1.0 INTRODUCTION This Monthly Progress Report is submitted in accordance with the ADMINISTRATIVE ORDER ON CONSENT (AOC) FOR REMOVAL ACTION Proceeding Under Sections 104, 106(a), 107, and 122 of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA), as amended, 42 U.S.C. SS 9604, 9606(a), 9607, and 9622 (U.S. EPA Docket No.: V-W-'03-C-747) effective July 31, 2003. The next monthly Progress Report, for the month of September 2004, will be submitted on or before October 15, 2004.

2.0 SIGNIFICANT DEVELOPMENTS IN THIS MONTH • Air monitoring has continued with the excavation activities. Final validated results of the air

monitoring program for August 2004 can be found in Table 1.1a (polyurethane foam (PUF) samples) and 1.1b (total suspended particulate (TSP) Groups 1, 2, and 3 results). The locations of the air monitoring stations are shown on Figure 1.

• Excavation of soil, sediment, and loose rock within the stream channel and along the stream banks in Parcels 3, 205, 4, 5, 6, 7, 8, 10, 11, and 12 (except for the Quarry area) has been completed. Final validated results from previous confirmation sampling events are presented on Figures 2 through 8. Restoration of these parcels began in August 2004.

• The revised Site Source Control (SSC) Work Plan: Addendum No. 1 was approved by the U.S. EPA on April 21, 2004. The cast-in place concrete foundation and containment walls for the collection system were installed in Trench 1 (Parcel 3) and Trench 3 (Parcel 205). The 6-inch HDPE underdrain system, including cleanouts, was installed. A layer of gravel was placed over the underdrain system and overlain by filter fabric. A geogrid was placed along the rock sidewalls and keyed into the gravel layer. Compacted backfill was placed over the gravel layer between the existing outfall and Containment Wall #2. Construction of the containment inlet structure at Outfall 002, storm water drainage headwalls, and associated piping is complete. Force main and electrical conduit piping from Wet Wells #1 and #2 have been installed from the wet wells to Outfall 002 area.

• The force main and electrical conduit piping from Wet Well #3 (AOI 4) have been installed to Outfall 002 area.

• Components for the two modular million gallon tanks have been received on-Site. The upper (westernmost) modular tank is installed. Construction of the tank pad for the lower (easternmost) modular tank continues.

• Components for the carbon treatment system have been received on-Site.

• Consistent with Upstream Parcels Removal Action Work Plan: Addendum No. 3, removal of materials on Parcel 205 is complete. Validated data for Parcel 205 is presented on Figure 8.

• Excavation activities on Parcel 401 are complete. Figure 9 presents the final validated results. Backfill placement, topsoil, and hydroseeding are complete. Garage replacement on Parcel 401 is complete. Restoration of the area around the garage is complete. Fence installation between Parcel 401 and GM property began.

• Upstream Parcel Removal Action Work Plan: Addendum No. 5, regarding the quarry on Parcels 11 and 12, was submitted to the U.S. EPA and IDEM for approval on August 11, 2004.

• Excavation on the area north of AOI 4 is ongoing. During August 2004, a total of 3,757.95 tons of waste material has been removed and disposed both of at Heritage’s Roachdale Facility and Republic’s Sycamore Ridge Facility. Floor soil samples (used for informational purposes) and creek channel sediment samples (used for verification purposes) were collected in the area north of AOI 4 as presented on Figure 10.

• ‘Hot spot’ removal in a section along the northern slope of the Zipp Lot is complete. Figure 11 presents the final unvalidated results. Backfill placement, topsoil, and hydroseeding are complete. A double row of silt fencing remains in place.

• The final RCRA Facility Investigation (RFI) Work Plan: Addendum No. 5 was submitted to the U.S. EPA on July 27, 2004. Twelve groundwater monitoring wells were installed within the bedrock and two wells were installed within the overburden, as part of the implementation of this Work Plan. All newly installed monitoring wells have been developed and ten wells (eight bedrock and two overburden) have been sampled (groundwater samples could not be collected at four bedrock locations due to insufficient volume and parameter stabilization).

• On August 30, 2004, 250-milliliters of Fluorescein dye were injected at monitoring well MW-X233Y087S. The dye injection was followed by approximately nine-gallons of deionized water in order to flush the dye into the rock formation. Dye was visually observed at the location of Wet Well #1 of the SSC system (within the drainage pipe which collects groundwater from immediately down gradient of the pump house) approximately 1.5 hours after injection. Dye was also visually observed at Wet Well #1 within the drainage pipe collecting groundwater from the lateral trench shortly thereafter. Water samples (grab and from the automated sampler) are currently being analyzed for the presence of dye. Dye recovery monitoring was completed at the locations and frequencies indicated in the Work Plan. Visual evidence of the dye was only observed at Wet Well #1 of the SSC system. It is anticipated that this recovery monitoring will extend through the early part of the next reporting period for all locations where dye has not been detected. The results of this dye trace testing will be presented in the form of a RFI Technical Memorandum upon completion of data analysis.

• An on-Site construction meeting was held on August 23, 2004. Meeting minutes are attached in Appendix B.

• A bi-weekly all parties conference call was held on August 3 and 18, 2004 to discuss project progress.

3.0 SUMMARIES OF ALL ANTCIPATED PROBLEMS AND PLANNED RESOLUTIONS

• Access Agreements, as outlined in Appendix 6 of the AOC, have been provided to all

property owners where access was not previously obtained. Access had still not been received from property owners at 2499 Peerless Road as of the end of August 2004. − GM has continued discussions with the property owners of the remaining Parcel where

access has not yet been obtained. GM is clarifying the requirements of restoration of a section of the property currently enrolled in a Conservation Reserve Program with the United States Department of Agriculture (USDA).

− GM has submitted a letter to the Lawrence County Farm Service Agency (August 19, 2004) regarding the cleanup schedule and restoration commitments for this property.

• Three swallets are present within the stream channel downstream of the current cleanup activities; one is located on Parcel 15, and two are located on Parcel 216. − An Addendum No. 3 to the SSC Work Plan will be submitted which summarizes the

investigation and control of the swallets. • The lower quarry located on Parcels 11 and 12 have been separated from the creek channel to

assist in dewatering.. − If work activities in the creek progress beyond the quarry prior to quarry remediation, a

temporary berm will be constructed between the quarry and the creek to isolate the quarry from creek flow until removal activities are completed in the quarry.

− The quarry will be dewatered; accumulated sediment above the cleanup criteria will be removed, characterized and disposed using the same procedures as are being used for the rest of the Upstream Parcels.

• During August 2004, there were several days when TSP concentrations exceeded the action level. The action level for TSP is defined as 67% above the ambient upwind concentration. Cause of the exceedances included disposal truck and Zipp truck traffic in the Zipp lot, grading of fill material in close proximity of the air monitoring stations, inadequate watering of the haul roads, and dust generated during street sweeping activities. Corrective actions included increased watering of the haul roads and Zipp lot, instruction to off-road truckers to drive slower along the haul roads, and procurement of a more efficient street sweeper. In addition, the contractor did a test application of Soiltac, a vinyl acetate copolymer emulsion, along the haul road west of the storm lagoon. Soiltac penetrates the haul road soils, binding the particles. The test application was successful for a few days in keeping dust down, but did not appear to be a good long term solution. The contractor has currently returned to water suppression to control dust.

• Soil and sediment excavated from the area north of AOI 4 has been difficult to handle due to saturated nature of the material. GM has proposed to add a non-hazardous material (such as Fly Ash, Lime Kiln Dust (LKD), or Cement Kiln Dust (CKD)) to stabilize the excavated soil and sediment prior to shipment. Information regarding this proposal will be submitted to the U.S. EPA and IDEM for concurrence.

• Comments/questions on the July 2004 Monthly Progress Report was received via email from the U.S. EPA. − The U.S. EPA’s comments/questions and GM’s responses are presented in Appendix C.

4.0 PROJECTED WORK FOR THE NEXT REPORTING PERIOD • The following is a list of anticipated work for the next reporting period:

− Efforts to obtain an Access Agreement from the remaining resident in the downstream area will continue;

− GM is continuing preparation of the Wetland Mitigation Plan for submittal to IDEM and the United States Army Corps of Engineers (ACOE);

− Restoration on Parcels 4, 5, 6, 7, 8, 9, 10, and 11, will continue in September 2004; − Installation of the SSC components will continue in September 2004; − Restoration of Parcel 401 is expected to be completed in September 2004 with the planting

of trees and shrubs; − Excavation will continue in the area north of AOI 4 along the tributary on the north side

of GM property in September 2004; − The Dye Tracer Study, as part of RFI Work Plan: Addendum No. 5, will continue in

September 2004; − The preliminary data, as well as the results for additional sampling conducted for the dye

trace testing, will be forwarded during the next reporting period; − SSC Work Plan: Addendum No. 3 regarding the investigation and control of the swallets

is being drafted and will be submitted to the U.S. EPA and IDEM in the near future; − SSC Work Plan: Addendum No. 4 regarding changes and additions to the seep/spring

monitoring locations is being drafted and will be submitted to the U.S. EPA and IDEM in the near future;

− The Supplemental Stormwater Management Plan is being drafted and will be submitted to the U.S. EPA and IDEM in the near future;

− RFI Work Plan: Addendum No. 6 (swallet investigation) is being drafted and will be submitted to the U.S. EPA and IDEM in September 2004;

− Parcel 22 Removal Action Work Plan: Addendum No. 1 (revised excavation limits based on additional sampling) is being drafted and will be submitted to the U.S. EPA and IDEM in the near future;

GENERAL NOTES:(1). Cleanup Criteria

a.) Soils to < 1.8 mg/kg.- if all results are < 5.0 mg/kg, the cleanup objective can be verified in the Verification Area (approximately 100ft x 200ft) by calculating the Upper Confidence Limit (UCL) of the average concentration using statistics, if the UCL is < 1.8 mg/kg the cleanup objective will be met for the given Verification Area. A value of 0 mg/kg is used in the UCL calculation for sample grids excavated to bedrock.

b.) Sediments to < 1 mg/kg.(2). Composite side wall samples were collected in the location where the sidewall of the excavation exceeded 6 inches in height.(3). The surficial confirmation samples (8 composites per Verification Area except where bedrock is encountered) were collected after the soil/sediment removal was performed.(4). This email is being sent to address the Peter Ramanauskas' comments on the July 2004 Monthly Progress Report. We've included Peter's comments, followed by our response. Please let me know if you have any further questions. Comment: A release from the emergency storm water flow discharge. This is noted as being Outfall 002, but isn't the emergency stormwater Outfall 003? Since the sub-bullets talk about clogged pumps north of AOI-4, it leads me to believe this is Outfall 003. I think the NPDES permit has sampling requirements for when Outfall 003 discharges.

Response: This comment is based on an apparent misunderstanding of where the stormwater overflow occurred. The pump station at Outfall 002 has 3 pumps that are used to pump stormwater from the pump station holding tank to the stormwater retention basin. At the time of the rain event on July 20, 2004, two of the pumps were set to be turned on automatically. The third had to be started manually. In our discussions with waste water treatment personnel, the third pump was not immediately started when the rain began. As such, the tank holding capacity for stormwater at the pump station was exceeded and stormwater overflowed at Outfall 002. There was no stormwater overflow at Outfall 003 from the storm water retention basin during the rain event on July 20, 2004. Since July 20, 2004, the plant has made improvements to the system at the pump station. All three pumps are set to be turned on automatically. New floats were installed. These floats were set lower that the previous floats.

Comment: The second sub-bullet here (top of page V) notes that all floor and sidewall sampling from Parcel 3 to the dam at Parcel 12 met the cleanup criteria. Is this an accurate statement as figure 6 notes a "pending" 95% UCL calculation for verification area 9 and figure 7 notes "To Be Sampled" for verification area 11 along with 95% UCLs "pending"?

Response: A more accurate statement would have been "As of July 22, 2004, Verification Areas 1 through 8 met the cleanup criteria. Final UCL calculations are pending on Verification Areas 9 through 11." You are correct that at the time the Progress Report was written, the UCL calculations for Verification Areas 9 and 10 were still pending. Each of these areas had one sample point above 1.8 ppm. Since the time the report was issued, the UCL calculations have been completed for Verification Areas 9 and 10 - the cleanup criteria has been met. We have not completed excavation in Verification Area 11 because the excavation has not extended beyond the dam on Parcel 12. The dam is located at grids 11C and 11G. We have some individual points in Verification Area 11 above the 1.8 ppm criteria. We expect that once we have data from the entire verification area, that the UCL calculations will show we meet the cleanup criteria.
