RATIONAL

The Chronic Lymphatic Leukemia (LLC) is the most frequent leukemic form in the Western world, and its incidence is significantly higher in elderly. Although in the past the changes in clinical attitudes have developed very slowly, mainly due to the lack of novelty on the knowledge of the biology of the neoplastic cell, in the last two decades we are witnessing a progressive and rapid increase in knowledge about this disease. This evolution of knowledge has rapidly transformed a relatively homogeneous and lethal disease into a heterogeneous haemopathy that can be controlled for even very long periods, from the moment it needs to be treated, thanks to the new drugs we are now in our armamentarium. In fact, for many years chemotherapy was the only possibility of treatment of these neoplasms until the introduction of monoclonal antibodies changed the scenario. With the most recent biological acquisitions on the mechanism through which the disease develops it was possible to prepare new agents capable to focus on the neoplastic cells in a targeted manner. In this exciting scenario, in continuous movement, agents capable of inhibiting the intracellular signaling of the B-cell receptor (BCR) are inserted, drugs that mimic the activity of the physiological antagonists of Bcl2 in order to restart the mechanism of apoptosis, and new monoclonal antibodies to CD20. Given these premises, a moment of recapitulation of the multiple changes that led to the pathogenesis of CLL, of classification, of risk stratification to reflect on complex decisions about patient management appears more than necessary. To do this, the meeting will try to address in the same context both the “basic” research on the disease (role of genetics, BCR and interactions with the microenvironment) and the clinical dynamics (prognostic and predictive factors, role of chemo-immunotherapy and therapies target) proposed in synthetic form by national and international experts of the subject favoring the interaction with assistants in training, expert hematologists and researchers in order to update the state of the art on this haemopathy.
