Commentary

The crude birth rate is one of the main demographic statistics describing fertility. It is calculated as the number of live births in a year per 1,000 population. The denominator is the mid-year estimated resident population (ERP).

The Western Sydney had the highest and the Far West Local Health District the lowest birth rates in NSW in 2010. The birth rate may be underestimated in the Local Health Districts with interstate boundaries due to mothers resident in this areas giving birth interstate.

Commentary

Birth statistics can be reported as the crude birth rate, age specific fertility rate, total fertility rate and general fertility rate. Total fertility rate (TFR), presented here, represents the number of children a female would bear during her lifetime if she experienced current age-specific fertility rates at each age of her reproductive life.

The TRF was the highest in the Western NSW Local Health District and the lowest in the Southern NSW Local Health District in 2010. The TRF results may be underestimated in the Local Health Districts with interstate boundaries due to mothers resident in this areas giving birth interstate.

Notes

Includes total births (live births and stillbirths) to mothers resident in NSW and mothers where the state of residence is missing. Excludes total births to mothers residing in other states giving birth in NSW hospitals. 

Commentary

Between 1997 and 2011, the proportion of total births (including stillbirths and live births) in NSW to women 35 years and older increased from 15.8% to 23.9%, while the proportion to teenage mothers decreased from 4.8% to 3.1%.

Younger and older mothers have an increased risk of medical complications of pregnancy such as hypertension (high blood pressure) and their babies are more likely to be low birth weight, premature or to be stillborn.

Notes

Includes live births to mothers resident in NSW and mothers where the state of residence is missing. Excludes live births to mothers residing in other states giving birth in NSW hospitals. 

Commentary

Birth statistics can be reported as the crude birth rate, age specific fertility rate, total fertility rate and general fertility rate. Age specific fertility rates, presented here, are calculated by dividing the number of live births in each age group by the total female population (in thousands) in each age group.

The teenage fertility rate in NSW in 2011 was 13.3 live births per 1,000 female population aged 15-19 years, down from of 20.0 per 1,000 in 1997. The fertility rate in women aged 35 years and over was 30.0 live births per 1,000 female population in NSW in 2011, up from 19.6 per 1,000 in 1997.

Definitions:

1.1 Spontaneous vaginal birth: The number of selected primipara who have a spontaneous vaginal birth as a percentage of the total number of selected primipara who give birth.

1.2 Induction of labour: The number of selected primipara who undergo induction of labour as a percentage of the total number of selected primipara who give birth.

1.3 Instrumental vaginal birth: The number of selected primipara who undergo an instrumental vaginal birth as a percentage of the total number of selected primipara who give birth.

1.4 Caesarean section: The number of selected primipara undergoing caesarean section birth as a percentage of the total number of selected primipara who give birth.

Selected primipara is a woman 20-34 years of age at the time of giving birth, giving birth for the first

time at greater than 20 weeks gestation; singleton pregnancy; cephalic presentation; and at 370 to 410 weeks gestation ('0' means zero days). 

Types add up to more than 100% because induction of labour can precede an instrumental vaginal birth and a birth by caesarian section. Total NSW (or All LHDs) includes births at home assisted by independent midwives. Data include all mothers who gave birth (stillbirth or live birth) in NSW

3.1 Intact perineum: Total number of selected primipara with an intact perineum or unsutured perineal tear as a percentage of the total number of selected primipara delivering vaginally.

of the perineum for fourth degree tear as a percentage of the total number of selected primipara delivering vaginally.

Selected first time mother (primipara) is a woman 20-34 years of age at the time of giving birth, giving birth for the first time at greater than 20 weeks gestation; singleton pregnancy; cephalic presentation;

and at 370 to 410 weeks gestation ('0' means zero days). A 3rd degree perineal tear is a laceration that extends from the birth canal up to and including the anal sphincter. A 4th degree perineal tear also includes the anal or rectal mucous membrane. Episiotomy is an surgical incision that enlarges the birth canal. See Methods Tab for further information.

Commentary

A baby's birth weight is an important outcome measure of the health of the mother and her care during pregnancy. Low birth weight is defined as less than 2,500 grams. In 2010 in NSW, rates of low birth weight were the lowest (4.1%) in the Southern NSW Local Health District (LHD) and the highest (8.5%) in Mid North Coast LHD.

Any smoking during pregnancy is included. Bayesian smoothing was used to calculate the smoothed percentages and prevalence ratios. Interpretation: '0' result not statistically different than state average, '-' lower than the state average at the 5% level of statistical significance , '--' at the 1% level; '+' greater than the state average at the 5% level of significance, '++' at the 1% level.

Local Government Area boundaries used were defined in 2008.

Commentary

Smoking during pregnancy doubles the risk of having a low-birthweight baby and significantly increases the risk of perinatal mortality, sudden infant death syndrome and other adverse pregnancy outcomes.

Compared to the NSW level set at 100 (to facilitate comparisons) the smoothed  Prevalence Ratio (see Methods) of smoking during pregnancy ranged from 6.4 in Mosman LGA to 407.5 in Central Darling LGA.

Smoking rates in NSW are highest among young adults, people in the lowest socioeconomic groups and Aboriginal people.  The variation in rates of smoking during pregnancy among LGAs reflects the geographic distribution of mothers from these high-risk groups.
