Parental Permission for Cheerleading
If selected, my child,___________________________________, has my permission to be a cheerleader at Fairhope Middle School if she is selected. I understand that she must abide by the rules and regulations set forth by the coaches and the principal of Fairhope Middle School. I understand that my child and at least one parent/guardian must attend the Try-out meeting on Thursday, February 12, 2015 @ 6:00 and that ALL required forms must be turned in by February 27th or my child will not be allowed to try out for cheerleading. I understand that my child must attend the Try-out Clinic to be considered for a position on the squad.
I understand that qualified judges will evaluate my daughter, and we agree to abide by the decision of the judges.
I understand the financial obligations involved if my child is selected to the squad. I understand the very nature of the activity; cheerleading and gymnastics carry a risk of physical
injury. No matter how careful the participant and the coach are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. I understand the risk and will not hold Fairhope Middle School or any of its personnel responsible in the case of accident or injury at any time.
