Student Name: ____________________________________________________________________________ Student Number: ____________________________________________________________________________ This Course Request Form (CRF) allows a student and his/her family to make course selections for the following year. All Grade 7 students are required to take the following core courses: English, math, science, social studies, and health and physical education. Additional core courses may also be required based upon a specific program in which a student is enrolled. Descriptions of all courses can be found in the Middle School Program of Studies. Select only ONE overall program by checking the appropriate box. Grade 7 Program:

High Intensity Language Training Program:
Students enrolled in the High Intensity Language Training Program (HILT) are grouped by English language proficiency into HILT A, HILT B, HILTEX A and HILTEX B. Recommendations for math courses are made by staff. Parents will be notified of the recommendations before a final assignment is made.

ACT II - provides students an opportunity to take additional electives after school, courses are offered based on student interest. Attendance for ACTII courses are required and classwork is graded. See the full description of ACT II in the Middle School Program of Studies. ACT II Classes meet twice weekly Mondays, Tuesdays or Thursdays from 2:35-4:15PM.
