Surrender Value: The surrender of the policy within the term of policy postthe free look period. The surrender value is the discretion of the insurer.

Free Look Period: 15 days from the date of issuance of the policy or COI.

Point to Remember: If any customer is COVID infected he or she will not beeligible to accept the policy. But in case post issuance of policy if anycustomer is found positive, and death happened his/her nominee will be paidwith the death benefit.

The page opens where the details of the policy holder is required to be filled. ‘Select product’ ‘Master Policy Holder Name’ and Master Policy no.’ will be auto generated and cannot be changed.

On the 2nd page Application Id will be generated and the VLE needs to keep the Application id saved and it will be valid for 24hrs. On this page the ‘Nominee Details’ needs to be filled

Female questionnaire: In addition to the Sound Heath question Female life to answer the pregnancy question if the answer is ‘YES’ she will not be able to avail the policy. Then ‘Calculate Premium’ button to be clicked at the end of the page.

Claim Process

Basic Documentation for death claims:

• Mail from MPH to initiate the Claim process.•Death certificate issued by the Government / Municipal authority under section 12/17 of the Registration of

Birth and Death Act 1969.•Claim form with NEFT details (personalized cancelled cheque).•Identity and Residence proof of the nominee (with relationship).•Statement of Loan A/c (as per the format mentioned in claim form or as per Annexure VII).•COVID questionnaire form and MEF as attached need to be submitted.

Additional Documentation:

• Cause of death certificate issued by the doctor / hospital.• A copy of medical records for past illnesses / ailments / diseases and for treatments taken at the time of death.• Additional requirements may be raised on case to case basis.
