*Self reported at voucher referral and 30 day post voucher use.
Sample size 148 individuals received vouchers and had been hospitalized within 30 days prior to receiving
vouchers. 17 individuals were re-admitted to within 30 days after voucher use.

BCOFA Transportation Services
BCOFA contracts with BC Public Transit:
1. OFA Mini Bus: 60+ and have a valid OFA ID card; covers urban areas of Broome County Advanced reservations required Funding used: CSE (State Funding & Broome County Funding) ; Title IIIB (Federal Funding); Client contributions (cover about 16% transportation spending) 2018: 547 clients; 23,157 rides
2. BC Lift Bus: Riders with an ADA card; OFA uses this service to transport clients to two Social Adult Daycare sites in Broome County Funding used: Title IIIE; State Transportation; MLTC contracts; Client contributions 2018: 20 clients; 930 rides
3. Shopper Bus: 60+ with a valid OFA ID card; residents of 9 senior housing buildings; once a week shopping trip to grocery stores; no reservation needed Funding used: same as OFA mini Bus 2018: 120 clients; 2121 rides
Challenges Transportation needs/questions continue to be one of the top 5 reasons
people call BCOFA
2018: close to 15% of NY Connects calls were about transportation
Current Challenges:
Travel outside of service area & within rural areas
“Convenience” of public transportation (access issues; hours of operation vs medical appointment times)
Cost of “door-through-door” transportation options
Lack of affordable “same day” transportation

MOU in place from March – December, 2018
Goal: Promote Getthere including Connection to Care Program through
outreach to older adults & disabled individuals residing in rural areas of
Broome County
Getthere: provided training and support to BCOFA outreach staff including promotional materials to be distributed at outreach events/locations
BCOFA: - hired OFA staff experienced in providing outreach to older adults - researched rural locations to reach older/disabled individuals - monitored outreach staff (hours worked; mileage)
Connection to Care Outreach Project
Locations visited: senior centers, senior clubs, food pantries, food truck locations,
senior housing, dentists, physical therapy places, churches, libraries, town halls, service
organizations, grocery stores, primary care sites, car repair locations, nursing homes,
pharmacies, shelters and many more
Drop off sites versus Presentation sites
Outcomes: distributed about 7000 Connection to Care postcards
presented to 1155 people throughout Broome County
Lessons learned
More locations were willing to take promotional materials (drop off sites) versus
agreeing to a full presentation about CTC
Reaching low income individuals: food trucks, food pantries, community meals,
shelters, case managers, churches
Doctor’s offices: are you talking to the right person who will help you promote the
service?
Pharmacies: agreed to insert the CTC brochure to their medications’ delivery bags
Outreach staff: creative in finding outreach locations; comfortable with connecting
with people; friendly; approachable;

Fixed route bus system with wheelchair lift for shopping assistance
and medical services serving residents 60 and over and disabled
adults, operating 3 days per week (Wed. Thurs. & Fri.) run by OFA
Funded through Title IIIB, County match and client
contributions; vehicle replacement funded through Delaware
County Senior Council fundraising efforts & grants
Demand Response service, a volunteer driver system for Non-
emergency Medical Transportation for residents 60 years of age
older who are NOT ON MEDICAID, (Current pool = 11 volunteers),
operating 5 days per week
Funded through CSE, client contributions and grant from Rural
Health Network of South Central New York to recruit and
expand volunteer pool

Challenges
No public transportation options within the county
Limited vehicles for bus service
OFA bus service has one 18 passenger bus in fleet (Use ARC buses or VA bus as backup when OFA bus is down, so dependent on their vehicle availability)
Very small volunteer pool base for Non-Medicaid transportation service, not enough to meet demand
Non-handicap accessible vehicles for demand response medical transportation
Demand Response service for medical needs requires client to be able to self transfer and assistive device to fold up and be stowed in vehicle.
Increased demand for NEMT
Transportation #1 issue in Delaware County for accessing services and daily needs
