WHAT IS HOSPICE CARE?

Hospice care is comfort-focused care for patients

with a life expectancy of six months or less, no

matter the illness. St. Joseph Family Hospice

focuses on managing symptoms, relieving pain

and improving the quality of life.

“We had no idea what Hospice really does for its patients and were enlightened by your vast knowledge, compassion and support in all that you do. We’re not sure we would have been able to continue home care without you – in fact we KNOW we couldn’t.”

WHAT CAN YOU EXPECT?

• Regular visits for assessment and

care coordination

• On-call nursing support 24 hours a day,

seven days a week

•Medications related to comfort and

symptom management

•Pain and symptom management

education and coaching

•Home medical equipment for comfort

and safety

•Holistic support from nurses, social workers,

chaplains, volunteers, personal care aides,

and specialty therapists for counseling and

guidance to resources

•Access to our free, comprehensive

bereavement program
