Decisional Traps and Effective Responses

Use this document as a guide to recognize common decisional traps and implement effective responses to help minimize their impact.

Decisional Traps

Decisional traps get in the way of effective decision making, often introducing significant error to the decision process and diminishing the quality of the decision.

Trap Description

The Juice is Watery The "juice" is slowly getting watered down, but no one realizes it. This snag occurs when people continue along a course of action without a critical analysis of measurable progress against a specific objective. The outcome is that the team puts a lot of work into a plan that does not yield results commensurate with the level of effort.

Short-Term Gain (Long-Term Pain)

A decision is made without thinking through long-term consequences or without coming up with contingencies if the assumptions of the original plan change.

Finger Crossing Gaps of knowledge are bridged with hope.

Herd Mentality Groups generate energy and enthusiasm that can stifle disagreement or healthy conflict. No one wants to speak up and derail the train.

Form Over Function In contrast to the value of form and function defining the end state, the system takes on a life of its own. Its known processes are comfortable and well-understood, and thus, seem to carry little risk. In contrast, decisions that conflict with the system have all kinds of unknowns, carry all kinds of risks, and are much more likely to be scrutinized.

Jumping to Conclusions

The human ability to make sense from fragmented pieces of information represents a powerful capability for increasing understanding. In some cases, however, especially when the context is vague and the situation urgent, the ability to fill in the blanks may result in jumping to conclusions and incorrectly likening current circumstances to previous experience.

Slippery Slope This decisional trap can center on an ethical issue or take the form of scope or mission creep. Before you know it, a line has been crossed.

Effective Responses

When decisional traps become apparent, the following represent ways of countering the flawed thinking:

“We started out making some progress here, then we went to two steps forward, one step back. Now we‟re only taking one step forward and two steps back. Let‟s reevaluate this, and consider a change in tactics.”

War-game second- and third-order effects

“I want to hear from our weather people and analysts—if we implement this plan, what‟s the most likely outcome? What about the most dangerous outcome?”

Challenge assumptions

“OK, let‟s not confuse enthusiasm with capability. What I just heard is that „with any luck‟ we‟ll have it done by nightfall. Are you comfortable allowing hope to be the plan?”

Demand minority opinions

“Dave, be our devil‟s advocate here. Put yourself in the public‟s shoes. What are your thoughts? What are we missing?”

Utilize a 70% solution “We‟re getting in the weeds here. We can‟t over-engineer this, too much is going to change if we try and plan everything out. We have very capable DIVS out there. Let‟s get them our intent and let them do their jobs.”

Validate perceptions “Let‟s not start making a plan based on a rumor. Where did you hear about the trucks being delayed? Let‟s get that verified before we start getting spun up.”

Use team trigger points

Handout 3-2 Agency Administrator’s Briefing

I’ve been asked to facilitate the briefing. The incident consists of an overturned tractor trailer

involving fifteen 300 gallon containers or “totes” full of the pesticide diazinon, plus about 200

gallons of diesel from the truck’s fuel tanks itself. The driver sustained moderate injuries during

the accident and has been transported to a hospital. The exact cause of the accident is under

investigation and your team will not be responsible for that aspect of the incident - though you

need to coordinate site visits by the investigation team. In a few minutes you will receive a more

detailed specific briefing from the Initial Response IC. For now we are going to cover overall

agency direction.

We have made a joint decision to assign the Jefferson County IMT to manage this incident. Joe,

as the IC of the county team you are being delegated to represent Jefferson County in Unified

Command with the ICs from the DEQ and US EPA. For now we feel these are the only

jurisdictions that will be in Unified Command. The State police and highway department will act

as Assisting Agencies and will not be in Unified Command. The county IMT will fill all the

command and general staff positions under the Unified Commanders. The three of us share the

same concerns and priorities. Aside from the obvious health threat of the Diazinon and diesel

fuel, I-265 is a major corridor for commerce and commuting. The longer it is kept closed the

greater the impact to the local area, and this will be a major issue until it is resolved. We

imagine It won’t take long for the elected officials to be fully engaged until the interstate is

opened. The trucking company advised us it is in chapter 11 bankruptcy status and will not fund

any response or cleanup. Because of the potential financial impacts to the government at all

levels, Cost will be a significant concern. Where equally viable options are available, whenever

possible, select the lowest cost options. We have included Management Objectives and

priorities with the Letter of Delegation. You will need to formulate the appropriate operational

objectives to safely and efficiently mitigate the incident. Please inform us once you have

established your incident objectives. One item we ask you to consider is that you make every

attempt to open the highway by the afternoon commute time.

There are no major policy issues between the three lead agencies. The regulations and policies

covering response operations are compatible enough that we do not anticipate difficulties. There

policies are compatible. The primary stakeholders are the residences and businesses in the

local area. They are currently being notified. Water quality may be also be at risk. The

secondary stakeholders are the commuters and others that use the highway. We are attempting

to get the word out over the local radios, but you will need to follow up. Our priorities are: life

safety of the public and responders, property protection, environmental protection, and

recovery.

We expect you to assume Command at 1000hrs today. All necessary maps and aerial photos

can be obtained from the County GIS Office. They are already staffed and printing products.

They will deliver them to the Incident Command Post when they are completed. The help of

several local and state responders in this area will be needed for at least a couple days. This is

a significant factor. The county is making all their resources available. EPA is mobilizing their

analytical and remediation contractors from their local offices.

The media is just getting up to speed on the incident. They have started to arrive at the scene

and place their phone calls to us. You will need to get your Public Information Officer function up

and running right away. The county Emergency Operations Center has been activated. All local

and mutual aid resources should be ordered through them. State and Federal resources will be

coordinated by those agencies, but we want to place all orders through the Emergency

Operations Center to reduce duplication and maintain control of the ordering process. If you

don’t think the current facilities are adequate, look for some alternates, but run them by the

County Administrator first. That’s all we have for now.

(Instructor Use Only) Joe says in the interview before the initial strategy meeting that the following will be determined in this meeting: establish what their planning process is going to be; when the next meeting is going to occur and what each member has to do to get ready for the, to prepare that lAP for the next operational period. Attendees: Joe, Ben and John - UCs, Rich - OSC, Geoff— PSC; Jon — SOFR; PlO; LNO; LSC; FSC IC: OK let’s get started. Since we are using the Jefferson County MT Ben and John asked me to facilitate this meeting. So what would like to do is start out by looking at the incident objectives we have posted on the wall. Those are the unified incident objectives that the 3 of us came up with at our unified commanders meeting. Rich you had a chance to go over those right? OSC: have had a chance to go over them and all of it looks good to me. The only exception that I’ve got is with the objective therefore of opening 1265 by 1600 this afternoon. I’m not sure that we can get that done. We do have some tactics in place to try and make it happen but just want you to understand that I do have some concerns about it. IC: OK. Well we know that is a high priority for the Agency Administrator, but of course we do not want to do that until it can be done safely. So I am glad that you are taking a look at that. OCS: Understood IC: While we are on you, do you want to talk about the strategies we discussed? OSC: Yea, as far as the chemical goes, essentially we are looking at a containment strategy. Just trying to hold it where it is. Use some vacuum trucks and such to pull the chemical back out and may be into a different transport vehicle or something of that nature. For the general area we are really looking at an isolation strategy. We will get some roadblocks set up and make sure that no one is coming into the area. Along those lines we may end up doing some evacuations for people that are already inside the incident perimeter. I do have a question for the environmental side that there is some flammability characteristics associated with this chemical. My question to you is if we do have ignition is there a potential that we can just go ahead and let it burn out? UCs: Nods Yes OSC: Ok great thank you, so we’re good then. complete with all required attachments. Each rate covers all costs eligible under the Robert T. Stafford Disaster Relief

and Emergency Assistance Act, 42 U.S.C. § 5121, et seq., for ownership and operation of equipment, including

depreciation, overhead, all maintenance, field repairs, fuel, lubricants, tires, OSHA equipment and other costs incidental

to operation. Standby equipment costs are not eligible.

7 SOFR LE STL One of the Officers assigned to Perimeter Security slipped and cut his hand on barrier. We need to have him evacuated for medical attention. Not too serious but probably needs stitches. All our vehicles are tied up; we need an ambulance at the NW ramp of Highway 71 and 265.

At this time it is unclear how this response will be funded or reimbursed. We are attempting to sort that out now. There may be different funding sources depending on the specific type of response.
