Dear Foster Family,

Welcome to Oklahoma City Indian Clinic. Thank you so much for all that you do and for becoming a foster family for a Native American child. We want to take excellent care of your foster child and provide them with a stable medical home. We are located in southwest Oklahoma City and we provide culturally sensitive, comprehensive medical services to Native American children.

Services provided include:

• Complete medical care by board certified pediatricians including well child exams, sick visits, vaccines, full laboratory and radiological evaluation. We also have a complete, stocked pharmacy to meet all of your child’s medical needs.

• Behavioral health services include counseling, individual and group mentoring programs

• Pediatric specialists including pediatric endocrinology and pediatric psychiatry

• Vision• Dental • Wellness support including nutritional counseling, exercise programs and fun

camps/programs all throughout the year and during the summer

hOw dO i make an appOintment at the CliniC?Great question! The first thing you need to do is set up a chart at the clinic through the pediatric department.

Our staff is trained to help you locate tribal information on your child that you may not have immediate access to, such as tribal cards. Please do this at least one week before your appointment. Please see the next page for

more information on HOW to set up a chart at our clinic.

The second thing you need to do is call our pediatric clinic at 405-948-4900 ext 633 and request to make an appointment with our FOSTER CLINIC.

what if i dOn’t have sOme Of the required infOrmatiOn needed tO set up a Chart?

Don’t worry about it. Bring what you have and when you do receive more information at a later date (such as social security card and birth certificate) – simply bring it in.

hOw lOng will my appOintment last?The first visit will be a comprehensive 4 hour visit. This visit will include an orientation presented by our

behavioral health staff, a provider exam and dental/vision assessment. Any visits after the initial one will be 20 to 40 minute visits to address any additional medical needs or concerns.

what time dO i need tO be here fOr my appOintment?Please show up at 8 a.m. and go to the Oklahoma City Indian Clinic at 5208 West Reno (across the street from

the main clinic). Please to go the 3rd floor by the elevator and follow the signs to the conference room.

when will the CliniC be held?This could change at any time but right now it will be held on the second and fourth Wednesday of every

month from 8 a.m. to 12 p.m. Please call for exact clinic date.

To establish a chart at Oklahoma City Indian Clinic, you must schedule an appointment for a New Patient Physical and identify that you have a foster child or are a DHS worker. You can call the Turquoise Team/Pediatrics Department at extensions 633 or 635.

Patient Financial Responsibility: Under the Indian Health Care Improvement Act Amendments, Public Law 100-713, Oklahoma City Indian Clinic is considered the “payer of last resort” and required by Federal law to seek and collect payment from any medical program that patients may be eligible to participate in such as SoonerCare. It is the financial responsibility of each patient to present all current or new insurance cards during check-in and apply for available medical programs with the assistance of a clinic Benefits Technician.

You MUST submit ORIGINALS of the items listed below:

Eligibility Documents:

YOU MUST PROVIDE PLACEMENT PAPERS OR DHS CASEWORKER PAPERWORK• CDIB (Certificate of Degree of Indian Blood) card issued by the Bureau of Indian Affairs, or• Federally recognized tribal enrollment card, or• Letter of Descendancy from a federally recognized tribe

Social Security Card: All persons must have a Social Security card within one year from date of birth.

Certified Birth Certificate: Required for all applicants under 18 years of age. Appointed guardians must have court documentation for minor children.

Immunization Record: For persons under 16 years of age.

Medication List: Needed for all applicants. Please provide a list of any prescriptions and over-the-counter medication you are taking (Including herbals/supplements, and vitamins/minerals).

Medicaid, Medicare, private or tribal insurance card: If you are receiving Medicaid, Medicare or if you have private or tribal insurance, please provide a copy of your card(s) to the registration staff upon each visit. The clinic is required by Federal law to seek and collect payment from any insurance the patient may be eligible to participate in and patients are required to provide this information.

Moccasin Movers Senior Chair Exercise Classes A class that is designed to provide low impact

exercise and physical activity for elders.

Tuesdays and Thursdays from 10 a.m. to 10:30 a.m.

Family BikingFamilies can bike together on trails at

Lake Overholser. Rides start at 10 AM. Bikes provided for

patients ages 10 and over. Younger patients must provide their own bicycle. Kids 10 and under must be accompanied by a parent.

Y o u t h C a m pPatients ages 6-17 learn about diabetes prevention through healthy eating, being active, arts, and crafts.

NYPD Camp Native Youth Preventing Diabetes

Children ages 8-12 can pack their bags for a five day/four night diabetes prevention camp.

Patients of all ages can learn about produce seasonality and local agriculture, while using OKCIC shopping bucks to purchase their family fruits and vegetables.

A dance fitness program for patients ages 12 and above to build strength and stamina in a fun environment.

Martial Arts CampPatients ages 10-17 can attend this two-day camp that will help children thrive mentally and physically, while learning the basics of karate and boxing.

Soccer CampPatients ages 6-17 can attend this three-day camp that will teach fundamental soccer skills. Children will have fun, while learning the importance of practice, control, good sportsmanship and letting go of mistakes.

Cultural Camp Patients ages 10-17 will build self-esteem, leadership and health and wellness skills from the traditional knowledge passed from Native elders and community leaders.
