You could qualify for Hoosier Healthwise if:

• You are a child under the age of 19 living in a low-income household.

• You qualify for Temporary Assistance for Needy Families (TANF) and/or food stamps (SNAP).

• You are a pregnant women not already enrolled in or eligible for the Healthy Indiana Plan.

You could qualify for HIP if you meet all of the following requirements:

• You are between the ages of 19–64. • You are not eligible for other Indiana Health

Coverage Programs or Medicare. • You are a pregnant woman.

How do I apply?

Contact an enrollment center near you to help complete your application. The central Indiana locations are listed inside this brochure. To apply online, go to in.gov/fssa/dfr/2999.htm and click on your county.

Take with you to the enrollment center the following documents:

• Social Security numbers for all applicants. • Proof of identification: driver’s license, state-issued ID or school ID. • Proof of U.S. citizenship/residency: birth certificate, U.S. passport or

Alien Registration card. • Proof of all income: pay stubs, child support, employer statements,

unemployment, disability payments, income tax forms. • Proof of private health insurance: insurance card or policy number

(if applicable). • Proof of pregnancy: written statement from medical professional

(if applying for pregnancy coverage).
