General Consent Form - Preschool 2020.1
Overview:
Schools and Preschools need to seek parental permission to cover a wide range of activities and situations. This general consent form will cover your child whilst they are enrolled and will be in effect until we receive correspondence indicating otherwise. Consents: YES NO
Obtaining Emergency Medical Assistance: I give permission for the school to take the appropriate action (including calling an ambulance if necessary) in the event of a medical emergency.
Borrowing Library Books: I give permission for my child to borrow books from the library & I accept responsibility for any book damaged or lost by my child.
Applying Sun Block As Necessary I give permission for my child to have sun block as necessary.
Permission To Publish Names: I give permission for my child’s name to be published (by the Preschool, School or concerned third parties) should they receive commendations, prizes or acknowledgement of participation in competitions, programs or events.
Being Checked For Head Lice: I give permission for staff to perform a discreet head lice check if necessary. If permission is not given & head lice is suspected, I understand it is my responsibility to arrange collection of my child when notified.
Going On Supervised Local Group Walks: I consent to my child taking part in local walks during the school year for educational purposes. Parents will be notified in advance of the local excursion when possible.
Individual Consent Forms are Required for: ICT Acceptable Use Agreement
This agreement outlines the site’s roles and responsibilities in supporting safe digital learning, as well as the expected behaviours we have of children when using digital or online spaces – both at school and at home.
Consent – Image Creative – Publications (Online & Print): Permission to use image, video, voice, and/or creative work of students for school photographs, newsletters, media etc.
Swimming & Aquatic Consent Form (For School Pool Recreational Use) This covers recreational swimming at the Nairne School pool under staff supervision and under Department for Education guidelines for water activities. Please be aware that instructor-led swimming lessons will require an additional permission form to be filled out annually as these are run by external providers.
