The goal of Home Care Medical’s best+fit Ventilator Care Program is to ensure each patient receives the appropriate ventilator system for the duration of their need.

Achieving this objective aligns with Home Care Medical’s mission to enhance the quality of life of those we serve.

Our Ventilator Care Program is a collaboration between the patient, family members and caregivers with Home Care Medical’s respiratory therapists, certified dietitian and registered pharmacists. In addition, we work closely with the patient’s physician, hospital discharge coordinators and home health agency personnel to ensure the patient’s transition from the hospital to the home. Our collective goal is to provide the training and support necessary to enable the ventilator-dependent patient to live at home safely – and to achieve the highest level of independence.

experienced staff can suggest additional products and services to enhance the patient’s quality of life, foster independence, and increase patient safety.

These products include: pulse oximeters; incentive spirometry; nebulizers; ambulatory aids; wheeled walkers, wheelchairs and scooters; bathroom safety products; aids for daily living, and much more.

Home Care Medical has been providing retail service to the community since 1974. We recently remodeled and expanded our flagship store in Milwaukee. All lines of care are represented along with certified orthotic and prosthetic fitters and highly trained Personal Care Consultants. In addition, we have a second retail location in West Bend, near the West Bend Clinic to serve Washington County and points north.

4Respiratory Clinical Support Staff: Coordinate discharge and set-ups for new respiratory patients. They are our lead in handling customer questions and concerns.

4Respiratory Support Coordinators:

Help with scheduling, coordinating compliance, and working closely with Respiratory Intake Specialists to meet the needs of our customers.

4Respiratory Intake Specialists: Process calls from clients and referrals that are placing initial and ongoing orders for respiratory products and services.

4Home Care Technicians: Are trained to deliver, set-up and educate patients on oxygen therapy

equipment. They teach clients on the safe and effective use of the equipment.

Our goal is to make the process of living at home or an alternative site on a ventilator more personal and comfortable by instilling confidence and empowering both the patient and the caregivers. This is what saves lives . . . caregivers who have the confidence to perform routine tasks and handle emergency situations.

While the patient is still hospitalized, we assist coordination of educating caregivers about everything from anatomy and physiology of the pulmonary system, tube feedings, tracheostomy care and medication compliance to equipment management and emergency procedures. The patient is not released from the hospital until the caregiver is competent or demonstrates essential caregiving and ventilator management. This is provided by a coordi-nated effort of hospital and homecare personnel to meet the needs of every patient, family and home environment.

Our patient’s safety comes first. From the initial set-up with our home assessments for our best+fit Ventilator Care Program, we take every opportu-nity we can to ensure the health and safety of our patients.

Primary duties of the respiratory team are delineated as follows:

4Respiratory Therapists: Teach utilization of equipment to users and family members and provide patient care for people needing ventilators (invasive and non-invasive), apnea monitors, oxygen, suction and pulse oximetry.

4Respiratory Specialists/Emergency Medical Technicians: Follow-up on patients new to oxygen. They evaluate patients for the most

appropriate oxygen delivery system, which could be LOX, concentrator, oxygen conserving device or a self-filling oxygen system. They also review the home safety aspects of having oxygen in the home, including oxygen outcomes and compliance monitoring.

Continuation of CareOur Ventilator Care Program assists in the management of chronic respira-tory insufficiencies and/or respiratory failure, which requires invasive “home” mechanical ventilation. Our experienced clinicians or respiratory therapists are trained to manage some of the most medically challenged and complex – but rewarding – patients and their families.

We are pleased to offer the following Continuation of Care:

4 Clinical and environmental patient assessment – monthly/quarterly

4 Equipment review and management

4 Reviewing and assessing ongoing supply needs

4 Ongoing competency review

4 Respiratory therapist available 24 hours / 7 days a week

Transition Home Once at home, the patient and caregivers are initially visited on a daily basis by their assigned respiratory therapist during the first week. When the caregiver is comfortable with the ventilator, the respiratory therapists visits the patient three times a week – then gradually the visits are reduced to once a month or quarterly, depending on the needs of the patient and the caregivers. A presence in the home and building a partner relationship with the caregivers is what ensures successful treatment and patient safety.

In addition to the training provided in the hospital and scheduled home visits, Home Care Medical’s Ventilator Care Program provides:
