A total of (80) cubic yards of Stoneway mix 458374c was placed. One set of (5) 4x8” lab-cure cylinders was cast to represent this placement with an expected compressive strength of 4000 psi at (28) days. Reinforcing Steel Inspected Level 2 lower elevator deck between grid lines E.4-F.8/11.6-12.2. Reinforcement was inspected for bar size, grade, lap splice and spacing per RFI 248.12, 4/S514 and 13/S549. Verify the following prior to placement: Install horizontals per detail 4/S514 (#4 at 12”). GENERAL INFORMATION

General Inspection

Concrete PlacementA concrete placement observation was delayed from 8:00 a.m. to 10:30 a.m. today for the Level 11 slab-on-metal deck at grid lines A.7-D.4/11.4-13.6 and (3) AHU curbs on Level 11 at grid lines D.4/12, E.5/11.8 and G/11.8. The reinforcing steel and final clearances were approved previously. The concrete was placed via line pump and mechanically consolidated into the elements. A total of (80) cubic yards of Stoneway mix 458374c was placed. One set of (5) 4x8” lab-cure cylinders was cast to represent this placement with an expected compressive strength of 4000 psi at (28) days.

Reinforcing SteelInspected Level 2 lower elevator deck between grid lines E.4-F.8/11.6-12.2. Reinforcement was inspected for bar size, grade, lap splice and spacing per RFI 248.12, 4/S514 and 13/S549. Verify the following prior to placement:

• Install horizontals per detail 4/S514 (#4 at 12”).• Correct DBA spacing per RFI 248.12 and detail 13/S549. Complete.

Performed a scheduled reinforcing steel inspection of the first lift of Level 1 waler beam from grid lines A-A.2/9-11 and A-A.7/11 and A-A.2/9 for bar size, grade, spacing and splice in accordance with schedule S002, S201C, 11/S239, 10/S624 and S627. Items needed prior to placement:

1. Finish installing (3) #9 bars with hook at line 11 on far west side, of top of A-line wall, per detail 10/S624. Not completed.

Performed an unscheduled reinforcing steel inspection of Level 11 AHU curbs at grid lines D.4/12 for bar size, grade, spacing and splice in accordance with schedule S002, S211C and S515. The reinforcing steel was per plan, clearances were verified, and concrete was placed immediately after.

Continued reinforcing steel inspection of the AHU curbs on Level 11 at grid lines E.5/11.8 and G/11.8 for bar size, grade, spacing and splice in accordance with schedule S002, S211C and S515. Items needed prior to placement:

1. Install #5 top horizontal bars in all curbs, per detail 13/S515. Completed.2. Final clearances. Completed.

Continued reinforcing steel inspection of the Level 11 slab-on-metal deck at grid lines A.7-D.4/11.4-13.6 for bar size, grade, spacing and splice in accordance with schedule S002, S211C, S539 and S540. Items needed prior to placement:

1. Install trim reinforcement around any blockouts or penetrations. Completed.2. Clean loose wood and debris off deck. Completed.3. Final clearances. Completed.

Post-Installed Anchors, Level 11An unscheduled epoxy inspection took place at approximately 9:00 a.m., nearly (2) hours before concrete placement started on the Level 11 slab-on-metal deck at grid lines A.7-D.4/11.4-13.6. It was identified that (5) #4 construction joint dowels were missing, and (3) #4 construction joint dowels had been cut to where they only developed 2” to 3” into the slab area that was about to be placed. The construction joint location on Level 11 at grid lines D.4/13-13.6 was where these missing construction joint dowels were identified. Clark Lewis QC was present during the inspection, and the inspection was based on a similar condition in RFI 4222 that was approved by the engineer on 06/25/21 for a horizontal embedment of 7” that was observed.

The Clark Lewis QC authorized a horizontal embedment of 8”, based on the similar condition that was in RFI 4222. The contractor installed #4 candy-cane dowels at 12” on center, with the hooked end of the bar developing into the slab that was being placed. The holes were checked for cleanliness prior to installation. The epoxy used was Hilti HIT-RE 500 (lot no. 1825 17 L3, expiration 02/2022). See photos below for visual observation.

Structural Masonry

Level 1J&S Masonry started laying 8” CMU block on Level 1 at grid line D.0-G.0/13.0-13.3 from 0’0” to 2’0”. J&S Masonry mixed Mutual Materials Professional Grade Type S mortar on site per manufacturer’s recommendations, and re-tempered as needed on site and used in laying of 8x8x16” CMU blocks for masonry walls. Mortar joints were per plan and the fins were acceptable; the joints were struck off and within specified tolerances of MSJC ACI 530-13. Reinforcement was inspected for size, grade, lap splice and spacing per schedule S522 and S521.

Seismic ClipsObserved the installation of 1/2” Hilti KB-TZ wedge anchors horizontally installed in place of missing anchor bolts in masonry wall at Level B2 along grid lines S-U.8/12. Anchors were 6” in length achieving roughly (6) 3” effective embedment couple locations. RFI 3373, reviewed by MKA, instructs the installation of a third 1/2” Hilti KB-TZ with 3-1/4” of embedment. Verified what was installed met RFI and manufacturer’s specification.

• The embedment for (5) anchors ran into reinforcement. Clark is seeking approval for shallow embedment. Shallow embedment approved RFI 4275.

• (2) still need to be installed at grid lines U.5-U.8/12. Complete.

Observed the following conditions concerning seismic clips for top of masonry walls:

• Level B1 along grid lines U-U.9/14, missing seismic clips – (3) appear to meet specifications, and (3) have at least a 1/4” gap between wall and clip with what appears to be shallow embed (no access to see the anchor designation).

Phase 2

Concrete PlacementA concrete placement observation started at 8:00 a.m. today for the basement wall in Sector A on Level B2 from the top of the footing to the top of wall from grid lines A/17-z.0 and z.0/A-A.3. The reinforcing steel and final clearances were approved previously. The concrete was placed via boom pump and mechanically consolidated into the elements. A total of (96) cubic yards of Stoneway mix 665174 was placed. Five (5) 4x8” lab-cure cylinders were cast to represent this placement with an expected compressive strength of 6000 psi at (28) days.

Reinforcing SteelPerformed a reinforcing steel inspection for the helix truck ramp slab located at grid lines z8.0/y21.5-y23.0 to z2.0-z7.0/y19.0 and Beams B6, B10, B11 and B12. Reinforcing steel for these items is installed per the approved plans and documents with the following exceptions. Verified bar size, grade, spacing, clearances and lap lengths. Items to be verified prior to concrete placement:

1. The reinforcing steel at the construction joint is not complete.2. The west end of the B6 beam still has debris in it.

Performed a scheduled reinforcing steel inspection of (3) columns in Sector A at grid lines y5.8/z9, B.8/18.2 and A.3/19.2. The elements were inspected for bar size, grade, spacing and splice in accordance with schedule S2B2A, S509 and S002. The columns are in conformance with the approved construction documents, and final clearances will be verified prior to placement.

Performed a scheduled reinforcing steel inspection of the Sector A podium shear wall and connected concrete pier at grid lines B.4/17.1-18.2. The element was inspected for bar size, grade, spacing and splice in accordance with schedule S002, S509, S2B2A, S514, 9/S307 and 5/S311. The following items are needed prior to placement:

Continued reinforcing steel inspection for the following Zone B Level 1 Beams B5, B6, B10, B11 and B12. Reinforcing steel is installed per approved plans and documents. The beams have clearance issues throughout. Beam B6 and B12 have vertical construction joints. Note #6 on detail 7/S513 states that all construction joints in beams need to be authorized by the engineer-of-record prior to placement of forms. These (2) vertical construction joints have not been authorized at this time. This has been brought to the attention of Clark and Lease Crutcher Lewis.

Continued reinforcing steel inspection for the Level 1 type B5, B9 and B17 beams that intersect with the core wall at grid lines y19.3/z7.0, and the type B19 beam that intersects with the core at grid lines y19.3/z12.0. This inspection only includes where these beams connect to the core so Lewis can place some form work. The remaining lengths of these beams still need inspection. The intersections have the correct bar counts, bar size and grade of steel.

Continued reinforcing steel inspection for Level 1 B3 beam at grid lines L.7/17-z.0. Reinforcement was inspected for bar size, grade, lap splice and spacing per schedule S201B, S201BRT, S201BRB, 13/S332, S513 and RFI 3815 and 2810.1. Will continuously verify final clearances.
