LAB-WI-3.3 Last Modified: 5/24/2019 Page 1 of 14 Fee Schedule General Policies The following are general policies pertaining to the submission and testing of veterinary specimens at the Oklahoma Animal Disease Diagnostic Laboratory (OADDL). A few exceptions apply and are outlined in the OADDL Standard Operating Procedures (SOPs). Please contact the laboratory at 405- 744-6623 for more information. OADDL charges in this Fee Schedule apply to specimens submitted for diagnostic testing. This Fee Schedule does not apply to specimens that are clearly submitted at the time of submissions for research and test development use. Excluding research submissions that only involve histology, the researcher must coordinate their submissions with individual subject matter experts or project collaborators at OADDL; if this does not occur, the submission will be processed and charged as a diagnostic submission. Animal owners and producers desiring to submit specimens are encouraged to contact their veterinarian. Fees charged by veterinarians usually reflect their time, expertise, expense to collect appropriate samples and ship the samples to OADDL, interpret the results, and implement effective intervention strategies; it is standard for fees charged by veterinarians to exceed the laboratory test fee. Consultation services, legal depositions, court appearances, research support, and work performed for the purpose of product validation or commercialization may incur charges above the fees in this Fee Schedule. The submission of specimens for testing at OADDL constitutes a contract for delivery of test results to the submitter. The submitted specimens, as well as products, isolates, data, and any biologic or chemical material derived from the submission shall be the property of Oklahoma State University. Unused portions of submitted specimens are generally retained for thirty (30) days. OADDL reserves the right to forward specimens to reference laboratories for tests not currently available at OADDL, and utilize subcontractors in the event that the laboratory is unable to perform tests that it currently offers. All work performed at reference laboratories will be noted on the case report. Clients will be charged the actual referral laboratory testing fee and a flat-rate shipping fee (for routine referral laboratory test requests). Sensitive information provided on diagnostic submissions to OADDL will be maintained in strict confidence by the laboratory. Select diseases are considered reportable under current state and federal regulations. When positive test results are subject to state or federal disease reporting, OADDL will release the required information to the appropriate agencies. This may include veterinarian information, owner information, animal identification(s) and any findings on the case. OADDL does not perform cosmetic necropsies, and does not return animal remains to veterinarians or owners. Private cremation services are available by third parties; it is the responsibility of clients to promptly make arrangements for private cremation, if this service is desired. Live animals are not accepted at OADDL. Testing of non-human primate specimens at OADDL is restricted; please call the laboratory prior to submission of these cases. STAT/Rush testing is not available. Please contact the laboratory in advance if expedited testing is necessary.

Fee Schedule General Policies

A Summary of Charges will be attached to the Final Report from OADDL. The Summary of Charges is not an official invoice. A monthly billing statement will be sent from the Oklahoma State University Bursar’s Office.

GENERAL FEES

OADDL does not charge an Accession Fee or Administrative Fee. Reports are emailed (preferred) or faxed at no cost. A $3.00 fee will be assessed to mail reports via

USPS (excluding regulatory papers). Legal and/or Insurance Fee on Necropsy Cases ($150.00)

Extended storage of specimens on OADDL cases will incur an additional fee (specimen restrictions apply). Emergency/after-hours submissions for necropsy and rabies testing ($125.00)

OADDL accepts dead animals for necropsy and/or rabies examination from 8:00 AM – 5:00 PM on weekends, most holidays, and weather-related closure days on an emergency basis. Emergency services are not available on Thanksgiving or Christmas day. The emergency fee also applies to necropsy or rabies examination submissions after 3:30 PM on Friday or the day before a scheduled holiday closure. Rabies testing ($35.00-$50.00); disposal fee applies to animals weighing > 100 pounds. Please note

that actual testing for rabies is not performed at OADDL. This laboratory service entails brain removal and shipment to the Public Health Laboratory at the Oklahoma State Health Department in Oklahoma City, OK for rabies testing. Specimens may be submitted directly to the Public Health Laboratory for quicker results; restrictions apply.

Necropsy charges include gross examination and histopathology. Laboratory testing and disposition of animal remains incur additional fees. Multiple, related neonates (<1 month old), submitted at the same time

DISCOUNTED ABORTION PANELS (fees are capped for testing outlined in the panels below)

Panels include up to 2 (cattle, goat, sheep, horse) or 3 (dog, pig) fetuses on one submission submitted at the same time. If more than one fetus is submitted, samples will be pooled. Testing will be performed on placenta, if available. Select testing is performed at referral laboratories. NOTE: Serological testing on serum from the dam and fluid from the fetus is available at additional cost (exception: Brucella card test in dogs; see below). The submission of acute and convalescent serum samples from the dam is encouraged.

BACTERIOLOGY

Bacteriology fees are levied on a per sample basis unless otherwise specified. Test packages include up to 2 antibiotic susceptibility profiles on organisms selected by OADDL microbiologist. Additional antibiotic susceptibility profiles performed at submitter’s request will be charged at the routine testing rate. The urine package below does not include free-catch samples.

HISTOPATHOLOGY

Biopsy accessions will be reported in short or long (detailed) report formats. The short report format consists of the diagnosis along with interpretative comments regarding the biologic behavior and tissue margins. The long report format includes an additional detailed description of the microscopic changes. The default reporting format will be short report unless requested otherwise. Histochemical staining (special staining) and tissue decalcification are included in the histopathology fee.
