4) Once you have completed the CBL, please return to “Page 3 of 5” and click on the “Next” button to continue.

5) Please answer all of the questions below.

6) Click on the “Check Answers” button at the bottom of the page. This will submit your quiz and provide you with the score.

7) You may click on the “Retry” button if needed. After receiving a passing score, please click on the “next page” link.

8) You have now completed your HIPAA Privacy & Security Awareness Training.

Please “Print Certificate” (Recommended Option) or “Print Score Summary” and provide it to your supervisor. To do so please type in your name or identifier and choose an option.

9) If you chose to email your score, you will see the window below. Please note that you will need to contact the Office of HIPAA Privacy & Security at 305-243-5000 to obtain a copy of that email.

10) For “Print Certificate” option, below is the popup page you will see. Please provide it to your supervisor and keep a copy for your records. This is your evidence that you have completed the training.
