The prevalence of diagnosis of hypertension in children and adolescents is increasing. This is due in part to obesity among children ages 2 to 19 years. The rate of childhood obesity over the past 10 years remains at 17% (or 12.7 million children.) Childhood hypertension usually persists into adulthood. The presence of childhood hypertension may contribute to the early development of coronary artery disease.

According to the Fourth Report on the Diagnosis Evaluation and Treatment of hypertension in children and adolescents (2004), hypertension is defined as “systolic blood pressure or diastolic blood pressure that is at or above the 95th percentile for sex/age/height on three separate occasions.” Blood pressures between the 90th and 95th percentile in childhood is classified as “pre-hypertensive.”

Children over the age of 3 seen in the medical setting should have their blood pressure measured at least once during every health care visit. If blood pressure is elevated on three separate occasions an evaluation by a pediatric cardiologist is recommended.

Unlike adults, most children don’t have an underlying pathology for high blood pressure, however, some children have structural pathology that should be addressed. We have seen children with pheochromocytoma, renal artery stenosis, renal pathology and coarctation of the aorta. Our expertise is to evaluate and treat these possible causes of hypertension.

Here at Tri-City Pediatric Cardiology, P.C., we will take a detailed health history, perform an echocardiography to rule out structural heart disease as a possible cause, order

comprehensive lab testing, and can order more in-depth testing such as renal ultrasound/duplex doppler or abdominal MRA for further workup.

We offer 24-hour ambulatory blood pressure as an option for diagnosis. We also introduce weight management counseling, the importance of physical activity, and adopting better eating habits. We recommend cutting sodium intake and caffeine. Having the patient monitor blood pressure and weight at home helps the practitioner to manage care and encourage the patient to be accountable for their actions.

If hypertension persists, we can initiate pharmacological treatment and manage the patient as well. Tri-City Pediatric Cardiology, P.C. can see patients up to the age of 25.

Choosing the right doctor is an important decision, not to be made simply on the basis of insurance coverage, or solely upon a list of physicians provided to you by your insurance company. A far better way to select a physician is to choose a physician board certifi ed in his/her fi eld, thus ensuring additional levels of training, current knowledge and the highest degree ofpeer-based testing and scrutiny.

All physicians are licensed, but not all are board certifi ed. What does it mean to be board certifi ed?

Board certifi cation is a formal recognition of the highest qualifi cations in a chosen medical fi eld, and is given only to physicians who have passed examination before specialty boards comprised of their professional peers. Board certifi ed physicians have been highly trained and tested in their areas of medicine.

A board certifi cation examination is a stringent exam given to a physician to test skill, knowledge, ability and medical judgment.

Board certifi cation is not only scientifi c testing

regarding medical knowledge; it includes medical judgment, performance and practice standards.

Some physicians who are board certifi ed become board certifi ed in a further subspecialty. Medical specialty certifi cation in the United States is a voluntary rather than mandatory process. The certifying organization is called the American Board of Medical Specialties (ABMS); an umbrella organization overseeing the 24 approved medical specialty boards in the United States.

Physicians with specialty credentials have completed an approved and monitored ACGME (Accreditation Council for Graduate Medical Education) training program after going to medical school, and they have taken certifi cation examinations to establish their profi ciency in a particular specialty of medicine.

Obesity is a global epidemic affecting people of all ages. According to The Centers for Disease Control and Prevention, more than one-third of Americans are considered obese, while Statistics Canada notes about 25 percent of Canadians are obese. Being overweight or obese has been linked to cardiovascular disease, diabetes and some forms of cancer, and children who are overweight are likely to be overweight or obese adults.

While eating a diet that is low in fat and high in whole-grain fiber, vegetables and lean proteins is important, exercise remains one of the key factors in maintaining a healthy weight. Many adults and children, however, simply do not get enough exercise during the day. An active lifestyle is often hampered by the prevalence of digital devices or sedentary jobs that encourage more screen time than fitness time.

But there are ways for the entire family to get fit. Exercising together is not just a great way for families to get healthy, but also a great opportunity to spend time together.

The following are a handful of ways families can work together to incorporate more exercise into their daily lives:

• Tacklechores. Yes, tasks around the house can count as exercise. Working together not only gets you moving but also cuts down on the time it takes to tackle tasks around the house. Mowing the lawn, vacuuming the rugs, sweeping the floor - all of these activities can get the heart pumping. Make it fun by setting chores to music or competing against one another to see who gets their task done first or the best.

• Competeinthepool. Swimming is a low-impact activity that works many muscles in the body. Taking a dip in the pool can be both relaxing and beneficial to your health. Compete with family members to make the most of the time spent in the water. Host relay swimming races, see how long each member of the family

• Createanoutdoorobstaclecourse. Set up a series of obstacles in your yard and host your own triathlon. You can jump through tires, climb through tubes and scale trees. Make it fun by having adults try to ride tricycles or pair up an adult with a child for sack races. The opportunities for fun are only hampered by your imagination.

• Goforfamilybicyclerides. Traverse your neighborhood or blaze new trails by riding bikes together. Young children can be strapped into trailers that are pulled behind the bike or ride in seats attached to the bike itself. This gives everyone a chance to enjoy the great outdoors and hone their cycling skills.

• Gohiking. Hiking is another fun family activity that also happens to make for great exercise. Choose a trail that may not be level and push everyone’s endurance to the test.

• Hitthedancefloor. If you’ve been invited to a wedding or a Sweet 16 party, cut a rug and take advantage of the DJ and dance floor. When enjoying yourselves, you probably won’t even realize how much you’re exercising.

Contrary to popular belief, adopting a healthy lifestyle is not a difficult undertaking. In certain instances, convenience may need to be sacrificed in favor of nutrition, but many people find that living healthy is not nearly as difficult as they assumed it would be when they initially decided to make a change.

When men and women decide they want to start living healthier, many mistakenly assume they must abandon their existing habits entirely and start from scratch. But the following are some easy ways to start living healthier every day.

• Eatmorefruitsandvegetables. One of the best and easiest ways to live healthier is to begin eating more fruits and vegetables. Instead of unhealthy snacks like potato chips and cookies, snack on a piece of fruit, and never sit down to a meal unless you include some vegetables to go along with the main course.

Studies have shown that men and women whose diets are high in fruits and vegetables are less likely to develop certain types of cancers, including cancers of the digestive tract. In addition, the United States Department of Agriculture notes that people

whose diets are rich in fruits and vegetables have a lower risk for heart disease, including heart attack and stroke.

• Slowdownyoureatingroutine. Some people may already be eating the right foods, but they may just be eating too much of them. It takes time for your body to let you know it’s had too much to eat, so eating too fast can increase your risk of overeating.

While eating, try to limit distractions that can take your attention away from how much you’re eating. If you’re always watching television or checking emails on your phone while eating, try a few days of distraction-free, careful eating and you may find yourself eating less and feeling more energized after a meal.

• Skipthesecondglassofwine. The much publicized medical benefits of wine are somewhat misleading. According to the Mayo Clinic, when consumed in moderation, red wine can help prevent heart disease. That’s because alcohol and antioxidants found in red wine have been shown to increase levels of high-density lipoprotein, often referred to as “good” cholesterol, and protect against artery damage. But wine also contains sugars that can fatten the liver, and a fatty liver can contribute to a host of serious health problems.

If you already drink wine, limit yourself to one glass per day. If you are not a wine drinker, then it’s important to note that many doctors believe the potential benefits of drinking wine do not outweigh the potentially negative consequences associated with alcohol consumption, which include neurological problems and an increased risk for heart disease.

• Getmoresleep. Inadequate sleep affects the body in a variety of ways. Many people are aware that one poor night’s sleep is certain to affect their energy levels the following day, but fewer may know of the link between sleep duration and chronic disease. For example, the Harvard Medical School notes that studies have linked insufficient sleep to type 2 diabetes, as the body’s ability to process glucose can be compromised by poor sleeping habits. Other medical conditions that have been linked to insufficient sleep include obesity, heart disease and mood disorders.

While you might be proud of your ability to function on minimal sleep, the long-term effects of insufficient sleep can be dire, so be sure to get at least seven hours of sleep each night.

Choosing to live healthier does not mean you need to completely overhaul your existing lifestyle. In fact, you can make several easy everyday changes to dramatically improve your overall health.

Quillen ETSU Heart is a fi ve physician cardiology group located in Johnson City. They are dedicated to providing the highest quality cardiovascular care to the people of East Tennessee and the surrounding region.

“One of my goals is to develop a Heart Failure Disease Management Program for the Tri-Cities area.” Said Dr. Steve Lavine, Professor of Medicine, Chief, Division of Cardiology Quillen School of Medicine. “I am emphasizing a dedicated program to educate patients in dietary and fl uid management, assist patients in the optimal use of their medications, and provide access to heart failure care as needed to reduce hospital admissions and improve quality of life. This program is directed and managed by physicians who are assisted by qualifi ed nurse practitioners.”

One in every 20 Americans over age 50 has peripheral arterial disease. Although peripheral arterial diseases are highly prevalent, only less than a third of patients present with typical symptoms. The American Heart Association and American College of Cardiology recommends screening asymptomatic patients 50 years and older with a history of smoking or diabetes.

“Our main goal of this program is to provide education to patients on heart failure and prevent exacerbations or ‘fl are ups.’ We treat patients acutely in the clinic to hopefully prevent hospital admissions.

Our disease management program will assure increased access to the heart failure cardiologist and optimization of drug therapy, patient and family education and counseling, emphasis on self-care, vigilant follow-up including discharge planning from the hospital, attention to signs of fl uid overload, and coordination of care with other physicians.”

The basic component of this program is communication between program visits and telephone contact between the nurse practitioner and patient. This ensures self-care, monitoring medication use and access for answers to their questions. This program is expected to reduce admissions for heart failure, potentially reduce mortality, improve exercise tolerance, and increase the use of guideline-directed medical therapy and other core measures for heart failure.
