IMPORTANT INFORMATIONAPPLICABLE TO ALL BENEFITS

Applying to joinServing officers can apply to join thescheme at any time by completing amedical underwriting application formwhich is available from the FederationOffice. New student officer recruits tothe police service may join the schemewithout the need for the completion ofthe medical underwriting applicationform and their first 104 weeks ofservice are free of charge. TheFederation and/or Philip Williams & Coreserve the right to decline anyapplications.

Subscription collectionSubscriptions are collected monthlyby deduction direct fromsalary/pension unless that is notpossible when alternativearrangements may be agreed.Monthly subscription payments mustbe maintained in order to remain amember of the schemeand to qualify for any benefits.

InsurersA list of the insurers is available onrequest. Full policy wordings forcertain policy sections are available todownload via QR codes. These mayalso be available at the FederationOffice, on the Federation Website orby visiting the Group Scheme sectionof www.philipwilliams.co.uk

How to cancel your coverIn the event that you need to cancelyour cover please contact theFederation Office.

Cohabiting Partner ExtensionsAny cohabiting partner extension willcease when the member orcohabiting partner reach 70 years ofage. Any cohabiting partner coverand/or extension will cease when themember ceases to be a memberof the scheme.

Career breaks, maternityleave, secondment or livingoverseasThose going on a career break,maternity leave, secondment or areliving overseas must contact theFederation Office to identify if covercan be maintained.

Retirement from

the Police ServiceServing officers upon retirement mayremain in the scheme as a retiredmember by submitting a completedmembership continuation form to theFederation Office priorto retirement.

Individuals are not eligible to join thescheme after their retirement date.

Transfer, resignationor dismissalServing officers who transfer, resignor are dismissed from the policeservice are not eligible to remain inthe scheme and all membership andbenefits will cease including anycohabiting partner extension.

Complaints procedureThe Federation Insurance Scheme is arranged on behalf of the trusteesby Philip Williams and Company who are authorised and regulated by theFinancial Conduct Authority (Registration Number 308860). The trusteesare responsible for organising the policies and dealing with the insurancebroker. Any complaints about any aspect of the scheme should in the firstinstance be directed to the Federation Office. The insurance broker will thenbe asked to investigate the complaint and resolve any matter either via theFederation Office, directly with the member, or through the appropriateunderwriting organisation.

This booklet is a summary only. The benefits are subject to certain exclusions and policy conditions notstated in this booklet which in the event of a claim will be applicable. The benefits are provided strictly underthe terms of the insurance policies taken out by the trustees of the scheme. Subscription to the schemeentitles the member to the benefits provided by the scheme but confers no ownership of any of theunderlying policies which are vested in the trustees. The trustees retain the full policy wording detailing allbenefits and exclusions which may be viewed on request to the Federation Office.

EXPLANATION OF BENEFITS

Life insuranceOn death of a member or cohabiting partner who arecovered under the scheme the cash benefit detailedin the current benefits table will be paid. The policy iswritten in Trust so that if a member dies, the proceedscan be paid, by the Trustees, to the member'sdependants quickly free of tax and without havingto wait for probate. Members should ensure that theyhave an up-to-date beneficiary nominated and havelodged the details with the Federation Office, to assistthe Trustees in the event of a claim. If a member aged68 or under receives a terminal prognosis of 12months or less, they may apply to the Trustees foran advance of the death benefit of 49% of therelevant sum insured.

Child Death GrantThis benefit is paid upon the death of a dependent childof a member, aged between 3 months and 17 years.

Permanent total disablement (any occupation)Payable when a member is unable to performany gainful employment and is unable to existindependently, requiring continual supervision foractivities of daily living, as a result of an accident.

Accidental Loss of Use and Permanent DisablingInjuries Benefit Variable Benefits paid in the event of:- - Loss of sight in one or both eyes- Loss of use of one or more limbs- Total loss of hearing in one or both ears- Temporary Total Disablement (14 day excess)- Other injuries of varying severity

Unsociable Hours BenefitIf, as a result of a period of Temporary TotalDisablement, a Benefit Participant suffers a lossof Unsociable Working Hours Pay, we will pay 75%of the Unsociable Working Hours Pay that wouldotherwise have been received, up to the followingmaximum benefits:

Constables: £60 per week. Sergeants: £75 per week. Inspectors and Chief Inspectors: £95 per week.

This benefit is payable for up to 24 weeks and excludesthe first two weeks.

Dependent Childcare CostsWe will pay up to the benefit shown below, if, due tounforeseen operational circumstances and with lessthan 15 days’ notice a beneficiaries work commitmentsare altered such that additional dependant care costsare necessarily incurred.

The benefit will be £15 for each full hour that theadditional dependant care costs are payable, up to amaximum of £200 for any one claim (maximum £1,000during any one Period of Insurance).

On-Duty Assault benefit as a result offirearms or knivesIf a serving police officer sustains accidental bodilyinjury in the course of duty directly caused by thedischarge of either firearms or shotguns or caused byassault involving a knife and as a consequence of theinjuries is unable to work for a period of at least 14consecutive days immediately after the attack thepolicy will pay: (a) £1,500 as a result of Firearm or Shotgun injuries (b) £750 as a result of knife injuries

Hospitalisation benefit- Payable when admitted as an in-patient to hospital in the UK between midnight and 07.00- Unplanned admission arising from accident or emergency, payable from first night- Planned admission payable after three nights- Benefit payable for up to 7 consecutive nights

Sick Pay BenefitIf a member suffers a pay cut under regulations orterms of employment, the benefits illustrated on thescheme benefits table will become payable after 26weeks absence for up to 26 weeks.

Please note: If full pay is reinstated and back dated,the benefits received during the applicable periodMUST be repaid.

Infection of HIV/Hepatitis B on dutyCover is provided for serving officers only. If, as a resultof a documented duty related incident you becomeinfected with the HIV/AIDS virus, a benefit paymentwill be made.

Unrecovered Criminal Court CompensationCover is provided for serving officers only. If, as a resultof an on-duty assault, compensation you have beenawarded by the Court has not been paid within sixmonths from the payment date of the award a benefitpayment will be made.

The scheme benefits will be payable if a member, member's partner (if additional subscription paid)or their child, aged from 30 days to 17 years, suffers from an insured illness and survives for morethan 14 days from the date of diagnosis or surgery.

This policy covers the member, their cohabitingpartner and any number of their unmarrieddependent children under 22 years, all normallyresident in the family home, for any number oftrips in any year up to 60 days per trip. It coverstravel worldwide and also in the United Kingdom.

The main sections of cover are:- Cancellation and curtailment up to £5,000- Emergency medical expenses up to £10,000,000- Personal Baggage up to £2,000- Personal Money up to £500- Public liability up to £2,000,000- Personal Accident up to £20,000.

Main conditions and exclusionsThe policy will not cover you if any of the following applyto you, a travelling companion, relative, businesscolleague or non-traveller at the policy start date andeach time you make arrangements for a trip.

1. You/they were aware of any reason why the trip couldbe cancelled or curtailed.2. You/they were travelling against the advice of amedical practitioner or in order to get medicaltreatment abroad.3. You/they have been diagnosed as having aterminal illness4. You/they are on a waiting list for surgery, treatment,or investigation at a hospital or are awaiting the resultsof hospital tests or investigations.

If there is any change in a person’s health between thedate the policy is issued and the start date of a trip youmust still comply with the important conditions relatingto health. If you are unsure please notify VoyagerInsurance on 01483 806816 immediately.

An excess of £50 is applicable for most policy sections.The excess is payable per person, per section, perinsured incident subject to a maximum of £100.

There are significant limitations and exclusions of coverfor property, including valuables and money, that is leftunattended or out of your immediate control andsupervision. How to make a claim If the vehicle you or your cohabiting partner aretravelling in, suffers a breakdown, please call our 24hour Control Centre on

telephone +44(0) 1737 815 406You should have the following information available. - The vehicle's registration number - Your name, home postcode and contact details - The make, model and colour of the vehicle - The location of the vehicle - An idea of what the problem is - An SOS box number (if this applies).

All use of this service is available for the coveredmember and their partner living in the family home.Any claim will be validated with Philip Williams andCompany, or other such administrative organisation,with costs for any claims from non-eligible personsbeing the responsibility of the claimant.

The vehicle must be permanently registered in the UKand, if appropriate, have a current MOT certificate andvalid road fund licence. The vehicle should be kept in agood condition and have been serviced regularly in linewith the manufacturer's recommendations. It must beless than 16 years old since first registration (11 yearssince first registration if you want help within Europe).

Cover is provided 24 hours a day, 365 days a year:

Insured eventsCover is provided for domestic emergencies for thefollowing insured events:- Plumbing and Drainage- Failure of internal electrics- Security (i.e. keys, glazing & locks). - Pests - Gas supply pipe - Boiler and heating system - Roofing

EmergencyThe result of a sudden and unforeseen incident at thehome which immediately: a) Exposes you or a third party to a risk to yours or theirhealth or; b) Creates a risk of loss of or damage to the homeand/or any of your belongings or; c) Renders the home uninhabitable

Claims Helpline 0344 701 3953Please have as much information as possible to handincluding your policy reference 10034/60027, or as aminimum, the name of the Police Federation to enableus to assist you as quickly as possible.

What will happen next: If you suffer an emergency at your home, you should tellus on the emergency telephone number. We will then: - Advise you how to protect yourself and your home immediately; - Arrange for one of our authorised suppliers to get in touch with you to make an appointment or to settle your claim on a reimbursement basis;

- We, along with our authorised suppliers under our delegated authority, will then manage your claim from that point onwards and keep you updated throughout your claim journey; - We will organise and pay up to £500 per claim inclusive of VAT, for call out, labour, parts and materials to carry out an emergency repair; - In the event of your home becoming uninhabitable and remaining so because of a covered event, we will contribute up to £100 inc VAT towards the cost of your (including your pets) alternative accommodation including transport, on a reimbursement basis; - Once we have carried out an emergency repair and contained the emergency for you, we would always recommend that you arrange for a permanent repair to be completed by a qualified tradesperson as soon as possible. In many cases the emergency repair will only provide a temporary solution to the problem.

Claims under this policy can only be made by you,your immediate family, lodger or anyone calling onyour behalf.

If the emergency repair is going to cost more than the£500 limit inclusive of VAT, we will require you tocontribute the difference before we complete theemergency repair.

Subject to our prior agreement and on receipt of yourcontractor’s fully itemised and paid invoice, we wouldpay you up to £500 inclusive of VAT as a contribution toa repair, which you arrange yourself, taking into accountcosts already reasonably incurred by our authorisedsupplier, for the initial visit. Any costs already incurred byour authorised supplier will be added to any costsincurred by your own contractor to determine whetherthe £500 inclusive of VAT policy limit has been reached.

This policy provides immediate assistance in the event of a domestic emergency. Home emergenciescan be stressful and sometimes difficult to resolve, but with Home Emergency Assistance you willhave peace of mind knowing that we are with you every step of the way, by not only appointing asuitably qualified contractor to attend your home but also meeting the costs associated with this.
