Cancer Risk

Mammography is controversial, politically and emotionally charged - what is its role in screening for breast cancer? Some comments to consider follow:

1) Overdiagnosis of breast cancer is about 25%

2) Biennial vs annual screening has an impact was for obese women who had larger tumor size with biennial screening than annual screening

3) Annual screening was not beneficial in women of normal weight

4) Women at higher risk for breast cancer should seek advise about screening before 40

5) Some studies did not find an effect of mammography on total cancer mortality (including breast cancer) at 10 years

6) Numbers of lumpectomies and mastectomies were significantly larger in groups who were screened.

7) Finding suspicious areas by screening 2500 women and subjecting 5-10 women unnecessarily may save ONE life

8) With average risk, begin screening at 40 (NCI)

9) The critical factor is early detection

10) The onslaught of negative media will be to support the medical equipment industry (politics and big business)

11) 3D mammorgraphy has twice the radiation and twice the time spend being "squeezed" - little data on efficacy

12) Traditional mammography causes about 20 cases of breast cancer for every 100,000 mammograms, 1 cancer caused for every 2 lives saved

13) If 3D mammography adds 2 more exposures for each screening

QUESTION:

SOLUTION:

What unbiased information is there that

will help me decide when to get mammograms.

Mountains of information are out there to help one decide if mammography screening is right for them. Personality, politics

and economics all drive the interpretation of the current data - the source of the information needs to be considered before you make a decition. Medical equipment providers want to continue in their lucrative businesses, health care providers make money on this procedure. Hospitals derive revenue to help offset costly care giving for other conditions. Radiation causes cancer. Would a "tailored" regimen of screening make more sense.

Below are some samples of the types of data to be found. To save one individual = 25,000 screenings and 5-10 women have to

undergo treatement for breast cancer unnecessarily. Consider, in contrast, a staggering benefit of 40% reduction in breast cancer risk by adopting a vegetable, fruit and grain diet with exercise.

It is advisable to make a list of suggestions from a few dozen sources noting their possible motives, perspectives and

biases.

"According to breast surgeon Susan Love of UCLA, at least 30 percent of tumors found on mammograms would go away if you did absolutely nothing. These tumors appear to be destined to stop growing on their own, shrink, and even go away completely.

Nearly everyone has cancerous and pre-cancerous cells in their bodies by middle age, but not everyone develops cancer. The better you take care of your immune system, the better it will take care of you." (ed, but it is also true that the body can detect bacteria and viruses and parasites easier than its own cancer cells and so may not detect and destroy them all). "One way to strengthen your immune system is to minimize your exposure to mammograms and other sources of ionizing radiation, especially mega sources such as these new 3D scans and CTs. But you can also build up your immune system DAILY by making good diet and lifestyle choices".(Far more breast cancers will be eliminated by healthful living than by mammography.)

Breast cancer screening: Controversy of impact. Donald A. The NCI has consulted with a variety of experts in the field to determine whether a change in its position on mammography is warranted. After careful consideration, the NCI continues to recommend that:Beginning at age 40, women with an average risk of breast cancer should be screened annually with mammography.Women who are at higher than average risk of breast cancer should seek expert medical advice about: Whether they should begin screening before age 40At the same time, the NCI will continue to address the uncertainties surrounding screening mammograms.

Wikipedia (paraphrased by ed): For the average woman, the U.S. Preventive Services Task Force (2009) recommended mammography every two years for women ages 50 - 74. The Canadian Task Force on Preventive Health Care (2012) and the European Cancer Observatory (2011) recommend mammography every 2–3 years between 50- 69.

These reports point out that unnecessary surgery, anxiety, more frequent radiation from mammograms yield a small but significant increase in breast cancer. The Cochrane Collaboration (2011) concluded that mammograms reduce mortality from breast cancer by an absolute amount of 0.05% or a relative amount of 15%, but also result in unnecessary surgery and anxiety, It is not clear whether mammography screening does more good or harm.

Breast self-exam1) In the Shower: Using the pads of your fingers, move around your entire breast and armpit area in a circular pattern moving from the outside to the center. Check both breasts each month feeling for any lump, thickening, or hardened knot. Get changes and lumps evaluated by your healthcare provider.

2) In Front of a Mirror: Visually inspect your breasts with your arms at your sides and with your arms raised high overhead looking for changes in contour, swelling, dimpling of the skin, or changes in the nipples. Then rest your palms on your hips and press firmly to flex your pectoralt muscles while looking for dimpling, puckering, or changes.

3) Lying Down: Place a pillow under your right shoulder and your right arm behind your head. Using your left hand, move the pads of your fingers around your right breast gently in small circular motions covering the entire breast area and armpit area with light, medium, and firm pressure. Squeeze the nipple to check for discharge and lumps. Repeat for the left side.

The Nordic Cochrane Center (comments paraphrased by ed)

An evidence-based leaflet for lay people is available in several languages on www.cochrane.dk. Because of substantial advances in treatment and greater breast cancer awareness since the trials were carried out, it is likely that the absolute effect of screening today is smaller than in the trials. Observations show more overdiagnosis and very little or no reduction in the incidence of advanced cancers with mammographic screening.

25,000 mammograms are required to save one life, yet we can cut the breast cancer rate almost in half by each of us changing our life style.

(The American Institute of Cancer Research estimated that about 40 percent of US breast cancer cases could be prevented if people made wiser lifestyle choices).

But, in 1999, the EU at least resolved the fat issue by allowing up to 5% of chocolate's content to be one of 5 alternatives to cocoa butter: illipe oil, palm oil, sal, shea butter, kokum gurgi, or mango kernel oil.[15]

A recent workaround by the US confection industry has been to reduce the amount of cocoa butter in candy bars without using vegetable fats by adding polyglycerol polyricinoleate (PGPR), which is an artificial castor oil-derived emulsifier that simulates the mouthfeel of fat. Up to 0.3% PGPR may be added to chocolate for this purpose.[16]

Mindfulness-based stress reduction (MBSR) is a behavioral medicine program devised by Jon Kabat-Zinn that uses the psychological concept of mindfulness to help people cope better and be more at ease in their life.[1] It is thought to have potential for helping people cope with stress and chronic illness, and research has shown it having a useful effect. While MBSR has its roots in spiritual teachings, the program itself is secular. Called by many names, then, the basic tennent is the same though the techniques vary from singing, chanting, stretching, prayer, repetitious motions or movements, quoting scriptures, even playing an instrument.

The spiritual roots are linked to all the major nonotheisms and those which are meditation based as well. It becomes quite clear, though research efforts have not produced quantities of data for analysis yet, that these ancient techniques were developed out of observations in past cultures that they "WORKED". Yoga combines the spirituality aspect but also the main "the body has a head" concept. That is - that body and mind can work together tower stress, pain, emotional ups and downs, where the monotheisms do not.

Previous issues of this newsletter provided steps which can be taken by virtually anyone, carried out on a regimen of several times a week. The benefits for both mind and body can be amazing.

Variation 1 (single sided).Choose which direction you want the ornament. Cross stitch that pattern, then cut ONE pattern the toe pointing the opposite direction from backing fabric (see last page for pattern). Right sides together, sew the ornament together leaving top OPEN. Turn right sides out, fold under the top seam allowance. Add ribbon hanger or other hanger, and whip stitch closed.

Variation 2 (closed top, two sides cross stitched)

Make 2 stockings, one in mirror image of the other (see mirror image enlarged patterns). Put right sides of the stockings together and sew together leaving top open. Trim off excess aida cloth, then turn right side out.

Attach ribbon hanger and/or bells in the upper corner of the stocking top, or the toe. Then whip stitch the top closed.

Variation 3 (stocking lined with soft fabric and left open at the top to receive tiny box or gift).

Trace the outline of the two mirror image stockings that you have cross stitched. Add 1/4 inch seam allowance on all sides (or use mirrored patterns on last page)(cut one each). Put right sides of lining fabric together and sew stocking leaving TOP OPEN. Leave the right sides together. Turn down the seam allowance at the top toward the outside.

Sew the cross stitch pattern right sides together leaving TOP OPEN. Trim away excess aida cloth, and turn right sides outTurn cross stitched stocking right side out, and turn top to inside, and baste.

Nestle lining stocking into the cross stitch stocking, keeping turned down edges even at the top.

(hint, choose the lining material of aweight fabric that makes you comfortablewith the stiffness of the final product).

Add ribbon hanger, and/or or ells to top of stocking, or on toe..

Optional: Add a row of braid, or beadsaround the edge and top to finish.
