This brochure has been developed to assist you in preparing for the 2017 annual benefits enrollment It is important to review each benefit option available so please take time to go through this brochure along with the Important Steps listed on this page
Whatrsquos New In 2017
Annual enrollment begins November 14th and ends November 23rd All
eligible teammates must access the ADP Vantage HCMreg system and
complete the online enrollment process to elect or waive benefits Some
of the 2016 benefit elections will not carry forward into 2017
UnitedHealthcare (UHC) and Optum Health Services have been selected to
provide the administrative services for the
MedicalRx plan options
Health Savings Accounts (HSA)
Flexible Spending Accounts (FSA)
Commuter Benefits (Transit andor Parking)
Enrollment for Commuter Benefits (TransitParking) offered by a UHC
partnership with WageWorks will be a separate enrollment time frame
and process directly on the WageWorks website wwwwageworkscom
Refer to the WageWorks instructions posted on the MyBenergy benefits
portal
Virtual Visits will be offered under the UHC contract for our teammates
that enroll in one of the medical options Teammates can access a UHC
virtual visit provider from a video capable mobile device or computer
without an appointment for non-emergency services
Two new group voluntary insurance offerings from Aflac Accident
Insurance and Critical Illness Insurance
The Guardian High Dental plan will be amended to include adult
orthodontics and dental implants
The 2016 BCBS Wellness Rewards Program Incentives will end on
12312016 Details for the new 2017 UnitedHealthcare Rally Engage
Wellness Program will be announced during the first quarter of 2017

medical options will utilize the national UHC Choice Plus
network Teammates residing in Massachusetts New
Hampshire and Maine will utilize the UHC Choice Plus
with Harvard Pilgrim network
A UHC network comparison to current network providers
indicates more than 95 of teammates will find their
current doctors and hospitals within the UHC Choice Plus
network
The UHC Choice Plus national lab network provider is
Lab Corp
The medical plan options include prescription drug
benefits through Optum Rx and Specialty Rx through
Brovia Rx a UHC company
A mobile app is available to provide easy access to your
UHC medical ID card(s) find nearby network providers

The Health Savings Accounts (HSA) will be administered
by Optum Bank For active teammates enrolled in the
High Deductible Health Plan (HDHP) and elect an Optum
Bank HSA the HSA is now offered without a fee for the
basic account
New UHC MedicalRx ID cards along with HSA and FSA
debit cards will be issued
Elected Commuter Benefits (TransitParking) passes or
cards will be issued via WageWorks
A customized UHC pre-member website is available
during the enrollment period to introduce the UHC

In 2017 You Can Continue To
Choose from two medical options a copay Preferred Provider Organization (referred to as Choice Plus Copay) or a High
Deductible Health Plan (referred to as Choice Plus HDHP) Both are administered by UnitedHealthcare
Contribute up to $3400 for single coverage or $6750 for family coverage on a pre-tax basis to a Health Savings
Account (HSA) through Optum Bank if you elect the HDHP plan ($1000 catch-up contribution available for 55+
participants)
Choose from two fully insured dental plans offered by Guardian Life Insurance Company a High or Low dental plan
Elect a vision plan offered by Guardian Life Insurance Company utilizing the national VSP network
Elect Group Term Life with Accidental Death amp
Dismemberment plan (GTL) through Lincoln Financial
Elect Voluntary Term Life with Accidental Death amp
Dismemberment plan (VTL) through Lincoln Financial
Evidence of Insurability (EOI) will be required for late
enrollee elections and incremental increases
Elect Personal Accident Insurance (ADampD) through Cigna
Elect Long-Term Disability (LTD) through Cigna
Contribute up to $2600 to a Limited Purpose or Full
Purpose Healthcare Flexible Spending Account (FSA) on a
pre-tax basis administered through Optum
Contribute up to $5000 to a Dependent Day Care Account on a pre-tax basis administered through Optum As a
reminder medical expenses for your dependent(s) are not eligible for reimbursement under the Dependent Day Care
FSA
Contribute to a Transit andor Parking Commuter Benefit on a pre-tax basis administered by UHC through partnership
with WageWorks
IMPORTANT NOTES 1 Annual Enrollment is the only time you can change your benefit elections unless you experience a qualifying life event If
you do not make your elections by the enrollment deadline your current elections will not carry over into 2017 You must access the ADP Vantage HCMreg system and complete the annual enrollment process to elect or waive benefits
2 You must make and confirm your benefit elections no later than 1159 pm Eastern Time on November 23 2016 Be sure
to submit your information and receive a confirmation number for your 2017 elections It is important that you retain a copy of your printed confirmation as proof of your enrollment

CHECK to be sure each of your eligible dependents are enrolled and verify their Social Security Number(s) and
date of birth(s) as you enroll for 2017 coverages
MAKE your annual enrollment elections between November 14 2016 and November 23 2016 on the ADP
Vantage HCMreg Annual Enrollment website To make your elections log in to httpsadpvantageadpcom go to
Myself gt Benefits gt Enrollments then click on Enroll Now Newly hired teammates will need to register in ADP Vantage HCMreg prior to logging on the website and
selecting benefits Access your ADP Vantage HCMreg account before the 2017 Annual Enrollment period to ensure you can make
your elections If you need assistance with your user name and password email BBVantageISbbinscom With the Vantage enrollment system you must elect or waive each benefit offered Keep in mind if you leave
the system prior to receiving a confirmation number you will need to return and complete your enrollment
The enrollment instructions for WageWorks are posted on the MyBenergy benefits portal
REVIEW your 2017 enrollment summary page to ensure you and your dependents have the coverage you
elected and verify your beneficiary information
CONFIRM your 2017 elections and SUBMIT your enrollment
PRINT the 2017 Election Confirmation page and check your elections for accuracy including the Pre or Post Tax
deduction basis selected and retain for your records This printed confirmation page is proof of your enrollment
WATCH the mail for your new benefit ID cards (including MedicalRx Dental Vision FSA debit cards andor
Optum Bank HSA MasterCard(s)) which should be received in late December Start using them on January 1 2017

Annual enrollment begins November 14th and ends November 23rd All
eligible teammates must access the ADP Vantage HCMreg system and
complete the online enrollment process to elect or waive benefits Some
of the 2016 benefit elections will not carry forward into 2017
UnitedHealthcare (UHC) and Optum Health Services have been selected to
provide the administrative services for the

Commuter Benefits (Transit andor Parking)
Enrollment for Commuter Benefits (TransitParking) offered by a UHC

Contribute up to $3400 for single coverage or $6750 for family coverage on a pre-tax basis to a Health Savings
Account (HSA) through Optum Bank if you elect the HDHP plan ($1000 catch-up contribution available for 55+

Choose from two fully insured dental plans offered by Guardian Life Insurance Company a High or Low dental plan
Elect a vision plan offered by Guardian Life Insurance Company utilizing the national VSP network
