Foreword

Dear Doctor, “Essential drugs are those that satisfy the health care needs of the majority of the

population; they should therefore be available at all times in adequate amounts and in

the appropriate dosage forms, and at a price that individuals and the community can

afford”.

Careful selection of a limited range of essential medicines results in a higher quality of care,

better management of medicines (including improved quality of prescribed medicines),

and more cost-effective use of health resources”

(WHO Expert Committee on the Use of Essential Drugs, 1999).

Many states in India including Karnataka, Kerala, Himachal Pradesh, Delhi, Maharashtra and

Rajasthan have prepared their essential drug lists. The Government of Chhattisgarh also has taken

the initiative to prepare and adopt the Essential Drug List for Chhattisgarh (EDLC). The relevant

Government Orders are enclosed.

The Basic Goals of the ELD Chhattisgarh are:

To ensure the availability and accessibility of essential drugs to all citizens of

Chhattisgarh;

To ensure safety, efficacy and quality of drugs in the public health system;

To encourage and support good prescribing and dispensing practices;

The Process: The Department of Health & Family constituted a committee headed by the Director,

Medical Education with the Director Health Services and the Dean of the Medical College as

members to draw up theEDL Chhattisgarh. In turn, this committee, with the technical assistance of

the State Health Resource Centre, Chhattisgarh set up one day three intensive consultation with

senior medical professionals, the faculty of Raipur Medical College, NGOs active in the health field,

and senior departmental officials. The State Advisory Committee also gave valuable inputs to this

process. Experts in rational drug policy from other states were also invited to this consultation. The

W H O Model List for the year 2015, NELM of 2015, British National Formulary 2009 and National

Drug Formulary 2011 were used as the base papers for this discussion in finalising theEDL

Chhattisgarh.

Selection of Essential medical list

The process by which medicines are selected is critical. An essential medicines list which is

imposed from above will not reflect the need of the users or be accepted by them. It is therefore

very important that the process be consultative and transparent, selection criteria be explicit,

selection of the medicines be linked to evidence-based standard clinical guidelines, clinical

guidelines and the list be divided into levels of care, and are regularly reviewed and updated. A

review of the clinical guidelines and the list should be carried out at least every second year, and

their use and the impact should be monitored. The process of developing a list of essential drug list

is mentioned below.

The Criteria: The principal criteria for the selection of essential drugs for primary, secondary and tertiary hospital care were based on the WHO guidelines for drawing up an EDL. They include the following points:

Any drug included must meet the needs of the majority of the population;

Sufficient proven scientific data regarding effectiveness must be available;

Any drug included in the EDL should have substantial safety and benefit/ risk ratio;

All products must be of acceptable quality, and must be tested on a continuous basis;

The aim, as a rule, is to include, only products containing single pharmacologically active ingredients;

Combination products, as an exception, will be included where patient compliance becomes an important factor or where two pharmacologically active ingredients are synergistically active in a product;

Products will be listed according to their generic names only;

Where drugs are clinically equally effective, the dugs will be compared on the following factors: 1. The best cost advantage, 2. The best researched, 3. The best pharmaco kinetic properties, 4. The best patient compliance.

Grading of the Essential Drug List: The EDL Chhattisgarh is published in two parts, first part consist

of list of drugs which are essentially required at primary, secondary and tertiary level of medical

facilities. Second part is drugs which are required at specialty centers and are prescribed by

specialists with extra attention. The main EDL Chhattisgarh covers the needs of all public health

facilities excepting the medical college. It is graded to indicate drugs that shall be supplied to all

health centers and those that are restricted to Primary, secondary and tertiary level medical

facilities.(which includes PHC, community health centers and District Hospitals).

Depending on the specialist and equipment available in each health care institution these facilities

would specify their drug requirements from within this list. The list of drugs for the Medical College

Hospital (tertiary care), was prepared by the faculty of the college and is published as a

supplementary list. Another supplementary list (Universal category) specifies the drugs that would

be made available to the multipurpose health workers –both male and female. While grading the list

we have kept in mind the specific demands of Chhattisgarh given the distances and problems in

transport and the fact that referral to higher centers is not easy. The broad principle followed is to

provide as comprehensive a care as possible, made available as peripherally as possible - given

the human resource and infrastructure available at that level. All drug purchase made by the

Government of Chhattisgarh would have to comply with the EDL Chhattisgarh.

Amendment of EDL/ Adding a New Product on EDL Chhattisgarh: We recognize that a good drug

policy requires to EDL Chhattisgarh dynamic and updated periodically. We welcome suggestions

towards this. Requests for products to be included on the EDL Chhattisgarh must conform to the

above criteria and must be supported by evidence-based data and appropriate references on its

advantages and benefits over existing products. A format for such requests has been given as

been appended.

The Department of Health and Family Welfare will be appointing a Committee to monitor the

implementation of the EDL Chhattisgarh and make periodic revisions of the list. An essential drug

formulary, and a graded standard treatment protocol as well as mechanisms to train various

categories of staff and promote the use of the list would be in place soon. Other supporting systems

to ensure safe and adequate essential drug supply to every citizen of Chhattisgarh are in our plans.

I hereby express my sincere thanks to all those experts, the medical community of Chhattisgarh

and the concerned NGO representatives whose valid contributions paved way to the EDL

Chhattisgarh and its publication. I hope the medical community of the state will welcome and

nourish this move as this is the threshold point towards a comprehensive and rational drug policy

Information provided in this form is handled instrict confidence. The causality assessment iscarried out at Adverse Drug Reaction MonitoringCentres (AMCs) by using WHO-UMC scale. Theanalyzed forms are forwarded to the NationalCoordinating Centre through the ADR database.Finally the data is analyzed and forwarded to theGlobal Pharmacovigilance Database managed byWHO Uppsala Monitoring Center in Sweden.

The reports are periodically reviewed by theNational Coordinating Centre (PvPI). Theinformation generated on the basis of these reportshelps in continuous assessment of the benefit-riskratio of medicines.

The information is submitted to the SteeringCommittee of PvPI constituted by the Ministry ofHealth and Family Welfare. The Committee isentrusted with the responsibility to review the dataand suggest any interventions that may be required.
