Congratulations to Carmel Carrigan, who again won the Best Paper by a Nurse at the FSA Annual Scientific Meeting in Sydney. Carmel’s presentation focused on the criteria used by recipients in semen donor selection with interesting insights into the preference of single women, heterosexual and same sex couples. Donna Close, Louise Younger and Carmel Carrigan, had the opportunity to attend ASRM in Boston late last year. Donna’s report on this conference is included in the newsletter. FNA will host 2 national workshops in 2014. The first will be held on The Sunshine Coast QLD 21st June and the second in Hobart 15th November. So please mark these dates in your diary and take advantage of booking flights and accommodation early ,to receive an advantageous deal. I encourage as many of you as possible to register to attend ASPIRE in Brisbane in April. It will provide an opportunity to network with local colleagues as well as collaborate with delegates from the Asia Pacific region. Happy New Year everyone!

I trust you all enjoyed a very happy Christmas and I hope you all got a moment to put your feet up over the festive season.

2013 was another successful year for FNA.

Our goals of providing quality educational experiences for nurses working in ART were again meet.

We were very proud of the workshop attached to FSA. The success of this workshop was due to the hard work and oganisation of my team, Linda Robinson, Sarah Harper and Jo Blake.

With a record attendance , we were entertained and informed by a programme of speakers, covering the theme of Nurturing the Patient, the Couple and the Nurse.

Congratulations to Carmel Carrigan, who again won the Best Paper by a Nurse at the FSA Annual Scientific Meeting in Sydney.

Carmel’s presentation focused on the criteria used by recipients in semen donor selection with interesting insights into the preference of single women, heterosexual and same sex couples.

Donna Close, Louise Younger and Carmel Carrigan, had the opportunity to attend ASRM in Boston late last year.

Donna’s report on this conference is included in the newsletter.

FNA will host 2 national workshops in 2014.

Congrats to carmel Carrigan!Winner of FSA Nurses’ prize 2013

Carmel Carrigan, Lynn Wilson, Allison Campbell, Keith Harrison. Queensland Fertility Group has a very busy sperm donor program. Our donor program sources donor semen from both local donors and international sperm banks which comply with Australian regulations for donor data release, counselling, and expenses reimbursement. Despite our best efforts, we noted that some donors are more popular with potential recipients than others.

Donor recruitment is a potentially expensive and time consuming activity and as such we thought it would be desirable to have a greater understanding of the motivations of recipients in selecting donors in order to better meet the needs of our recipients and contain operational costs.

Potential recipients accessing our sperm donor program were invited to particpate in the study. Participation was optional, all potential recipients were informed that it would not influence donor availability or bias access to treatment in any way. Upon finalisation of donor selection all recipients received an email which they could either return via email or Australia post. Respondents were asked to nominate the three most important criteria employed when choosing the donor. Participants were provided with a number of set criteria including – donor health and medical history, personality, level of donor education, appealing characteristics, matching to partner, matching physical appearance of self, lifestyle and interests, donor talents, occupation, matching parents and siblings physical characteristics. In order to reduce risk of bias participants were invited to nominate any additional reasons for donor selection. Respondents had to nominate their top three choices- one being the most important criteria, 2 the second most important criteria and 3 their third most important criteria for selecting a particular donor. We then compared the criteria selected against the social relationship status of the respondents.

In total we had 67 respondents, with 27 single respondents, 18 respondents were identified to be in same sex relationships and 11 participants in heterosexual relationships. Eight respondents did not identify their social relationship status.

Our data was quite interesting and dispelled some of our preconceived ideas in relation to donor selection and social relationship status. A clear health and medical history was the most important criterion nominated in all groups. This has highlighted the importance of pre- donor recipient counselling in managing recipient expectations with regard to clear donor medical histories. Patients need to understand that a clear medical history does not provide a 100% guarantee of a healthy child nor reduce the possibility of future illness in either the donor or offspring. Other popular categories nominated in all groups were subjective in nature including personality and matching physical characteristics.

Contrary to our preconceived beliefs, matching the partner in the heterosexual group was only the second most important criteria employed in the selection of the donor used. Interestingly the same sex group felt personality was the second most important factor with matching partner relegated to the third most important criteria.

No groups nominated first choices for matching parents or siblings, lifestyle interests, talents, occupation or educational status of the donor. As previously noted we were not surprised that other popular categories were subjective in nature, which highlighted the need for our program to recruit semen donors with a broad spectrum of both physical and personal characteristics to provide patients with a real choice.

Around the states and territoriesNSWNSW have certainly had a very busy last half of 2013.

In July Fertility East hosted a lovely meeting attended by 40 nurses. Dr Chris Copeland presented a fascinating talk titled, “Take home baby: egg number V’s egg quality – a statistical take.”

In November we had a fabulous evening attended by 40 nurses hosted by Bayer at The Pullman Hotel, Homebush. The guest speaker was Dr Marie Currie who spoke on exploring mental health and infertility.

Our last meeting of the year was held at Genea Sydney. It was a great evening with fascinating presentations. Dr Tammie Roy spoke about GAVI, which is an automated vitrification machine that Tammie helped pioneer. Dr Sonal Karia then spoke on cultural aspects of fertility treatment. It was a great interactive session and extremely interesting and educational to hear of how the culture in India impacts on couples fertility treatment.

We would not be able to hold these very successful education events without the very generous support from the FNA sponsors, we are extremely grateful to MSD, Merck Serono, Ferring, Orion, Bayer and Ashmed and our wonderful KAM’s who do so much to help at these occasions.

QlDOn Saturday July 27th, 40 members attended a very successful Workshop titled “Pre-Conception Medicine – Optimizing Conditions for Pregnancy”.

We are again thankful to Kirsty and Cathy from Merck Serono for their sponsorship of this day.

As always we were fortunate to have amazing speakers presenting topics such as:

“The Older Woman- Preparing for Motherhood” – Dr Clare Boothroyd, ACA

‘Moody Blues and Black Dogs” – Mary Williams, Belmont Private Hospital

“Good Glucose Control and Pregnancy” – Annette Parry, Mater Mothers Hospital

It’s always great to get together as a supportive nurses group, not only to understand and appreciate the bigger picture but to share new ideas.

A healthy and delicious lunch and wine and canapés at the end of the day were enjoyed ay all at the new QFG facility.

A great contingency of Qld nurses attended the 2013 FSA conference in Sydney in September. The FNA workshop was yet again a great day of learning and networking for us all. Congratulations to all involved in the planning of the day.

Our End of Year meeting is being held at Tognini’s Restaurant on December 12th with our Xmas themed dinner. Four guest speakers who attended ASRM will give us a Nursing and Scientific update. Thank you in advance to Carmel Carrigan, Lou Younger, Donna Close and Jacqui Irving.

Our Mid-Year Workshop will be held on the beautiful Sunshine Coast at the Mantra Resort, Mooloolaba on 14th June 2014. The theme for our Workshop is “Food for Thought” Our keynote breakfast speaker is the famous 4 Ingredients author and business woman, Rachael Birmingham. We have also secured several other exciting local speakers. We do hope many of our members will take the opportunity to attend this exciting weekend. A great escape to the warmer weather for you all!

Information will be forwarded to all our members in the New Year.

Margie, Carmel and Donna have also been busy with the local organizing committee for Aspire. It would be great to see many of you attend this innovative conference. A first for Australia!

ACTIn October Ferring kindly sponsored our last local meeting for this year.

Professor Robyn Lucas from the ANU spoke to us on Vitamin D deficiency and her area of research in this field which centres around Multiple Sclerosis patients. Robyn provided us with an insight into Vit D deficiency and the relationship to the autoimmune system and how difficult it is to accurately measure Vit D. She gave us advice to give our patients about the amount of sun exposure that is safe but necessary to maintain an adequate Vitamin D level.

Next year we have our Bayer dinner presentation booked on March 4th. We have one speaker booked for next year to date.

Our guest speaker is from ISIS Fertility and is a Fertility Scientist and Prenatal Reproductive Genetic counselor.

We also have sponsorship from Oripro, Serono and MSD for 2014 and plan to have three educational meetings.

Isis Fertility has a new Nurse coordinator Lisa Hamilton Duff. Julia Woods joins us soon in a full time Fertility care nurse position.

TASEducational sessions for our nurses and lab staff have mostly been conducted in the form of in-service education.

The nurses who attended the mid-year workshop in Adelaide have recently presented several topics of interest. The presentation about smoking produced a lot of interest and we are hoping to ask ‘Quit’ to provide us with some more information about how to assist our patients.

Dr Shlomi Barak was at TasIVF as a locum in July and presented an informative session on “Preserving Male Fertility”.

Our nursing staff attending FSA this year are looking forward to this opportunity to expand their own knowledge and return to the unit with some interesting information.

VICOn the 20th July, we hosted our Winter FNA Workshop, sponsored by Merck Serono, our Gold Sponsors. This workshop involved many interesting speakers and offered 6 hours of education for attendees

Then on the 4th October, Ferring sponsored a very successful evening held at Niche Restaurant, in Richmond. The topic for the evening, was Ultrasound in ART and the speakers where Dr Hossam Elzieny, from MIVF and Dr Suguna Gaenesan an Ultrasonologist from WUME. Much discussion was generated from this presentation.

On Wednesday 4th December, we will be hosting our final Xmas event for the year. This will be held at Baden Powell Hotel in East Melbourne. Dr Joe Sgroi, from Reproductive Services at the Royal Women’s Hospital will speak on a very interesting topic titled “Is she fit to have a baby”. Merck Serono will be sponsoring this Xmas function.

SAMSD sponsored a meeting on 8/11/13 titled “A Sexual Health Update” by Frances Whaley from Shine S.A.

The initial presentation was centred around STD’s in the single over 40’s and further discussion focused around sexual health in teenagers , as most of us had teenagers and found it very enlightening.

Our last FNA meeting was held at the Lenzerheide restaurant on 28th December. This was jointly sponsored by Brett from MSD and Alex from Bayer. A special thanks to both for organising such a lovely venue. The topic was “Nutritional Considerations for Fertility” presented by Anne Schneyder a specialist dietician and co-owner of Nutrition Professionals Australia. This was a relevant presentation considering a large percentage of our current patients are overweight. It was quite thought provoking and generated much discussion. The venue and meal were wonderful and was well attended by 27 nurses – wonderful turnout.

All clinics report being busy with very little ease off in work loads over the summer holidays.

Several new nurse have joined the ranks of Fertility Nurses so we look forward to them enjoying the steep learning curve that is Reproductive Health! Plans are to send some of these new staff members to the Start course in May.

There has been a change in the provision of publically funded treatment in the Auckland region with the service now to be provided by all three clinics in Auckland. An actual start date for this has not yet been confirmed by the Health Board.

Clinics are currently looking into attendance at ASPIRE, along with the two FNA workshops that will be held this year.

Looking forward to continuing the ongoing association with our fellow ART nurses across the ditch!

FNA MeetingSydney, 31st August 2013In August Sydney hosted the FNA meeting and NSW FNA put together a day of education on the theme of “Nurture”. The day was attended by 170 delegates and feedback is that it was a very successful meeting. Thank you so much to Jo Blake and Linda Robinson for all your hard work in assisting Jan and I in putting this event together.

We started the day with “Nurturing our Patients” and opened with our guest speaker Tracey Spicer who shared her story of “Fat, Forty and Fired”. It was a fun way to start the day hearing about Tracey’s journey to motherhood and the challenges of females working in the media. Thank you to Bayer for sponsoring the breakfast meeting, its invaluable in allowing us to extend the day of education and achieving 7 CPD points for the day.

Cherie Borosh a Counselling Psychologist gave us an insight into nurturing the couple during fertility treatment.

Liz Machin an Accredited Exercise Physiologist presentation on “Changing habits during a time of change”, had all the delegates attention. Feedback from everyone on the day is that this was a great presentation and they could have listened to Liz all day.

The next session was based on “Nurturing the Staff ”.

Penny O’Donnell, Clinic Manager from Genea Newcastle gave fertility nurses some great advice with her presentation “Managing Your Manager”. It’s always good to see one of our fellow fertility nurse colleague’s present, thank you Penny for a great talk.

Peter Stephenson is Manager, Multichannel Marketing, MSD and Peter spoke on Connecting with patients in the digital age and the modern IVF practice. It’s a topic I know we are all keen to keep up to date with.

Alison McTavish, Nurse Manager College of Life Sciences and Medicine, University of Aberdeen was attending FSA to present and kindly agreed to present at the FNA meeting on how the Senior Nurses Infertility Group functions in the UK and how it supports nurses working in ART.

The afternoon session was all about “Nurturing the Couple”.

We were extremely lucky to have Dr Rosie King presenting on how couples can create a healthy relationship whilst going through IVF.

Our final presenter for the day Tanya Koens certainly had everyone’s attention at the end of the day with her great presentation on “Lets talk about Sex”.

In the evening lots of the delegates headed off for dinner at a Greek restaurant and I believe a good time was had by all.

ASRM reportASRM in Boston was a very large meeting with 11,000 delegates from all over the world. Initially I was in awe but as the sessions progressed it was clear that we were streets ahead in terms of laboratory and clinical programs. A significant amount of lecture time was dedicated to the benefits of minimal stimulation protocols.

There was over a hectare of posters…. Seriously we could not possibly view all of them, there were so many. We did luteal support and some basic andrology rows (see attached photos). Queensland had a large representation of delegates at the conference, there was Jenny and Louise from CFC, Lou, Carmel, and Donna.

The session I appreciated the most was one on improving patient retention and experience. The basic tenants of the talk was reorienting the values of the IVF unit to include, service, innovation, fun, teamwork, quality and care. It was a very honest talk given by a business development officer from an IVF unit in Seattle. I do think customer service and organisational culture is something we could learn from the US units. They do it very well.

Sperm and egg donation centres raised some issues with people donating at multiple sites.

Another interesting group of talks were based around risk management looking at areas of risk in a clinic

• Patient termination

• Disruptive

• Scope of practice issues

• Patient satisfaction - acknowledge mistake, say sorry

• Medical records -Shred Release forms!

• Medication management

• Communication. Patient needs to be told 3 times for it to sink in. Translations

• Documentation. Not written was not done. Objective not subjective NEVER ALTER the medical record

• Call no shows

• Follow up of test results 25% all claims are mis-diagnosis. System in place to highlight follow up of what was sent

Suggestions to minimise risk were designed to improve patient care using

• Clear policies

• Train appropriately

• Be proactive

• Create QIP studies

• Document the informed consent process

• Document EVERY patient interaction
