Each year during the Annual Benefi ts Election Period, you have the opportunity to elect a Health Care Flexible Spending Account (FSA) or change your contribu-tions. You must make a new election if you want a Health Care FSA in 2012. Your 2011 election will not automatically continue into 2012.

Heath Care Flexible Spending Account Can Save You Money!

If you want to take advantage of the tax-savings opportunity a Health Care Flexible Spending Account (FSA) provides in 2012, you must make a new election during the Annual Benefits Election Period. Your current Health Care FSA election does not carry over into the following year.

Health Care FSA

Use this account to pay out-of-pocket healthcare expenses you and your dependents incur. Your dependents are anyone you claim as a dependent for federal income tax purposes – as well as children up to age 26 whether or not they are dependent on you for financial support. For example, you can pay deductibles, coinsurance, copays, amounts above reasonable and customary limits and other unreimbursed medical, dental, prescription drug, vision and hearing expenses. Using a Health Care FSA to cover expenses you would have paid for anyway reduces your taxable income by that amount, which in turn lowers your taxes.

Make Your FSA Election CarefullyEstimate your out-of-pocket health care expenses and corresponding Health Care FSA election conservatively to avoid forfeiture. Remember that you can always increase your contributions next year. To Make Your FSA Election

Log on to Montefiore’s Benefits Website − www.montebenefits.com − or call the Benefits Enrollment Call Center at 888.860.6166, Monday through Friday, between 8am and 8pm EST. You’ll speak to an enrollment specialist who will help you enroll.

•Review your current information (address, phone number, etc.) and notify HR of any corrections on a Change of Information Form (a copy is on Montefiore’s Intranet eMontefiore under HR).

•Health Care Flexible Spending Account − You must make a new Health Care FSA election if you want this account in 2012. Your current election will not roll over into next year.

•Qualify for the full annual 15+ Years of Service Catch-up Contribution $20,000 (17,000 + $3,000)

•Qualify for the Age 50+ but not the 15+ Years of Service Catch-up Contribution

$22,500 ($17,000 + $5,500)

•Qualify for the Age 50+ and the 15+ Years of Service Catch-up Contributions

$25,500 ($17,000 + $3,000 + $5,500)

At any time during the year, to elect the Voluntary Tax Deferred Annuity 403(b) Plan, change your election and/or if you are eligible for and want to make a “catch up” contribution contact Principal at 800.547.7754 or go online to www.principal.com.

Montefiore Voluntary Tax Deferred Annuity 403(b) Plan

The Montefiore Voluntary Tax Deferred Annuity 403(b) Plan provides an excellent tool to help you save for your future financial security. Your before-tax contributions are automatically deducted from your paycheck and accumulate earnings on a tax-deferred basis. You make your own investment decisions based on your investment strategy and the level of risk you are willing to accept. You can change your allocations and transfer amounts among investment options.

Maximum contributions are determined by the IRS and are adjusted annually. Plus, there are two “catch-up” opportunities.

Weight Watchers @ Work

Weight Watchers is a weight loss program based on nutritional guidelines and scientific research that encourages both healthy eating and exercise/activity. Montefiore is providing a 25% upfront subsidy for Weight Watchers meetings and online programs. After participating in the program, you will be eligible for an additional 25% rebate.

Smoking Cessation

Over the past year, Montefiore has initiated a tobacco-free campus and offered RNs a number of education and help options to assist them to quit using tobacco products.

We are all aware that using tobacco is hazardous to your health. And those health risks translate to a huge cost in quality of life, overall life expectancy and dollars spent on healthcare. Here are a few facts (from the Centers for Disease Control and Prevention website).

•Cigarette smoking is the leading preventable cause of death in the United States.

•Compared with nonsmokers, smoking is estimated to increase the risk of:

ocoronary heart disease by 2 to 4 times

ostroke by 2 to 4 times

o lung cancer – for men by 23 times, for women by 13 times

ochronic obstructive lung disease related death (such as chronic bronchitis and emphysema) by 12 to 13 times

•Smoking also has many adverse reproductive and early childhood effects, including increased risk for:

o infertility

opreterm delivery

ostillbirth

o low birth weight, and

osudden infant death syndrome (SIDS).

“Helping Others” Healthy Points Program

Feel good about yourself as you take charge of your health and you help us improve the lives of others. Through the “Helping Others” Healthy Points Program, you earn and accumulate points by participating in To Your Health! activities.

Accumulating Healthy Points is easy. Here’s how.

•5 points each quarter for simply registering and/or visiting and exploring the To Your Health! website

•40 points for completing for the first time or updating your To Your Health! (Health Risk) Assessment

•20 points for completing any one of the six Lifestyle Improvement Programs that are offered

•5 points for using the Symptom Checker on the website

•5 points for completing a Self-assessment

•25 points for having a routine physical/preventive exam and/or undergoing a health screening

•25 points for consulting with the OHS Nutrition Consultant

Healthy Points are automatically recorded when you complete an online activity. You have to self-report all other activities on the Rewards Lobby screen. Enter your activity date, check the box next to the activity and click ‘Submit’ to record your Healthy Points.

At the end of the year, a program selected to benefit from your participation will automatically receive an allocation based on the total number of points you – and all Montefiore associates – have earned. In 2012, a donation based on points earned in 2011, will be made to the Children’s Hospital at Montefiore.

Commuter Benefits Program

If you enroll in the Commuter Benefits Program, you can save money on your commutation expenses whether you use mass transit, drive or a combination of both. Your contributions are automatically deducted from your paycheck before taxes are calculated and withheld – lowering your taxable income and reducing the taxes you must pay! Critical Illness Insurance

Critical Illness Insurance complements your medical and disability income coverage and can reduce the financial impact of disability or serious illness. A lump sum benefit is paid directly to you for the first occurrence of a covered critical illness. You have full control over how the benefit is used. Disability Income Insurance

Eligible 1199 Members can purchase additional insurance designed to coordinate with other disability benefits.

•Voluntary Short Term Disability Insurance through Transamerica Financial Life Insurance Company – after 60 days of disability, you may receive benefits equal to 60% of your salary – up to a maxi-mum benefit of $5,000 each month for up to 365 days.

•Voluntary Long Term Disability Insurance through First Reliance Standard of New York – after one year of disability, you may receive benefits equal to 60% of your salary – up to a maximum benefit of $5,000 each month for as long as you remain disabled or up to age 65.

Universal & Whole Life Insurance with Long Term Care

Life Insurance offers financial protection for your family against possible financial hardship in the event of your death. Montefiore offers two different types of Life Insurance – Universal and Whole Life.

Both policies offer:

•Convenience – You may arrange for automatic premium payments through payroll deduction.

•Portability – You own your policy and can take it with you if you leave Montefiore, without any rate or coverage changes.

•Tax-Deferred Cash Value Accumulation – A portion of your premium payments may be deposited into an interest earning account. The earnings on your account grow tax-deferred as long as the policy remains in force.

•Accessibility – You may borrow against the cash value of your policy at a set rate of interest. While the loan is outstanding, you continue to earn interest on the remaining cash value, which helps offset the interest you pay on the loan.

•Dependent Coverage is available for your spouse and children.

•Long Term Care – use a portion of your benefit to help cover the cost of qualifying long term care events.

•Optional Riders – You may purchase and add to your policy.
