• Pre-existing diseases covered and new joiners covered from day 1

• Maternity covered for first 2 pregnancies- no waiting period

• New born babies covered from birth (new born to be enrolled within 30 days of birth on MyHRportal)

• Pre and post hospitalization covered – 30 day prior admission till 60 days post discharge

• Dental coverage – Rs.5000 (root canal/ extraction/ implant/ bridges/ fillings)

• Maternity Limit is 75K maximum for Normal & C-sec delivery. (Pre/Post not covered in maternity)

• Ambulance services offered for emergency cases only max. upto 4000 per case one way from home to hospital

• Special day care procedures included

• Robotic surgeries to be paid as per normal manual charges of the treatment• Corporate buffer for critical illness for another Rs. 3 Lacs per family (aggregate limit INR 1.5 crs)

parents policy User ID & password, Click on the name of the member & E card option will appear on the left side of the

webpage

For Current Year (2015-16) FHPL E card

NOTE: By default User ID & Passwords are : User ID – Employee’s NSN ID, Password – wil l be shared directly by FHPL through

email. After first t ime log in, Passwords can be changed by clicking on Password change option.

For any password related assistance, write to [email protected]

IMP NOTE: Dependents addition in case of newly married / new born needs to be done by the employee on MyHRportal within 30

days of the event. Accord ingly dependents will get covered in the policy within next 45 days or coverage will not be extended in

current policy.

Parents for new Joiners are covered upon the basis of their details / declaration on FHPL website in next month of

 joining, Premium to be paid by Employee.

Note : There is no refund after employee’s exit from Nokia, in case employee has submitted any claim under Parents

mediclaim policy . Complete annual premium is applicable to an employee if he has filed a claim against the policy.

Compulsory Documents For Reimbursement Process

• Original detailed discharge summary on hospital letterhead

• Original investigation/lab reports along with bill & doctor prescription

• Original Hospital Bill-consolidated & detail breakup of every component on hospital letter head

• Incase of surgical packages - detail breakup of the package

• Medicine bills along with doctor prescriptions

• Claim Form – (https://www.fhpl.net/Forms/UIIC_ClaimForm.pdf) Patient FHPL card copy & Patient Photo ID Proof. In

case of >1 Lac claim, provide Patient’s address proof as well

• MLC Certificate from the hospital in the case of Road traffic accident. Or a certificate from the treating doctor

mentioning that the patient was not under the influence of alcohol at the time of Accident.• Obstetric history (GPLA) in the case of maternity claims

• Payment receipt for the payment done to the hospital either advance/balance/final amount

• In case of Parents if single parent has been declared in the GMP then Death certificate of the expired parent to be

produced along with the claim documents.

• In Dental, proper doctor prescription with separate bill & receipt is required for treatment taken with breakup

• Consultation receipts with Doctor Prescription of same

• Hospital Registration Certificate in case of non network hospital of FHPL
