Based on a ‘companionship’, or ‘relationship model, Extended Families build community

To be eligible for services within the HACC Program, a person must be frail aged or
have a disability. The disability must be moderate to severe and be assessed as a
functional disability. Therefore it must interfere with the task of daily living, such
as dressing, showering, preparing meals, shopping, and utilising public transport.
Priority is given to those clients who are at risk of residential care.

is based at Mt Eliza Centre and delivers a range of community programs to assist older people
to maximise safety and minimize falls risk in daily activities. Home and centre based
assessment and management of falls and Falls risk for people living in the community.
Clients can be self -referred or referred by a local doctor or service providers.
A therapist will arrange a visit at home and assist the client to develop an individual program
to address issues around falls and safety.

The Peninsula Hospice Service (PHS) provides care at home for people living with a
progressive life threatening illness. Services include nursing and medical care, counselling,
pastoral care, art and music therapy, social work and welfare support, bereavement and
patient care volunteer support. Support is offered to families and carers in addition to
patients.
