Guidance has increased its staff by over 50 people, expanded hours that we can see clients for same-day appointments, and provided a new combination of assessment and treatment for mental health, substance use disorder, and physical/medical conditions, all under one roof. We are better able to reach vulnerable populations such as Veterans, housing-unstable individuals, and people struggling with co-occurring disorders.

In just the first six months, we increased the number of people enrolling in our clinic services by 42%, partly by reducing average waiting time for an appointment by 60% .

And we’re not done yet. In 2018, acknowledging many people’s comfort with technology, we’re seeking support to deliver treatment via telemedicine (secure video and audio) to reach more people in need who otherwise would have no access to care.

In 2017, this program served 1,065 clients (521 of them new), with 86% reporting high adherence to their medications.

Residential ServicesGroup homes, supportive housing, crisis respite, and family housing.

Personalized Recovery-Oriented Services (PROS)A comprehensive structured program for individuals with serious mental illness.

PROS provided 222 participants in 2017 the opportunity to choose from 130 groups per week to meet their diverse needs and individual recovery goals. Fewer than 5% of participants required inpatient treatment.

Assertive Community Treatment (ACT)Mobile psychiatrist, social worker, nurse, and peer specialist serving high-risk individuals out in the community, using a mobile and team-based approach.

Youth/Young Adult-Oriented Supportl Project FORWARD is a program that helps young individuals and couples develop relationship and communication skills.

l The Northport/East Northport Community Drug and Alcohol Task Force engages parents, teachers, youth, pharmacists, media, police, religious leaders, and others to help reduce substance use among youth. See our Color Run event!

Our Events 2017

Ribbon Cutting at New Crisis Respite Center2017 marked the opening of our new Crisis Respite Center: for people in crisis or at imminent risk, a place to stay for up to 30 days, with 24-hour supervision.

‘RUNDAY’ The 2017 Barbara Bartell Memorial Runday, engaged over 300 runners in a 5k and half-mile fun-run for kids. 2018 Runday to be held Saturday, May 26th at 8:30AM. Hicksville.

Golf OutingMore than 100 community members came to Woodbury in July to participate in a sold-out annual golf outing that supports the work of CN Guidance. 2018 golf outing to be held Monday, July 16th. 1PM tee time. Woodbury.

Color RunThis substance use awareness event, co-led by youth in the Northport/East Northport Drug & Alcohol Task Force, kicks off a campaign for Recovery Awareness and Prevention (RAP) Week.2018 Color Run to be held Saturday, October 20th. Northport/East Northport.

TASTE of HOPE 180 community members came together at CN Guidance’s third annual Taste of Hope gala in late October.2018 Taste of Hope Gala to be held Wednesday, October 24th. 6:30PM. Old Westbury.
