Hands-On Adult Anesthesia Emergency Simulation TrainingFRIDAY, February 19, 2016Managing Complications During Intravenous Sedation and General Anesthesia: A Team Approach to Crisis Management

INTENDED AUDIENCE:

This half-day course is for dental practioners providing moderate sedation (AM Course) and deep sedation/general anesthesia (PM Course). Emergency scenarios will be customized to the type and depth of sedation & anesthesia you provide, and the emergencies you are likely to encounter. Course space is limited to provide each student with a low student-faculty ratio and an optimal hands-on educational experience. We encourage staff to attend with their doctor to get the best team training experience but due to high demand for this course, it is limited to two staff per doctor. It is suggested that the staff be fully prepared by the doctor before coming to the course.

COURSE DESCRIPTION:

This unique highly popular, fast-paced program provides a realistic experience that addresses the assessment and management of complications associated with moderate sedation, deep sedation, and general anesthesia. Emphasis

is placed on “hands-on” training in airway management. Following a brief introductory lecture explaining essential emergency management principles, team skills, and hands-on practice at specific emergency task-training “stations,” participants will then break into small groups to practice emergency management on human patient simulators in real time. Each participant typically participates in 12 different emergency scenarios, and “leads” the management of several scenarios. A debriefing to discuss the efficacy of the interventions will follow each scenario.

All registrants are provided REQUIRED reading materials for study, including treatment algorithms.

Course space is limited to provide each student with a high student-faculty ratio and an optimal hands-on educational experience.

COURSE OBJECTIVES:

• Demonstrate the primary assessment of an anesthesia emergency.

• Understand the various anatomical and pathological causes of airway obstruction, and other forms of respiratory emergencies.

• Understand and demonstrate the recognition and appropriate management of respiratory depression, distinguishing it from airway obstruction, laryngospasm, bronchospasm, pulmonary edema, and other ventilatory emergencies.

• Understand and demonstrate appropriate management of bradycardia, hypotension, hypertension, tachyarrhythmia, etc.

• Understand and demonstrate appropriate management of acute cardiac events and cardiac arrest.

• Understand and demonstrate appropriate management of allergic reactions.

• Understand and demonstrate familiarity with the administration of emergency medications, and the use of intraosseous injection devices.

ABOUT OUR SPEAKER

Doctor Malamed completed a residency in anesthesiology at Montefiore Hospital. He joined the faculty of the University of Southern California School of Dentistry, where he is Professor of Anesthesia & Medicine. Dr. Stanley Malamed is a Diplomat of the American Dental Board of Anesthesiology, as well as a recipient of the Heidebrink Award [1996] from ADSA and the Horace Wells Award from the International Federation of Dental Anesthesia Societies, 1997 (IFDAS).

Doctor Malamed has authored more than 120 scientific papers and 16 chapters in various medical and dental journals and textbooks. In addition, Dr. Stanley Malamed is the author of three widely used textbooks: Handbook of Medical Emergencies in the Dental Office; Handbook of Local Anesthesia; and Sedation — A Guide to Patient Management and

two interactive DVD’s: Emergency Medicine and Malamed’s Local Anesthetic Technique.

COURSE OBJECTIVESMORNING SESSION

Sedation — Minimal, Moderate, Deep: What’s the Difference?

• Understand the accepted definitions of sedation and general anesthesia

• Understand why it is important to understand the level of sedation of the patient, and the implications for treatment planning

• Understand the complications that may occur at different levels of sedation & anesthesia

• Understand why and how patients can slip into deeper levels than intended

• Understand why it is important and how to “rescue” the patient from the level deeper than intended

• Understand why certain sedative/ anesthetic agents are not appropriate for moderate sedation due to their narrow therapeutic window

• Understand the use of appropriate monitoring to prevent patients from getting deeper than intended

as nasal and oral airways, LMA’s, other supraglottic airway devices, and endotracheal tubes.

• Demonstrate the “team” concept in Crisis Resource Management, including the use of “closed loop communications.”

• Understand the risks of “working in the airway” on sedated patients, and how to reduce the risks

• Understand why it is inappropriate to routinely reverse sedation for discharge

• Be familiar with the ADA Guidelines for training for sedation levels

• Understand Florida’s rules and regulations regarding depth of sedation training, management, etc.

AFTERNOON SESSION

Airway Emergencies in the Office

• Understand that the majority of emergencies in office based sedation and anesthesia are airway related

• Understand the different types of airway emergencies and their pathophysiology

• Understand how to recognize and treat airway emergencies: over-sedation, obstruction, foreign body, laryngospasm, bronchospasm, aspiration, anaphylaxis, etc.

• Understand how to evaluate the patients airway pre-operatively

• Develop an algorithm to systematically evaluate the patient and diagnosis the airway problem

• Understand appropriate airway monitoring and management

• Understand appropriate airway emergency management with BVM, and airway adjuncts

• Understand when and how to call for help in the office and what to expect from EMS (not much)

• Understand why the entire dental team must be prepared and trained for anesthesia emergencies in the office even when a medical or dentist anesthesiologist is utilized in the office

• Understand the importance of regular hands-on emergency simulation training for the entire office team, and how to effectively involve your dental team in emergency management

• Understand the use of “cognitive aids” such as emergency algorithms during an emergency
