Guitar Lesson Agreement Please read fully and print your information

This is an agreement for Patrick Franklin ____________

To provide guitar lessons to _______________________________________________

Each Lesson will last for __30__minutes

The fee will be _$75 per month

The fee can be altered by the teacher providing _ 30_ days notice to the student in writing

The lessons will be held at ___Coosada Baptist Church____

The lessons will be held from __________ to ___________ weekly

Payment Terms

Payment for monthly lessons should be made before or on the first lesson of the month

A discounted fee of ___$50_ per month is charged for a 6 month commitment

with two(2) payments of ___$150

The first payment due _________ and the second payment due _________

Payment should be cash, or check made payable to Patrick Franklin

Missed Lessons

Lessons cancelled by student can be rescheduled providing that notice is given four (4) hours prior to the missed class

If notice of cancellation is not received, and the student is unable to attend the lesson for any reason whatsoever, the lesson fee will be forfeited no matter

what the circumstance. To cancel a lesson the student should telephone ________________

Lessons will always begin and end at the arranged time regardless of the student’s arrival time. No reduced fee will be given for tardiness.

If the teacher cancels a lesson, the lesson will be made up at a mutually convenient time.

Other Requirements

Student must have their own guitar, pics, and other accessories needed for class

Student must have a 3 ring binder with clear plastic Avery sleeves. These can be bought at most stores, including Wal-Mart and Office Depot.
