    Improving Outcomes for Homebound Elderly

    I am honored to become the new President

    and Chief Executive Officer for Home Health &

    Hospice Care (HHHC). This proud organization has

    responded to important needs of individuals in

    our communities for over 130 years. During this

    time our services have become broader and more

    sophisticated. We now provide home health and

    hospice services in 25 communities in southern

    New Hampshire, in homes, nursing homes and in

    the Community Hospice House, annually caring for over 5,000 individuals of all

    ages. In addition, we offer bereavement support groups, wellness education and

    preventive care, such as diabetic foot clinics. Last year alone, we served 3,402

    individuals at our affordable foot clinics.

    Our success has been the result of staff and volunteers who demonstrate a passion

    for what they do. They are clinically sound and compassionate towards those they

    serve. Our organizational culture is driven by the many volunteers who give their

    time and energy helping others in a variety of capacities, from sitting vigil at the

    bedside to working in the gardens at the Community Hospice House. Often our

    volunteers have personally been touched by their own past experiences within

    the HHHC organization. They value the impact we make in the lives of patients in

    need and want to help others as they themselves have been helped. Our volunteer

    Board of Directors consists of 20 individuals from our communities who give of

    their time, treasure and professional experience to help ensure the continued

    viability of this critical and beloved community resource.

    According to research, the percentage of homebound elderly

    who suffer from depression and/or anxiety can be as high as

    46%. The condition can be made worse by a medical event,

    surgery, a new diagnosis or even a fall.

    On July 1, 2014 Home Health & Hospice Care (HHHC) introduced its new Behavioral Health Program to address this growing concern. HHHC’s Home Care Director, Lucy Saia, has known this was a necessary addition to the agency for quite some time. “I realized when looking at our patients who have many chronic illnesses that it is a permanent lifestyle adjustment. It is unlike a patient that has surgery and can return to a previous routine.” Lucy continued to explain, “Although these illnesses can be managed with medication, as well as other forms of treatment, they do not ‘go away’. Often, people develop depression from dealing with their diseases and the lifestyle changes that accompany them. I could see this first hand as a visiting nurse, but it was more evident when I saw a patient’s health decline due to the effects of depression.”

    By treating the emotional and behavioral symptoms that are often triggered by a physical illness, the quality of life for both patients and their families improve, unplanned hospitalizations are reduced and compliance with treatment increases. HHHC’s team is specially trained in both behavioral health and medical symptoms. They work in partnership with the patient’s primary care physician to improve outcomes of treatment and maximize independence. “I believe the Behavioral Health Program has given our staff a new view on how chronic diseases impact the patient’s and family’s

    lives. Our staff can now properly assist our patients in identifying how they perceive their illnesses and establish personal goals to work together for improvement,” Lucy discussed. “Once we’ve identified those patients who show symptoms of depression, we can work with their providers to better manage these concerns.”

    Moving forward, HHHC will assess all patients with chronic illnesses for mental health issues. The vast majority of home health patients seek help with their primary care provider. Home Care staff is in a unique position to identify if a patient has any of the most commonly seen disorders such as depression and anxiety. By identifying these disorders, the proper integration of treatment can be put in place. Managing the individual entirely allows for better patient outcomes which is achieved through medication management, counseling and cognitive interventions.

    Numerous healthcare professionals noticed the decline in health in patients who were dealing with chronic illnesses and recognized the importance of Behavioral Health Programs. The benefits linked to this program, or similar programs, are profoundly effective. Lucy concluded, “I think patients and families will see that this program goes hand in hand with all of our services. You cannot separate the medical from the behavioral, and they both need to be treated to improve patient’s and family’s lives.”

    Reasons to RememberWhen people described Jane Heineke, there were numerous things to be said. She was kind hearted. She was passionate. She was civic minded. While all of these statements are accurate depictions of Jane, she was best described as having high integrity.

    Despite her extensive experience and education as a registered nurse, Jane’s most natural quality was her devotion to helping others. Janice McDermott, HHHC’s

    previous Hospice Program Manager, spoke fondly about Jane. “She was an outstanding citizen. She was part of the state census; she was a Salvation Army bell ringer, and even a docent at the Currier Museum. Throughout the course of her nursing career she was a junior instructor, an on-call nurse and worked 9+ years in a cardiac intensive care unit to name a few.” Janice continued to describe Jane as being duteous based on her work and volunteer background.

    “In her mind, what benefited the community or cause was most significant.” Jane came to HHHC years ago through the Neighbor-to-Neighbor clinic and decided to take the hospice volunteer training program. Jane’s personality as a volunteer, according to Janice, was exacting. “Jane needed to know the exact reason she was doing something and it had to make complete sense to her.”

    For the past 16 years Jane was an invaluable component to the Community Hospice House, as well as the agency. Tanya Prather, HHHC’s Volunteer Coordinator, described Jane, “She was passionate about everything. Her drive for learning greatly contributed to her volunteer work. She had a true sense of serving others.”

    Jane spent her time as a volunteer sitting vigil, visiting the nursing homes, driving patients to doctor’s appointments, sitting for friendly visits with patients, and most often, spending her weekends at the Community Hospice House. Tanya added, “She was a dynamic, wonderful, unique individual who made a positive impact on our patients, families and staff.”

    In July of 2013, Jane spent her own last few days at the Community Hospice House. Even after her many remarkable years of service, Jane continued her generosity by leaving a very generous bequest to HHHC. Jane’s passion for serving others and her undeniable advocacy shines through this gesture. Although she is no longer physically with us, she has forever left handprints on our hearts.

    Honoring Irene Bouchard

    Each year, Home Health & Hospice Care (HHHC) receives numerous donations from individuals the agency has been fortunate enough to impact. Steve Bouchard, the son of a former patient, wanted to do something meaningful as a memorial to his father, Robert, and to honor his mother, Irene. To commemorate Steve’s retirement, the Comcast Corporation made a generous donation to the Community Hospice House in honor of his parents.

    During the winter of 1999, Robert Bouchard was admitted to the Community Hospice House. After his passing, his wife Irene struggled with her lifestyle as a widow. She reached out to HHHC’s bereavement programs and rediscovered the balance her life was missing. Irene explained, “My husband was diagnosed with lung cancer in November of 1998 and passed away that March. It was a quick transition and HHHC helped me adjust.” Deb Pelletier, HHHC’s Bereavement Coordinator, shared her

    involvement with Irene. “She came to HHHC looking for support and her positive experience resulted in her leading a couple of our bereavement groups. Irene was an absolutely wonderful addition to our programs.”

    Even though Irene had worked as an RN in a hospital setting, her most meaningful experiences came as a result of signing up for HHHC’s Hospice Volunteer Training Program in 2001. Irene explained, “Volunteering at the Community Hospice House presented me with a new purpose. My children aren’t primarily in New Hampshire so when my husband died, my weekends filled with family no longer existed. I found most comfort when I was volunteering at the house.”

    For seven years, Irene spent her weekends at the Community Hospice House sitting at the front desk answering phones and getting to know the staff, patients and families. “Since my family-life was no longer readily available, working at the Community Hospice House grounded me,” Irene shared.

    When Irene eventually moved to South Carolina, continuing to volunteer for hospice was a necessity. Through her church, she found a school that worked with hospice patients. Irene explained her on-going experience as a volunteer in South Carolina, “While they don’t have a hospice house, it’s still a way for me to volunteer for something I am passionate about.

    Irene returned to the Community Hospice House for a brief visit in July of 2014. She walked through the doors of the Community Hospice House and felt right at home and took a minute to share, “It was a bittersweet feeling to walk back into the house. I have such strong connections to the staff and to this community.”

    Because of the generous donation to the Community Hospice House, Irene had the opportunity to see her family’s name on the Tribute Wall. “I was so grateful Steve thought of this agency and remembered how much this place means to me. It is the most heartfelt gift anyone could provide.”

    Irene reminisced about her weekends spent at the Community Hospice House during her visit in July of 2014.

    Golf Tournament RecapThe sun shined over the dew covered greens of

    Sky Meadow Country Club on Monday, May 19th as

    Home Health & Hospice Care (HHHC) greeted their

    23rd Annual Golf Tournament guests!

    128 golfers gathered to play and help support HHHC’s Pediatric and Home Care programs. The weather was nearly perfect as the golfers began their morning with mimosas donated by Horizon Beverage Company and breakfast on the clubhouse patio before teeing off at the 8:30am Regency Mortgage ShotGun Start.

    For the past 23 years HHHC’s golf tournament has supported its Pediatric and Home Care services. The Pediatric service of HHHC tends to young patients with serious illnesses such as cancer or heart ailments and educates families on providing the best possible care for their sick child.

    Throughout the day, golfers enjoyed the sunny weather while competing in friendly competitions such as Closest to the Pin, Hole-in-One and Closest to the Line to name a few. The day continued as golfers and guests enjoyed a complimentary scotch tasting and mingled at the Bellwether Community Credit Union’s 19th Hole Reception. During the Merrimack County Savings Bank BBQ Dinner, guests listened to heartfelt and inspiring stories from HHHC’s Pediatric Nurse, Emily Sindoni, who was accompanied by Brock Cruz, a nine month old Pediatric patient. Kevin Slattery and Paul Eason were influential auctioneers for both our silent and live auctions throughout the afternoon.

    Several silent and live auction items were bid on as donors and sponsors alike showed their support for HHHC. HHHC’s Development Associate, Paula Telage explains, “Most people know that HHHC provides exceptional care to our elderly neighbors. What they do not realize is that HHHC also has phenomenal home care and pediatric care programs. Our exceptional staff provides outstanding care to all who desperately need our help in our community from birth to the end of life. It is only with the generosity of individual donors and the business community that we are able to provide this extraordinary care. This is our 23rd Annual Tournament and we are so very grateful to our Gold Sponsors Cityside Management Corp. and WZID, Silver Sponsors Associated Radiologists, P.A., CGI Business Solutions, Atlantic Charter Insurance Company and The Harbor Group, along with many other wonderful sponsors, golfers, guests and auction donors who made this tournament such a huge success!     Lyra Evans: One Year Later

    Regardless of Lyra’s health concerns, she still makes room to play even when Emily visits.

    During the Spring of 2013, little Lyra Evans made her debut on the cover of Home Health & Hospice Care’s (HHHC) Human Touch Magazine. In 2011 Lyra was diagnosed with pulmonary vein stenosis, a congenital malformation where the pulmonary veins are narrowed. Lyra’s parents were told after this diagnosis her life expectancy would only be a few months. By August of 2012, Lyra had outlived her physician’s prediction and moved from HHHC’s hospice care to HHHC’s pediatric home care. Over a year later, Lyra Evans is still defying the odds.

    Lyra has since undergone another cardiac catheterization to dilate her narrowing pulmonary veins and has continued to do well. She continues to grow, eating more food by mouth, even though most of her nutrition is still achieved through a Gtube. Along with the diagnosis of pulmonary vein stenosis in 2011, Lyra was also diagnosed with heterotaxy syndrome which is a disorder that results in certain organs forming on the opposite side of the body. After careful consideration and observation, Lyra’s cardiac team felt she could tolerate open heart cardiac repair surgery in 2014. On May 27, Lyra underwent multiple repairs for her congenital heart defects and a permanent pacemaker was put into place. Lyra’s doctors repaired both ventricles of her heart with bovine pericardium and placed an artificial valve where a stent was removed. HHHC’s Pediatric Nurse, Emily Sindoni, explained, “Her condition is no longer terminal. After her complicated surgery, I made visits to assess her vital signs and incisions and to monitor her for signs of infection or potential complications.” Emily continued, “The most exciting thing for me was to see her pink little fingers and toes, which have been blue since I first met her at 14 months old.”

    While Lyra continues to have many ups and downs, her condition is far better now than ever. In 6 months, another catheterization will be in done, followed by frequent visits by her nurse Emily. HHHC has also provided a Physical Therapist to help Lyra regain leg strength since her surgeries. HHHC is thrilled to see Lyra’s condition improving every day and wanted to share the happy news with our readers.
