Aswecommemoratethetenthanniversaryofherdeath,thisquoteremainsasrelevantaseverandmustremainaguidinglightforus.Weallfacetheprospectofdyingoneday.Mostadults,andmanychildren,experiencethelossofthosedeartothemwellbeforethathappens.Thisisanintenselypersonalhumanexperience.Everymomentcountsbecause,oncepassed,itcanneverberegained.

Thisnarrativeisanimportantmilestone.IthasbeencarefullyandpainstakinglyproducedundertheleadershipofNationalVoicesworkinginpartnershipwithNationalCouncilforPalliativeCareandotherorganisations.Ittellsus,unequivocally,whatpersoncentredcaremeansfromthepointofviewofsomeoneapproachingtheendoflife.WeusethisnarrativeasthecentrepieceforNHSEngland’sActionsforEndofLifeCare.

Asafront-lineclinician,Iknowthatintherealworld,itisdifficulttoachieveperfectionforeverybodyeverytime.Therecanbeamismatchbetweenwhatservicescanoffer,whatprofessionalshopetoachieveandwhattheperson,andtheirfamilies,carersandthoseclosetothemseekorexpect.Findingoutwhatreallymatterstothem–youcanalmost‘hear’thevoicesinthisreport–hastobeafundamentalstartingpointforallofus,andestablishesaproperpartnershipincare.
