Home visits in Cantanhede, Agueda, Anadia and Coimbra.
They were referenced by direct contact, family
doctors and Oncology Services. Home visits were divided in scheduled and
unscheduled visits according to the needs.

Results

In the first year, the team followed 84 patients. Took place 758 visits and 441 calls. Of the 84 patients, there was 17 hospital
admissions and 32 in emergency services. 63 patients died, 31 of which at her home, 26 in
health institutions and 6 in social institutions.
