The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assignedbyGarfield County Building Departnientposted'where readilyvisible from access road;.
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windowsinstalled, a complete kitchen with cabinets, a sink with hot & cold running.water, non - absorbentkitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, allnecessary plumbing;
4. A complete bathroom, with washbowl, tub or shower, toilet stool, hot and cold running water,,non- absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) Steps must have handrails, guard rails on balconies ordecks over 30" high constructed to all 1997 UBC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration ofextenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued untilall the required items are completed and' a final inspection made.

This letter is in regards to the conditions of Garfield Co. Building Dept for the C.O. of 0058 Peach Ct.New Castle CO. 81647, also known as the Domalik residence.
4 Seasons Home Sales subcontracted the excavation, back fill, final grade, and drainage of the aboveaddress on May 30, 2003 to Rick Bertolini, owner of Dig It Excavation. The excavation was done in atimely matter and he agreed to return to finish the contract if paid in full.
4 seasons agreed to do so if Rick signed a subcontractor's affidavit. That affidavit has been signed,therefore, 4 Seasons has paid the amount of $4800.00 (four thousand eight hundred dollars) in full.
Due to the variance application process and the meeting times of the Garfield Zoning Board ofAdjustment, the back fill and final grade of 0058 Peach a. was held off until late December and wastoo frozen to get to final grade. The home owner and the county inspector have given the owner of DigIt Excavation Company, Rick Bertolini, until April 30, 2004 to complete his contract.
4 Season has spoken to Rick on Jan. 2, 2004 at 11:34 pm regarding this deadline and Rick said "Noproblem, it will be done." It was also explained to Mr. Bertolini that as part of the conditions for theC.O.of this residence an additional signed letter and affidavit would be required from Mr. Bertolinistating as soon as the frost lifts the job will be completed on or before April 30, 2004.

Plans to be included for a Building Permit, must be on drafting paper at Least 18 "x24" anddrawn to scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides withdecks, balcony, steps, handrails and guard rails, windows and doors, including the finish grade line.A section showing in detail, from the bottom of the footing to the top of the roof, including re -bar,anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation, sheeting, house -rap, (which is required), siding or any approved building material.

A floor framing plan, a roof framing plan, roofmust be designed to withstand a 40 pound per squarefoot up to 7,000 feet in elevation, an 80 M.P.H. windshear, wind exposure B, windload of 15 poundsper square foot, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met oryour plans will be returned.
All plans submitted must be incompliance with the 1997 UBC, UMC and 1997 UPC.
1. Is a site plan included that identifies the location of the proposed structure or addition anddistances to the property lines from each corner of the proposed structure(s) prepared by alicensed surveyor and has the surveyors signature and professional stamp on the drawing?Properties with slopes of 30% or greater must be shown on the site plan. It has been brought to the attention ofthis office that a concrete foundation has been poured without abuilding permit on property owned by you at the above address. Further, a review ofthe plans that weresubmitted for a building permit application reveals that the building position on the lot is in violation ofcounty set -back regulations. The setbacks at Mountain Shadows subdivision are 25 feet front and rear and10 feet on the sides. Any construction that has taken place out side of these setbacks must be removed. It isa violation ofColorado Revised Statutes 30 -28 -124 & 124.5, zoning regulations, to build outside of propertyline setbacks. It is a violation of Colorado Revised Statues 30 -28 -205 & 210, Uniform Building Code, tobegin construction and pour concrete without a permit.
You are hereby given notice, pursuant to the above statutes, that you must completely correct the violationswithin thirty (30) days ofthe date this notice is received. Please be advised that violation ofthe abovestatutes is a misdemeanor crime punishable by a fine ofnot more than $100.00 or imprisonment for not morethan ten days, or both, and that each day that the violation continues is deemed to be a separate offense.
