Detailed Instructions 1. You will receive a URL to access the survey. Open the URL in a web browser, such as Internet

Explorer, Chrome, FireFox, etc.

If you don’t receive the URL you can access the survey here: http://www.oregon.gov/das/HR/Pages/equitysurvey-enter.aspx.

2. You will be asked to enter your respondent key. Your respondent key is your nine digit OR number

assigned in the personnel system, plus the four digit month and day of your birthday.

Your OR# can be found on your pay statement or any personnel action documents. If you don’t know what your OR# is contact your agency payroll or human resources office.

3. Click “Login” to go to general information about the survey. If at any point you need to leave the

survey before finishing it make sure to click the “Save” button to save the information you have entered up to that point.

6. This screen provides information regarding Race/Ethnicity, Disability, Age, Gender and Veteran

Status. Please review and indicate the protected class information you keep “Private” or “Not Private” before clicking “Next”.

7. The first section will be auto-populated with your data from the personnel system. You need to verify the information is correct. Select “Yes” if the information is correct. Select “No” if the information is incorrect and enter in the correct information in the appropriate textbox. Once the information is correct click “Next”.

8. The “Current Position” section is where you will provide information on the primary purpose of your job and the occupational group you believe it falls within. Note: About 50% of employees will not need to complete this section of the survey because the Chief HR Office already has adequate information about your job duties. If this question is presented to you in the survey, please answer it.

In the “Primary Purpose” field you will want to write a brief description of why your position exists and its role in reaching the program objectives. If you have an updated position description you can take the information from section 2B. Think in terms of describing a job to a new friend. A short paragraph is sufficient to describe most jobs.

You are limited to 750 characters for this field. Here are some examples of descriptions for a position’s primary purpose: • This job provides administrative support for the engineers in the unit by typing letters and

reports and keeping accurate records. • This job directs and monitors the work of technical and professional employees who collect and

analyze samples to ensure industry compliance with environmental rules and regulations. • This job does standard technical lab tests on body fluid samples such as urinalysis and blood

counts.

9. For the “Occupational Group,” select the one that best fits your current position. If you are unsure

or you don’t see it listed, select Not Sure/Not Listed. Otherwise, select the appropriate radio button and then click “Next” to go to the next section. Note: About 50% of employees will not need to complete this section of the survey because the Chief HR Office already has adequate information about your job duties. If this question is presented to you, please answer it. For example, if you are an Operations and Policy Analyst (OPA), working on education policies, please select “Education.”

10. The “Essential Duties of the Position” section is where you will summarize the specific duties and

responsibilities for which you are accountable. You will want to focus on the duties that take the largest percentage of your time on the job. It is not necessary to list every duty performed by the position. If you have an updated position description you can take the information from section 3. Note: About 50% of employees will not need to complete this section of the survey because the Chief HR Office already has adequate information about your job duties. If this question is presented to you, please answer it.

Limit your list to the top three (3) to five (5) most important duties.

You are limited to 750 characters for each duty field. Here are some examples of duty descriptions: • Duty #1 Provide oversight of budgets, expenditures, fiscal and financial audits. • Duty #2 Lead strategic planning efforts to include research and analysis, program evaluation, and

policy development. • Duty #3 Hire, train, supervise, coach and evaluate support staff and technical contractors. Click “Next” to go to the next section.

11. In the “Prior Positions Held” section, you will want to include other jobs you have held that provided you with the necessary knowledge and skills to perform the duties in your current position. For each position, summarize the specific and measurable tasks which you performed and were accountable and the results or outcomes for which you were accountable for each job duty of that position. These can be positions held in the private sector as well as the public sector. For each prior position held, you will need to enter the job title, the employer, the month and year you started the job, the month and year you left the job, and a description of the duties and responsibilities. Note: Depending on your classification, you will be asked to provide 10 to 15 years of experience or only positions that are relevant to your current position. This experience does not need to be consecutive years.

Here are some examples of responsibilities: • Approve all expenditures for the departmental and campus capital projects. • Manage departmental accounting, purchasing and disbursements operations. • Develop and monitor the department operating budget, which included reversing a $650,000

deficit within one year. • Manage installation of new computers and HRIS system. • Develop and implement accounting policy and procedures for the college. • Review construction and equipment lease contracts for compliance. • Develop and manage cash flow and expenditures for construction projects. • Assist with annual reporting and closing procedures. • Compile and organize business records for internal and external auditors. • Prepare and analyze reports for senior management. • Supervise ten accountants, reorganizing them into project teams.

12. If you need to add another position, select “Add another position” and click “Next”. If you are done

adding positions, select “I’m finished adding positions,” and click “Next”. You can add up to ten prior positions.

Click “Next” to move to the next section.

13. The “Relevant Education” section is where you will want to identify the highest level of education

you have completed or the degree you hold that is relevant to your current position. If you have attended some college but did not receive a degree, add the number of credits you completed and whether they are semester, quarter, or classroom hours in the “Other Degree” field.

If the field of study or school is not in the drop-down menus, add that information in the Other Degree field.

Click “Next” to move to the next section.

14. The “Related Training” section is where you can add any certification, licenses, or formal training you

have that is relevant to your current position.

Under the certifications, there are three areas with a drop-down menu listing various certifications and licenses. If you have one from the list click on it to select the certification or license.

15. To add any additional certifications, licenses or formal training you will add them below. If you keep your training and certifications in iLearnOregon, you should log into the system and save a copy of your transcript to add the information into the survey.

Click “Next” to move to the next section.

16. In the final section, you need to enter your name and the date you completed the survey. Make sure to click the “Finish” button to save all of your responses and complete the survey.

**IMPORTANT** Once you click “Finish” you will no longer be able to access the survey or change any

of your responses. Make sure to only “Save” the survey if you would like to view or change any of your responses.
