Welcome to the Department of Family Medicine, Alberta Health Services—Calgary Zone. We are pleased to have you join our community of family physicians providing family medicine care to the citizens of Calgary in the community.

As a department we seek to serve, support and strengthen family physicians in delivering care across all age groups and locations of service. I hope you will take the time to familiarize yourself with and take advantage of some of these services. Feel free to contact us at any time with your practice needs.

There are a variety of practice options with in Calgary including the opportunity to expand your work into areas of special interest, including Hospitalist work, teaching and leadership opportunities and long term care can enhance your enjoyment of your practice. The Department of Family Medicine and its Community Leaders work to enrich the environment in which family physicians practice and enhance the quality of primary health care delivered in the Calgary Zone. The Community Primary Care Section aims to improve Primary Health Care delivery by: Collaborating with the Calgary Zone Primary Care Networks (PCNs) (www.mypcn.ca) Advocating for, developing and implementing services which support family physicians and a Medical Home Model of primary care. Enhancing communication among community-based family physicians and developing a better understanding of issues that affect them. Ensuring that the family physicians’ perspectives are included in the development and implementation of Alberta Health Services programs and initiatives. Supporting family physicians to enhance their practice and develop both professionally and clinically. Advocating for patient safety in community primary care settings. Overseeing privileging for Day Medicine, Surgical Assisting, and Specialized outpatient clinics such as the East Calgary Family Care, the Sexual & Reproductive Health Program and the Elbow River Healing Lodge. Our leadership team encourages you to connect if you have any questions or concerns regarding Family Medicine in Calgary. The Community Primary Care section supports the following services to enhance community family practice: Offers a connection for family physicians to Alberta Health Services. Collaborates with the University of Calgary and Calgary Zone Primary Care Networks to provide

appropriate Continuing Medical Education (CME). Communicates with Family Physicians via our Bi-weekly DFM HomePage Newsletter and our website. Urgent Care Urgent Care facilities within Alberta Health Services – Calgary are non-hospital, unscheduled care facilities established by Alberta Health Services in Alberta to meet the urgent medical care needs of patients not requiring hospital-level care. An urgent care centre receives unscheduled patients who are seeking attention for non life threatening unanticipated health events. These may require human and technical resources more intensive than are available in most physician offices. Opportunities for Family Physicians: Physicians must be ACLS certified and experience in Emergency or Urgent Care is preferred. Opportunities for work include: full time, part time and casual. All work is shift work and includes weekends, evenings and holidays. There is a minimum shift commitment which varies from site to site. Remuneration: Varies from site to site. All sites are Fee for Service and some sites provide minimum guarantees.

The Palliative Care Section consists of a group of 30 Physicians, who provide consultative palliative support to palliative patients across all service areas in the Calgary Zone. The Palliative Care Physician Consultants come from a variety of backgrounds (including Family Medicine, Internal Medicine, Critical Care and Cardiology). In addition, all physicians (primarily Family Medicine) who work in a Calgary hospice, are part of the Palliative Care Section.

Dr. Aynharan Sinnarajah is the current Palliative Care Section Chief (Department of Family Medicine) and is also Medical Director for the Palliative / End of Life Care program of Alberta Health Services – Calgary Zone.

Consultation is provided in all urban and rural parts of Calgary zone. Sites include acute care hospitals, home care, hospices, long-term care and supported living facilities. There’s also an inpatient palliative care unit at the Foothills Medical Centre, where tertiary level patients needing complex interventions are admitted, under the care of a Palliative physician. On the educational side, the Palliative Care physician team supports all family medicine residents, in a mandatory rotation through the program every year. Other specialty residents also rotate through our service. These residents can come from community medicine, critical care, radiation oncology, medical oncology, haematology, anaesthesiology, neurology, nephrology, psychiatry and gynaecological oncology. There are also 1-3 palliative medicine fellows who spend time with the palliative care team, every year. Hospitalist Program The Program provides high quality responsive and timely care for patients requiring admission to an acute care facility. As the Most Responsible Physician (MRP), team members play a valuable role in the delivery of family medicine level care to unattached medical patients requiring hospitalization. Scope The patients receiving inpatient care under the Calgary Hospitalist Program are generally elderly and the top 6 Case Mix Groups (CMG) are currently: Dementia, Chronic Obstructive Pulmonary Disease (COPD), Pneumonia, heart failure, lower Urinary Tract Infection (UTI) and Gastrointestinal (GI) Haemorrhage. The combination of these conditions with the advanced age and of the patient population gives rise to a complex and demanding service population, care requirements and resources.

The program’s mandate is to provide continuous 24 hours per day/7 days per week in-house hospital care to patients at each adult acute care hospitals in Calgary. The Maternal Newborn program of the Department of Family Medicine is an active division of over 125 physicians. These physicians provide up to 50% of initial prenatal care, and are responsible for up to 40% of the deliveries in the Calgary Zone. We work within a dynamic multidisciplinary care model with close interaction with our obstetrical, anaesthesia, nurse and midwife colleagues. We are passionate about providing comprehensive, quality intrapartum care in a supportive environment.

All four adult hospitals provide antenatal, intrapartum and postpartum care – Foothills Medical Center, Peter Lougheed Center and Rockyview Hospital and South Health Campus.

There are many models to incorporate obstetrical care into your practice:

Low Risk Clinics - provision of antenatal care at independent clinics and shift based hospital care.

As Part of Family Practice - provision of antenatal care as practice and join into call groups of varying sizes and patient volumes.

As part of a Primary Care Network - provision of other new and innovative models are constantly emerging.

Physician Extender Shifts - available at some of the hospitals, allowing family physicians to provide care assisting the specialist obstetrician in outpatient, emergency and postpartum assessments as well as intrapartum care.

How do I know if I am not eligible?

Do you work in a program within AHS (ie. Hospitalist, PCN inpatient program, GARP, others with AHS contract service)? If yes, you are NOT eligible

Do you provide clinical service on a scheduled bases? Scheduled activity is considered being present or likely present at the site for an expected duration (ie shift) to perform clinical service for a patient or group of patients. This could include scheduled surgical assisting shifts, low risk obstetrical shifts for a given call group. If the clini-cal activity is scheduled, then you are not eligible.

Opportunities for Family Physicians in Primary Care Networks

What is a Primary Care Network? Primary Care Networks are a made-in-Alberta approach to improve access to, and better co-ordinate primary health care for, Albertans province-wide. In a Primary Care Network, a team of health professionals led by family physicians delivers and co-ordinates health services, resulting in better collaboration, more timely referrals and more comprehensive care. A Network can be comprised of one clinic with many family physicians and other health professionals (e.g. nurse practitioners, dieticians, pharmacists, social workers and mental health professionals), or several family physicians in several clinics in a geographic area. Each Network has the flexibility to develop programs and provide services in a way that works locally to meet the specific health needs of patients. Networks are established within the framework set by Alberta Health and Wellness (AHW), Alberta Medical Association (AMA) and Alberta Health Services (AHS).

Message from our Academic Department Head

The University of Calgary Department of Family Medicine has a large and vibrant Residency Program. We are responsible for training over 180 Residents in the Calgary-based two year program.

Our Residency program has a strong rural component in all levels of teaching. We have 28 residents per year in our Rural-based program. Residents graduating from our program are well prepared to practice either rural or Calgary family medicine.

We offer third year programs in several areas of enhanced skills including Emergency Medicine, Sports And Exercise Medicine, Palliative Care, Care of the Elderly, FP Anesthesia, Addiction Medicine, Global Health and Maternal Newborn Care. An important component of our program is the extensive involvement of community Family Physicians in the training of residents, clerks, and medical students. The teaching opportunities document (attached to this document) provides an outline of the teaching opportunities and remuneration for preceptor, teacher, and examiner roles available to practicing Family Physicians. The Department is always recruiting interested individuals as preceptors/teachers, and examiners.
