ELIGIBILITY

ELIGIBLE EMPLOYEES Employees who are actively employed in a benefit-eligible position are eligible to

be enrolled in Heifer-sponsored benefit plans. You are eligible to participate in

Heifer International’s benefit program on the first of the month following your date

of hire. During the annual Open Enrollment period, coverage becomes effective

the first of the new year (i.e. 01/01/xx), provided the employee has successfully

completed the necessary online enrollment.

ELIGIBLE DEPENDENTS Employees who are enrolled in Heifer-sponsored benefits have the ability to extend

coverage to eligible dependents. NOTE: Dependent coverage cannot be elected

unless the employee is also covered under the plan being selected. Individuals

who are considered to be eligible dependents of Heifer-sponsored benefits plans

are:

Spouses (unless legally separated or divorced)

Domestic Partners (same-sex and opposite-sex)

NOTE: Dependent coverage under this option will require employees to be taxed

for the fair market value of the premiums paid by Heifer; additionally,

completion of the “Affidavit of Domestic Partnership” will be required for each

plan year.

A child you have a Qualified Medical Child Support Order (QMSCO) for

A physically or intellectually disabled child who is incapable of self-

sustaining employment, regardless of age, provided they are covered

prior to the maximum age otherwise applicable

An adult dependent child of an employee is eligible for coverage through the end

of the month in which he/she turns age 26. Eligibility is regardless of whether he/

she qualifies as the employee’s tax dependent, is a full-time student or is married.

NOTE: If the adult dependent child of the employee is married, ONLY the adult

dependent child of the employee will be eligible for coverage.

Information regarding continuation of benefits under COBRA will be distributed

after benefit eligibility terminates. COBRA is administered through Consolidated

Admin Services (CAS).

DUTY TO NOTIFY OF INELIGIBILITY The employee is responsible for making the appropriate updates in the Benefits

Enrollment Portal (www.InfinityHR.com) for any changes that have an affect on

benefits. An enrollee ceases to be a covered dependent of all employee benefit

plans on the date the enrollee no longer meets the definition of a dependent,

regardless of when notice is given to Heifer.

ADDING AND REMOVING

EMPLOYEE OR DEPENDENTS Outside of the Open Enrollment period,

employee and/or dependent coverage

can only be altered if the employee

experiences an IRS qualifying life event

change1. Changes must be made within

31 days of the date of the life event

change. After that time, the employee

must wait until the next Open Enrollment

period. The effective date of a coverage

change, based on an IRS qualifying life

event change, will be the first of the

month following the life event.

(Exception: newborns are covered from

Date of Birth).

The following is a summarized list of IRS-

defined life event changes:

Employee’s legal marital status

changes, due to marriage, divorce,

legal separation, or the death of a

spouse;

A change in the number of

dependents you cover, due to birth,

legal adoption, a Qualified Medical

Child Support Order (QMCSO), or

death;

Changes in employment status of the

employee, such as moving from full-

time to part-time employment;

Changes in employment status of a

spouse, which can include the ending

of their employment, new or different

working hours resulting in a change of

their employer-sponsored benefits, a

benefit change due to a strike, a

benefit change for them due to a

change in work hours, or if they begin

an unpaid leave of absence resulting

in a change of benefits;

A dependent who becomes ineligible

for the benefit plan due to age or by

obtaining coverage via other means.

and provided to all Heifer employees and

their dependents who are enrolled in the

Medical plan at no cost to you. This benefit

allows employees with the ability of

CONSUMER?

What is a generic drug? Generic drugs are “copies” of brand-name drugs

that have exactly the same quality, active ingredients, intended use,

effects, safety, and strength as brand-names.

Why are generic drugs less expensive? Companies that develop

brand-name drugs pay for years of research and development. Once a

brand-name drug is ready, a patent is granted allowing the company an

exclusive right to sell the drug. Introducing the brand-name drug to the

medical community and the general public is expensive. Just watch TV for

a few minutes and you will likely see an ad for a brand-name drug.

Once the patent expires, other companies can apply for permission to

make and sell a generic copy. Generic drugs do not require the research,

development or any of the marketing costs—and those cost savings are

passed on to you, the consumer.

What can I do to switch to generics? If you are looking to switch from a

brand-name drug to a generic, talk with your doctor. Here are a few

questions to ask:

Is there a generic version of my medication?

Are generics right for me?

Can I substitute my brand-name drug with the generic?

Are there any risks of side effects or reactions if I change my

medication?

Does the generic look different than my brand-name medicine?

Are there special instructions I should know about before taking the

generic?

Vision coverage is a voluntary benefit that features

coverage for prescription glasses and contact lenses, as

well as other vision-related items. As with the dental

plan, you have the freedom to receive services from any

vision provider; however, enrolled employees will receive

a higher level of benefits by using providers who

ELIGIBILITY

Because of the tax-advantaged nature of HSAs, there are specific eligibility requirements,

including:

You may not be covered by another non-HDHP health plan (for example, a spouse’s

traditional medical plan that covers you);

You and your spouse may not enroll in a medical Flexible Spending Account that could

reimburse your medical expenses. Participation in a limited Flexible Spending Account

that covers only dental and vision expenses is allowed;

You may not be enrolled in a government health plan, such as Medicare or Medicaid;

Domestic partners covered by the HDHP are not eligible for reimbursement from the HSA;

Children who are not your tax dependent are not eligible for reimbursement from the HSA;

You may not have an HSA and be claimed as a dependent on someone else’s tax return.

HEALTH SAVINGS ACCOUNT (HSA)

If you enroll in a Heifer-sponsored High Deductible Medical Health Plan, you can also

choose to contribute to an HSA on a pre-tax basis through a payroll deduction. Keep in

mind that annual contributions are limited by federal law, depending on the level of

health coverage you elect. For a full list of qualified expenses, visit:

Your HSA works like a savings account. You decide

whether to use the funds to pay for health care

expenses out of your pocket or from your HSA using a

KEEP YOUR RECEIPTS!

Since your HSA is tax-advantaged, it is subject to IRS

regulations and a possible audit. Always be sure to

keep your receipts.

By allowing you to set aside money directly from your paycheck before taxes are taken out, flexible spending accounts (FSAs)

are a great way to save money for eligible expenses and to lower your taxable income. You can use that tax-free money to pay

for eligible out-of-pocket health care and dependent care expenses.

Heifer offers the following FSA options, administered by Consolidated Admin Services (CAS):

HEALTH FSA: Pay for eligible medical, dental, and vision care expenses such as copays, coinsurance, deductibles, medical supplies and

equipment, mental health and substance abuse treatment, orthodontia, and eyeglasses and contact lenses for yourself and

Heifer has made a commitment to your heath by focusing on wellness, which

SHORT TERM DISABILITY Short-Term Disability (STD) Insurance helps protect your
