the acute care for Elders (acE) strategy mount sinai hospital

Mount Sinai's ACE strategy improves how care to older patients isdelivered. e hospital implemented a series of evidence-informed buttailored interventions. e strategy links these interventions to create amore seamless, integrated delivery-model spanning the continuum ofcare. It’s enabled by an interprofessional team-based approach to care aswell as technological innovations with a focus on maintaining theindependence of older adults in the community for as long as possible.

e strategy includes a multi-year action plan to evaluate progress andmake refinements using a balanced scorecard and a benchmarkingsystem that allows for quarterly, regional performance comparators toidentify areas of improvement.

Since 2009, Mount Sinai Hospital has seen a 31% increase in the numberof admitted older adults (65+) it serves on an annual basis. In that time,the strategy allowed Mount Sinai to reduce the average total length of stayper patient by more than 28% and decrease the average ALC days by 18%.Patients are now more likely to go directly home, are less likely to bereadmitted and are more satisfied with the care received. Despite theincrease in patient volumes, this strategy which required minimalfinancial investments, but rather a different approach to the way we work,has reduced the hospital’s overall care costs by more than $6.4 million in2012-13 alone.

Better patient journeys: community-leadstrategies to improve hospital flow

island health

To support Island Health’s strategic priority to improve the quality,coordination, and timeliness of transitions from hospital to community,the continuing health services team implemented community-leadstrategies to support hospital flow in three large hospitals and fourcommunities across Vancouver Island. ese strategies were designed tomake the workflow more efficient to community, residential and mentalhealth care centres as well as create services to meet the needs of the mostfrail and complex clients in their homes and communities.

is multi-site, multi-community, cross-continuum work includedpatient partners in service planning, and relied heavily on developingpartnerships with hospital staff and community family physicians.

Over the last two years, 11,400 bed days have been saved, hospital staywhile waiting for assessment or residential care has been reduced by 57%,and alternate level of care numbers have been reduced by 28%. Significantimprovements have been made in hospital flow, including a shi towardsmanaging complex clients in the community. Perhaps most importantly,our learning from this work is helping create better patient journeys.

Forensic sexual behaviour programtreatment manual

What works to prevent sexual reoffense? As therapists, have we maintained aclient-centered approach while improving the safety of communities?

To answer these questions, Dr. Angela Connors and her team at CapitalHealth thoroughly researched best practice in effectively working with sexualoffenders. eir population analysis now spans over a decade and containscrucial details on over 900 clients. Characteristics such as average risk level,offense profile, education level and other variables were captured andscrutinized, and a program designed to prevent re-offense and increase clientadaptation was developed.

e forensic sexual behaviour program is a provincial resource. erapistsacross Nova Scotia were trained to deliver the program in a client-centredmanner for maximum effectiveness. is encompassed considerable changeof practice and philosophy for some, but the outcomes outweigh the effort.Research shows an approximate 40% reduction in sexual offenses by tailoringtreatment to risk and needs.

Finally, Dr. Connors and her team craed a detailed master documentoutlining supporting research, objectives, cognitive behavioural therapystrategies, resources and skill-practice components for eight treatmentthemes. Each one is iterative, designed to build upon the previous one inskill-development. Approach goals are identified and supported throughout.e ultimate goal of this manual is to support evidence based treatmentacross the province in an effective and consistent manner to reduce sexualreoffense in Nova Scotia communities, while increasing client life satisfactionand adaptation.

mark neskar, chE

Mark Neskar recently retired as Chief Operating Officer of the RANARespiratory Care Group. For over three years Mark helped to lead RANA inthe provision of oxygen, sleep apnea and other respiratory care services inManitoba, Alberta and Nova Scotia.

Prior to RANA, Mark worked for more than 30 years in the public healthcare system in Manitoba, leading a number of rural hospitals and personalcare homes. In 1997, he worked for the Canadian Red Cross as Director ofthe Winnipeg Blood Centre. From 1998 to 2008, he served as Chief ExecutiveOfficer of the Seven Oaks General Hospital in Winnipeg. Under Mark’sleadership, Seven Oaks became a value-based organization with a significantfocus on employee health & wellness. Seven Oaks was recognized twice asone of Canada’s Top 100 Employers by Maclean’s Magazine. As CEO, Markalso served as executive sponsor for the Manitoba Renal Program and theGenetics Program with the Winnipeg Regional Health Authority.

Mark has been a member of the Canadian College of Health Leaders since1981, receiving his CHE designation in 1985. He served on the Board ofDirectors as Manitoba Director from 2007 to 2012, and served as Chair of theAudit & Finance Committee. Mark has also served extensively on theManitoba provincial chapter executive, including a term as chair. In 2007,Mark was recognized for his leadership receiving the Manitoba ChapterLeadership Recognition Award.

Mark received his Certificate in Health Care Administration at the Universityof Saskatchewan in 1980 and his BGS degree at Brandon University in 1998.He currently serves as a Director on the boards of St. Josephs Residence andManitoba Blue Cross in Winnipeg and is a member of the Faculty of RedRiver College, Health Services Management Certificate Program.

Elma heidemann, Fcchl, lFachE

Elma Heidemann is the former Chief Executive Officer of AccreditationCanada, a position she held for eleven years. Since then, she has worked as aninternational consultant on various projects related to health care planning,quality, evaluation and accreditation.

She has held a number of board appointments, most notably on the boards ofthe Canadian Healthcare Association and the Ontario Hospital Associationand was a past chairman of the boards of both of these organizations. For 12years, she served as trustee on the board of directors of the RiversideHospital, Ottawa, including a two-year term as chairman of the board. Shechaired a special committee established by Health Canada to develop a modelof palliative care for persons with AIDS and served as a special consultant toHealth Canada to examine the capability of major Canadian cities to providepalliative care for persons with AIDS.

She is a former member of the Scientific Council of L’Agence Nationaled’Accréditation et d’Évaluation en Santé, now HAS (Paris, France) and theIrish Health Services Accreditation Board (Dublin, Ireland). She is past-president of the International Society for Quality in Health Care (ISQua) andpast member of the governing council of ISQua’s International AccreditationProgram, IAP. She continues to serve as an international surveyor for theIAP program. She served as a member of the Audit Advisory Committee ofthe Public Service Commission of Canada and was a Canadian MedicalAssociation appointee to the Committee on Accreditation of ContinuingMedical Education which accredits medical schools in Canada. She is thepast co-chair of the Quality Council of the Victorian Order of Nurses and is afounding co-chair of the Canadian Health Leadership Network. Elma hasalso volunteered her time to the College as a fellowship peer reviewer and amember of various committees including those for awards and certification.

Elma was the recipient of the College Award for Distinguished Services in2010.

Trillium Health Partners is a recently merged hospital servingMississauga, West Toronto and the surrounding communities. Since2004, the Mississauga Hospital and Queensway Health Centre havemaintained an Environmental Management System to ISO 14001 whichwill be expanded to Credit Valley Hospital site by 2016.

By incorporating green conservation into its day-to-day roles, thehospital contributes to the efficient use of resources, which in turn isreallocated to patient care. It has implemented programs that focus onwaste, energy, water, air, hazardous material and emergencypreparedness. For example, an environmental protection policy wasrecently approved, a dedicated energy project manager was hired, aneducation campaign about waste management was delivered, and anenvironmental awareness training program is in the early phase of beingimplemented. Moreover, the energy conservation projects are anticipatedto reduce 600 kW demand energy and 8,000,000 kWh consumption, oncecompleted in 2014-2015.

e TC CCAC serves one of the most ethnically diverse communities inthe country. For the TC CCAC, cultural diversity encompasses theunique characteristics that all of us possess that distinguish us asindividuals and identify us as belonging to a group or groups. Diversitytranscends concepts of race, ethnicity, socio-economic, gender, religion,sexual orientation, disability and age. Diversity offers strength andrichness to the whole.

In keeping with its strategic directions, TC CACC is client-centred andcommitted to listening to its clients. It depends on every employee beingculturally competent to understand clients from a broad, openperspective, in order to achieve the best health outcomes and satisfaction.e TC CCAC provides training in cultural competency to all of its staffand managers as an important strategy in empowering staff to fulfill theirfull potential to serve clients respectfully, appropriately and creatively.

In fact, 87% of clients report overall satisfaction with their care from theTC CCAC. And the responses with some of the highest positive scores onthe recent employee engagement survey perhaps say it best: 88% ofrespondents stated that people treat each other with respect at TC CCAC;88% responded that people from diverse backgrounds felt welcome; and75.4% reported that they agree with the organization’s values.

Bcca patient transitioning project

e BCCA, in collaboration with Vancouver General Hospital, have workedclosely to “mistake-proof ” the process of transitioning patients to and fromthe BCCA's out-patient departments. e project began as a BCCA patientsafety event review between two departments. A failure modes effectsanalysis was conducted with frontline staff and leaders from bothorganizations and recommendations were rolled out throughout all of theBCCA centres and the sending facility.

Efforts were standardized to ensure that all centres were using the same formin the correct manner to gain information in advance about all in-patientstransitioning to the BCCA for out-patient treatment. A returncommunication handover form was implemented as well to ensure thatpatient information was consistently transferred back with the patient to thesending facility.

e sending facility implemented guidelines and a transition checklist toprepare patients for transport, to ensure that patients have a painmanagement plan for the trip and that staff can accompany them if required.ese guidelines and checklist have now been implemented in other healthauthorities in British Columbia.

Post-implementation results show a significant reduction in the number andseverity of patient safety events involving the transitioning of patientsbetween the BCCA and other health authorities.

Rob Devitt is a transformational leader who constantly has a student by hisside. One of Rob's many mentorship legacies has been the administrativeresident program at Toronto East General Hospital. He created a structuredyet dynamic program that provides learning opportunities for master'sstudents in health and business administration. He credits his ownexperience as a resident with his strong belief that the placement is aboutexposure to system, governance and executive level decision-making. Robencourages his mentees to utilize his network to gain experience in differentsettings during their residency or during their careers. As CEO, his belief indistributed leadership means that he creates space for growth and challengesindividuals to push their boundaries to become more well-rounded leaders.

Rob is not only a mentor to students and colleagues but a mentor to thehealth system and is frequently called upon to assist other organizations as apeer reviewer, interim CEO or hospital supervisor. He has been a voice andchampion for many issues like healthy workplace, violence prevention,governance and integration.

Rob has been a preceptor for master's students for over two decades andmore recently began a senior fellow role at the University of Toronto'sDepartment of Health Policy, Management and Evaluation. While Rob hasbeen an incredible inspiration to many health system leaders, he stronglybelieves that his mentorship and teaching work is critical for his ownpersonal development and that this work is constantly challenging him innew ways. Given he is rarely seen without a student, it's safe to say that Rob iscommitted to continual, life-long development.

Carol Kushner and Donna Davis are extraordinary volunteers andnational healthcare leaders. As co-chairs of Patients for Patient SafetyCanada (PFPSC), a patient/family network and program of the CanadianPatient Safety Institute, they are leading a new movement that isempowering and engaging patients and families in safety initiatives inCanada.

As one of PFPSC’s founding members, many of Carol’s strong leadershipskills were used and valued in the program’s formative years. Since 2006,Carol has helped to shape the vision, collaborative relationships, andstructure of this dynamic and growing initiative.

Following the death of her son Vance in 2002, Donna emerged as a leadergiving dozens of presentations to healthcare providers, leaders and policymakers demonstrating the importance of engagement with patients andfamilies. Like Carol, Donna is a dedicated mentor and demonstrates byexample how collaboration and engagement can start to bring aboutpositive changes at the local, provincial and national levels.

Aer a year working in research, Vicky chose to pursue her Master ofHealth Administration at Dalhousie for its prestigious CAHME designation andexciting residency options. roughout the program Vicky has worked to hone herleadership and critical thinking skills through election to the school's student society asFirst-Year Representative and later as Co-President. In her second year, she was selectedto represent Dalhousie as a team member in the school's inaugural performance at theUniversity of Alabama at Birmingham Heath Administration Case Competition. Vickyhas been the recipient of the First-year Academic Achievement Award, as well as theGlen Moore Memorial Award for leadership, congeniality, and potential to make anoteworthy contribution to the field of health administration. In August 2014, Vickywill start the next chapter of her career pursuits as she begins her studies at Dalhousie'sMedical School.

Geneviève plamondon, University of montreal

A physiotherapy graduate from Laval University, Geneviève worked as a healthprofessional within the health and social services network in Québec. While workingtowards a Master in Health Administration degree at the University of Montréal,Geneviève became interested in the organization of health services as well as differentassessment methods. She worked as a research assistant in health economics and herthesis focused on assessing the impact of cigarette prices and exposure to tobaccoadvertising in smoking among South American adolescents. In 2013, she joined theInstitut national d’excellence en santé et services sociaux, the health technologyassessment unit for the province of Québec. As an innovation scientific advisor, she hasbeen working to develop new strategies to optimize the assessment and the introductionof innovative technologies in the health and social services network.

derek clark, University of alberta

Derek Clark is completing a Masters of Public Health in health policy andmanagement at the University of Alberta’s School of Public Health. Derek earned aBachelor of Nursing from Mount Royal University in 2011. As a Registered Nurse, hehas a clinical background in blood and marrow transplantation, hematology andoncology. Now, he is interested in informing and developing policy to improve thehealth of populations, particularly in the area of health technologies. Currently, Derekis conducting projects aimed to develop practical tools that decision-makers inCanada can use to guide their processes for orphan drugs and personalized medicinesto support equitable, efficient and sustainable health care systems. He is lookingforward to integrating his nursing and policy backgrounds to effect change at theprovincial and national levels.

Taylor has always had a passion for healthcare management. Aer completing aBachelor of Life Sciences from Queen’s University, Taylor began her healthcare careerin Ireland, managing a home care service for individuals with physical and intellectualdisabilities. is line of work later took Taylor to Australia, where she led a communityoutreach program supporting adolescents with disabilities and mental healthstruggles. Since returning to Canada, Taylor has worked in the Aboriginal health andyouth services arenas. Taylor graduated from the University of Ottawa's Telfer Schoolof Management with a Master of Health Administration degree in 2013 and wasinducted into the Beta Gamma Sigma society this past March. Taylor currentlymanages the Children’s Hospital of Eastern Ontario's Youth Net program. Taylorproudly volunteers as the vice chair of the board of directors for the erapeuticRiding Association of Ottawa-Carleton. Taylor plans to continue learning throughinvolvement with the Canadian College of Health Leaders and aspires to impactaccessibility within our system in her healthcare future.

Julie ho, University of British columbia

Julie Ho is completing her final year of the Master of Health Administration programwith the University of British Columbia (UBC). She received her BSc from UBC andworked as a research technologist at the BC Centre for Disease Control. ere sheparticipated in influenza sero-epidemiology and vaccine studies during the H1N1pandemic. In 2010, she joined the Centre for Translational and Applied Genomics atthe BC Cancer Agency as a lab technologist, where she is involved in innovative cancerresearch projects. She is interested in personalized medicine and hopes to engage in amore active role in the public health sector to improve patient care and healthoutcomes.

Jeff Biddiscombe, University of toronto

Lieutenant-Commander Jeff Biddiscombe is a 24-year veteran of the Canadian ArmedForces, with overseas deployments to Bosnia and Afghanistan. He began his career asa nurse in the military, developing experience in such areas as general medicine,surgery, psychiatry, chemotherapy, palliative care, and flight nursing. Aer 16 years ofclinical nursing, Jeff moved into more formal administration and leadership positionswithin the Canadian Armed Forces before starting his Masters of Health Sciences inHealth Administration from the University of Toronto’s Institute of Health Policy,Management and Evaluation. Upon completion of his degree this summer, Jeff will beappointed as Commanding Officer of 31 Canadian Forces Health Services Centre inBorden, Ontario, which is a four-clinic network providing primary care services tomilitary personnel in south-central Ontario.

At 35, this is Sarah’s second vice-president role. At the Chatham-KentHealth Alliance (CKHA), Sarah lead the capital redevelopment projectwhich put forth a vision of a campus of care to support a sustainabledelivery system for the community of Wallaceburg, including joint capitaland service delivery planning with community based partners. Sarah’sbeen instrumental in leading the service integration, which included thehospital’s divestment of five programs to the Community Health Centreand to establish a fully integrated management team with the Lambton-Kent Canadian Mental Health Association. She is also currently chairingthe development of the local HealthLink.

is past year, Sarah led the organization through the second phase ofoperational changes in 16-months. CKHA is transitioning through newhealth system funding reforms in Ontario. Using a six-month intensiveprocess that engaged clinical leaders and physicians, the organizationcreated a series of comprehensive business cases to support a final plan tothe tri-board.

In addition to her role at CKHA, Sarah also lead the development of asupply chain shared services organization in the Erie St. Clair LHINregion (PROcure) in 2008 and then went on to lead the merger of twoshared services organizations.
