Part1 Task 3
ICC site at Mattakkuliya
Section of construction site- Building construction area
Name of the persons- Date-
Description of work- Walking around the site while working
Activity Hazards
Walking outside the building area Due to uneven ground
conditions there will befalling accidents occur
There were timber partwith full of nails just
outside the buildingarea.
Possibility of slip andfall because, There wer
some muddy areas withno suitable access to
walk.
Stepping on nails and
sharp objects.
Description of work- When doing excavation activities and earthwork activities
Activity Hazard
Working without using safety shoes Legcan get hit bysharpworkingequipmen
s.
Leg canget bite by any
kind of an animal.
Stepping on nails and
sharp objects.
Any kind of objects ca
fall on to the foot area the workers.
Working in the very near area where excavation machinery are working Accidents can occur duto the machinery
operating large mobile equipment, such as drillers, air hammers, pile drivers, tractors, graders, excavators, earth-moving equipment, and other large
machinery.
Whole-body vibration
Description of work- Arranging the reinforcement for a column onthe 3rd floor on a scaffolding (working at heights)

Activity Hazards
Working at heights (more than 10-15ft from the nearest floor.) Possibility of fallingfrom heights
Working on a scaffolding Possibility of fallingfrom one side (some
sides of the scaffoldingwere not covered in the
site)
Possibility of collapsin
the scaffolding (somescrews of the
scaffoldings were nottightened well enough
the site)
Possibility of falling
objects to the nearest
floor and damage thepeople in that floor(some sides of the
scaffolding were notcovered in the site)
In some scaffoldings, the foot area (where
workers stand) was notbuilt strongly. (Serious
accidents can occur incase of falling)
Some vehicles wereparked near the ground
area, where thescaffoldings were
built.(In case of vehiclehit the scaffoldings,
there will be seriousaccidents)
When fixing the scaffolding Single worker trying toget the parts of the
scaffolding to the top o
the scaffolding to fix itwithout any support orprotection. (In case of a
problem with regardingthe balance, there will
be serious accidents)
Description of work- When doing steel work

Activity Hazards
Working with the machines which have not guarded properly Can damage leg or hanby engaging with the
belt.
Working with damaged welding masks Can be harmful for the
eyes of the workers.Working with equipments which includes damaged wires. Electric shocks can
occur
Using electric equipments without Plug tops. Electric shocks can
occur in case of a loosconnection.
Working around the area where the welding plant wires have not being arrangedproperly.
Therell be fallingaccidents if someones
leg gets tackle with thewires.
And also, therell be
electric shocks occur ifsomeones leg getstackle with the wires.
Working around the area where some electric equipments were kept in some
dangerous places.
Electric shocks or even
burning injuries canoccur if someone steps
into an equipment.
Working around the area where fuel and chemicals are kept nearby to the
welding equipments.
Huge fire situation can
occur if a spark getsnear to that area.
Welding in a closed area welders receiving seve
arc burnsWorkers around thatarea could receive
severe arc burns.
Due to the compressed gass cylinders which are stored near to that particular
area.
hazardous if theresdamage to cylinder
valves, and because of
the risk of explosion orflashback (compressedgas cylinders need to beequipped with reverse
flow check valves toprevent flashback).
Description of work-When working with the electricity.
Activity Hazard
When working with the electricity. Electric shocks can
occur due to Static fromthe rubbing (friction
effects) of plastics and

from clothing made
from wool, wool blendnylons and polyvinyl
Electric shocks canoccur due to Personal
effects (e.g. rings,jewelry, cigarette
lighters, matches,hearing aids, mobile
phones and pagers,transistor radios and
similar);
Huge fire situation can
occur due to using ofgas flame near exposed
electrical conductors (a
flame is a conductor);Huge fire situation canoccur due to Obstacles
to getting the equipmenswitched off;
Huge fire situation canoccur due to lack of
ventilation leading touncomfortable, hot and
humid workingconditions;
Huge fire situation andalso, electric shocks can
occur due to Voltagesbetween live exposed
conductors andsurrounding metal
framework
Electric shocks can
occur due to electrictools and equipment
(e.g. hand lamps, drills
saws, torches and testinstruments);

Description of work-When doing false work
Activity Hazards
heavy, frequent, or awkward lifting Various body injuries including back pain.
Using improper jack andsupporting materials
supporting materials can be fall to the persons whoworking in the site.
Not selecting the proper materialsand systems according to the given
case.
Can collapse the full false work and form work structure.
Description of work-When doing formwork
When walking on the formwok Possibility to slip and fall. Can get collapse
Can get tackle becouse of the gaps between the
scaffoldings.
Can get hurt by stepping on to heequipments on the
scaffolding.
Description of work-When working with multiple source electricity systems (confined areas)
Electrical testing in confined area with explosive gas
mixture, fumes, vapor or dust which is inadequatelyventilated.
Working in situations where condensation, spillage, drainage or seepage occurs and results in wet
surroundings
Pipes, sewers, shafts, ducts, and similar structures.
Voltages between live exposed conductors and
surrounding metal frameworkVoltages between live exposed conductors and thesurrounding environment (including metalwork, damp
situations, other conductive surfaces and persons nearby).
General work activities (e.g. welding, cutting, brazing,
using hand saws, drilling of all types, hammering andchiseling);
Hot metal surfaces due to drilling, grinding welding, etc.

Excavation associated with electrical work;
Description of work-When driving vehicles in the site
Activity Hazards
Construction vehicles entering /
leaving traffic.
Vehicles striking services and obstructions
Site staff crossing live carriageway Can get hit by the running vehicles
Overturning vehicles (due toslpoes, uneven ground)
Workers who are opperating the vehicle can get hurt.
When reversing vehicles Vehicles or their loads striking people,
Unsafe loading and transportation of
materials on vehiclesPeople can get stuck by the vehicle or by the load.
Rough access roads Vehicle can get stuck and therefore the workers who areworking with the vehicle can get hurt.
