EVALUATION CONTRACT This is an agreement between David Robertson (hereinafter referred to as the Evaluator) and Lynda Duke, Digital Media Studio (DMS) Co-Manager, James E. Walker Library, Middle Tennessee State University (MTSU) (hereinafter referred to as Evaluation Client). GENERAL INFORMATION Title of Project: Evaluation of the staff training program of the Digital Media Studio, James E. Walker Library, Middle Tennessee State University. Scope of Work: The contractor shall provide all material to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The scope of this task order and associated deliverables include (a) performing literature review using university-based search tools, (b) preparation of agreed-upon evaluation questions that will drive and guide the evaluation, (c) evaluation of the DMS training program for new studio assistants, (d) use qualitative and quantitative evaluation methods to produce data, (e) creation and collection using methods and techniques that ensure valid and reliable data that relate, (f) creation and administration of document(s) or spreadsheet(s) for data collected in this evaluation, (g) preparation of reports and briefing documents and revising as required, and (h) on-site visits for briefings meetings, student interviews and surveys. WORK STEPS Work steps shall include the following: (a) conduct literary database search relative to the type of program being evaluated and tools and techniques used elsewhere in similar projects, (b) develop and obtain approval of evaluation questions that will drive data collection and findings, (c) prepare an evaluation work flow and schedule, (d) identify evaluation methods, means, processes, and techniques and relate each with evaluation questions, (e) identify and document data collection methods selected, (f) define data collection sites, sample, and resources required, (g) collect data, (h) reduce and analyze data using valid and reliable analysis statistics and tools, (i) provide initial findings with sponsors and revise as required, (j) prepare and submit draft final reports, and (k) revise as required. FIELD VISITS All fieldwork and on-site observations shall be coordinated with the Evaluation Client. EVALUATION CONTRACT

computing needs is effective.

Performance Period: The work shall begin within 10 calendar days of award, unless otherwisespecified. Work at the site shall not take place on MTSU holidays or weekends unless directed bythe Evaluation Client. Report delivery items and schedule to be set at kickoff meeting.

Type of Contract: No charge to the Evaluation Client.

CONTRACT AWARD MEETING

The Evaluator shall not commence performance on the tasks in this SOW until the EvaluationClient has conducted a kickoff meeting (kickoff meeting may be waived by the Evaluation Client).

GENERAL REQUIREMENTS

1. The Evaluator shall confirm work assignments by email or telephone with the EvaluationClient. A brief outline of work approach shall be reflected in the technical proposal.

2. All written deliverables shall be phrased in acceptable terminology of the field.3. This contract shall not require access to studio assistant files.4. All deliverables, except where specified otherwise, shall be provided in one electronic

copy via e-mail to the Evaluation Client.5. All deliverables shall be delivered in software compatible and accessible to the DMS

Evaluation Client.

SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES

Description of Tasks and Associated Deliverables

The Evaluator shall provide the specific deliverables described below.

Task 1: The Evaluator shall conduct evaluation studies and data in accordance with the evaluationmanagement plan and the agreed upon evaluation questions. The Evaluation Client shall reviewthe draft review and provide verbal feedback. The Evaluator shall update the review as required byguidance from the Evaluation Client. The deliverable shall be in electronic form transmitted viae-mail, using software compatible with software available at MTSU.

Deliverable 1: Evaluation Report

Task 2: The Evaluator shall provide detailed analysis of selected topics and issues identified in theevaluation report number, and type of topics shall be in accordance with guidance from theEvaluation Client. Work shall proceed on a no charge basis. The Evaluator shall presentsignificant and timely findings to the Evaluation Client using an appropriate evaluator-developedformat in paper and electronic format.

CHANGES TO STATEMENT OF WORK

Any changes to this statement of work shall be authorized and approved by the Evaluation Client.A copy of each change shall be kept in a project folder along with all other products of the project.

REPORTING REQUIREMENTS

The Evaluator is required to provide the Evaluation Client with weekly telephone, e-mail, orin-person progress reports.

TRAVEL AND SITE VISITS

Since the Evaluator is employed with the Evaluation Client, there will be no additional travel and aweekly check-in will occur during normal working hours with the Evaluation Client.

SCHOOL RESPONSIBILITIES

The Digital Media Studio shall provide access to technical and procedural information.

CONTRACTOR EXPERIENCE REQUIREMENTS

The Evaluator will perform this evaluation as an authentic learning experience in the ProgramEvaluation course (MEDT 8480) in the Ed.S. program at the University of West Georgia, Collegeof Education, Department of Educational Innovation. 1. The preliminary and final deliverables and all associated working papers and othermaterial deemed relevant by the Evaluation Client that have been generated by theEvaluator in the performance of this task order are the exclusive property of the DigitalMedia Studio under the custodial care of the Evaluation Client.

2. The deliverables will be submitted at the end of the evaluation initiative.3. The Evaluator shall be the sole authorized official to release verbally or in writing any data,

the draft deliverables, the final deliverables, or any other written or printed materialspertaining to this task order. No information shall be released by the Evaluator.

The Evaluation Contract, Evaluation Plan, and Final Evaluation Report will be submitted to Dr.Danilo Baylen at the University of West Georgia as partial fulfillment for the Program Evaluationcourse (MEDT 8480) in the Ed.S. program at the University of West Georgia, College ofEducation, Department of Educational Innovation. The following schedule must be met for courserequirements.
