Evaluating an integrated pre-registration training programme in North Wales: perceptions of pre-registration pharmacists and their tutors Report to MPC programme board- July 2017 Executive Summary Background Six pre-registration pharmacists (pre-reg) are currently experiencing training through an integrated programme in Betsi Cadwalader University Health Board (BCUHB) whereby they are exposed to hospital, community and primary care pharmacy environments. This programme is the first of its kind in Wales. Methods Following ethics committee approval semi-structured one to one interviews and focus groups were undertaken with a range of stakeholders in order to evaluate the programme. A broad topic guide with prompts and probes was developed and included sections exploring participant demographic details, experiences of pre-registration training/supervising pre-registration training and opinions towards respective pre-registration training programmes. Interviews were conducted at the six month and ten month time points, these were audio recorded, transcribed verbatim and thematically analysed. Results Twenty two participants were interviewed (six integrated pre-reg, five tutors, six hospital pre-reg and five community pre-reg) at the six month time point. All but three participants were followed up at ten months. Four themes were identified, namely 1) programme value to integrated pre-reg 2) programme value to other stakeholders (including hospital and community pharmacies as well as patients themselves) 3) value of cross-sector experience to hospital and community pre- reg 4) suggestions for change. Interviewees expressed overwhelmingly positive views towards the integrated programme. Its perceived benefits include producing “rounded” pharmacists with an ability to work in more than one sector upon registration, as well as improving pre-reg confidence in communicating and allowing pre-reg to take ownership for their training year. Conclusion This study provides an insight into the perceptions of integrated pre-reg and their tutors towards the pilot programme as well as comparisons with others on a traditional hospital or community pharmacy only programme. Participants were able to reflect upon the knowledge, skills, attitudes and values developed as part of the integrated programme and suggest ways in which the programme may be improved for future trainees. Those reviewing or considering implementing an integrated pre-registration programme may wish to pay careful consideration to the structure of the timetable, stakeholder expectations of integrated pre-reg, the importance of communication between stakeholders (hospital and community pharmacy tutors and tutors with trainees) and the geographical location of training sites involved.

FullReportResearcherThisstudywasconductedbyWCPPEeducationprojectmanagerandPhDcandidate,BethanBroad.Theresearcherwasnewtothejobpositionandowingtotheinductivenatureoftheresearchhadnosignificantpriorknowledgeoftheintegratedpre-registrationprogramme.Theresearcherdidhoweverhaveexperienceofqualitativeinterviewinginaseparatestudyexploringpharmacygraduatepreparednessforthepre-registrationyear.

BackgroundSinceAugust2016sixpre-registrationpharmacists(pre-reg)inBetsiCadwaladerUniversityHealthBoard(BCUHB)haveexperiencedpre-registrationtrainingthroughamulti-sectorprogrammewherebytheyspendtimeinhospital,communityandprimarycarepharmacyenvironments.Forthepurposeofthisreporttheschemehasbeenreferredtoastheintegratedpre-registrationprogramme,inlinewithwhattheschemeisreferredtolocally.Inordertoevaluatetheeffectivenessofthisnewprogrammeinpreparingtraineesfordayonepharmacypracticetheresearcherwascommissionedtoconductaqualitativeserviceevaluation.Thestudyincludedtheviewsofintegratedpre-regthemselvesaswellastheperceptionsoftheirtutors.Pre-regcompletingahospitalonlyorcommunityonlytrainingprogrammewerealsoincludedasameansofcomparison.

AimsThisstudyaimedtoexploretheopinionsofintegratedpre-regtutorsandpre-regfromarangeofsectors(hospital,communityandintegratedprogrammetrainees)towardspreparednessfordayonepharmacypracticeaswellastheirperceptionstowardstheirrespectivetrainingprogrammes.

MethodsQualitativemethodswereemployedforthisstudyowingtotheexploratorynatureoftheresearch.Stakeholderviewsweregatheredviasemi-structuredonetooneinterviewsorfocusgroupsbythesameresearcher.Opinionswereexploredattwotimepoints,firstlyatapproximatelysixmonthsintotheprogramme,thenagainapproximatelyfourmonthslater.Repeatinterviewingallowedtheresearchertoexplorewhetheropinionschangeaspre-regprogressthroughthetrainingyear,gainmoreexperienceandvariedresponsibilities.Topicguideswithpredeterminedtopicsandquestionsweredrafted,howevertheorderandexactwordingofthesequestionswerenotcraftedpriortointerviews.Topicguidesfirstlyexploredparticipantfactors(forpre-regthisincludedtheirgraduatingSchoolofPharmacy,priorworkexperiences,andwhethertheirprogrammewastheirfirstchoiceofpre-reg.Forsupervisingtutorsthisincludedtheirsectorofpractice,timespentintheircurrentroleand

experiencewithsupervisingpre-reg).Thesecondtopicguidesectionexploredthetaskspre-regwerecurrentlyworkingon,howtheywerefindingtheseandhowpreparedtheyfelt/wereperceivedtobefordayoneofpracticeuponregistration(includingcomparisonswithotherpre-reg).Finallyparticipantswereaskedtoreflectontheirrespectivepre-registrationprogrammesandsuggestwhatworkswell,whatcouldbeimprovedandhowthismaybeimproved.ThestudywasapprovedbyCardiffSchoolofPharmacyResearchEthicsCommitteeandBCUHB.Potentialinterviewees(n=36;allintegratedpre-regandtheirtutorsinBCUHB;allhospitalpre-reginBCUHBandaconvenience/purposivesampleofcommunitypre-regfromSouthWales)wereinvitedbye-mailandgiventwoweeksinwhichtorespondbeforearemindere-mailwassent.Respondents(n=22)wereprovidedwithaninformationsheetandconsentformbeforeinformedwrittenconsenttotakepartinthestudywasobtained.Interviewswereaudiorecorded,transcribedverbatim,anonymisedandthematicallyanalysed(BraunandClarke,2006).InterviewswereconductedfacetofaceinavarietyoflocationsincludingatrainingroomwithinCardiffUniversityandanoff-sitetrainingfacility.Wheretheselocationswerenotconvenientfortheparticipantinterviewswereconductedoverthetelephone.Analysis

BraunandClarke’sthematicanalysiswascarriedoutbyfirstlyreadingandre-readingthetranscriptstoachievefamiliaritywiththedata.Codeswerethenassignedsentencebysentencewherethedatarepresentedsomethingsignificantinrelationtotheresearchquestion.Repeatedpatternsofmeaningwereidentifiedandcodessortedintooverarchingthemesandsubthemes.Themesandcodeswerethencheckedforconsistencytoensuretheyaccuratelyrepresentedthedata.ResultsAtthesixmonthtimepointinterviewswereconductedwithfiveintegratedpre-regtutors,fivecommunitypre-reg,sixhospitalpre-regandsixintegratedpre-reg.Atthetenmonthtimepointfollowupinterviewswereconductedwithallbutonetutorandtwohospitalpre-reg,whowerelosttofollow-up.

Pre-registrationpharmacistattitudesandvaluesConfidencewasanattributerepeatedlymentionedwhendiscussingtheintegratedprogramme.Inparticulartutorsnotedintegratedpre-regconfidenceineachsectortobeincreasingsuchthatthey’llhavegreaterconfidenceondayoneofpracticeincomparisonwiththeirpeers.Integratedpre-regthemselvesattributedthisconfidencetohavingexposureofallsectors,being“thrownin”andreceivinggreaterresponsibilityinprimarycare. DiscussionTwentytwoparticipantsweresuccessfullyrecruitedandinterviewedatthesixmonthtimepoint.Nineteenofthesewerefollowedupattenmonths.TheinterviewerwasapharmacistandmemberofstaffatWCPPE.Participantsmayhaveagreedtotakepartinlightofthisalternativelytheymayhavewishedtocontributetothedevelopmentoftheprogramme,indeedmanyintegratedpre-reginparticularcommentedthattheirviewscouldbeusedtoshapethefutureoftheprogramme.Whileengagementwiththeliteratureanddetailsurroundingtheintegratedprogrammewaslimitedinterviewerbiascannotberuledout(NewmanandBenz,1998).ThisprogrammeisthefirstofitskindinWalestoofferpre-regtheopportunitytoexperiencetrainingincommunity,hospitalandprimarycarepharmacyenvironments.IndoingsosatisfyingtheGeneralPharmaceuticalCouncilrequirementforpre-regonsplitorjointprogrammestospend“forexample,six(orperhapsnine)monthsspentincommunitypharmacyandsix(orperhapsthree)monthsinhospitalpharmacy”or“six(orperhapsnine)monthsspentinapatient-facingsectorandsix(orperhapsthree)monthsinanon-patient-facingsector”(GeneralPharmaceuticalCouncil,2017).Whileothercross-sectorprogrammesareavailableinEnglandandScotland,there’scurrentlynopublishedworkontheireffectiveness.ApilotschemeestablishedinEnglandin2015offerspre-regtheopportunitytospendonedayaweekfor8weeksinaGPenvironment(PharmacistSupport,2017)aimingtoprovidethe“greatercollaboration”betweenhealthcareprofessionalscalledforinNHSEngland’s“FiveYearForward”document(NHS,2014).Theschemehasreceivedpositivefeedbackandhassincebeenrolledoutfurther(PharmacistSupport,2017).Thepilotaimedtogivepre-rega“deepunderstanding”ofhowGPsurgeriesoperate(Collins,2015).Whileanevaluationontheprogrammehasnotbeenpublished,resultsfromthisstudyshowthatthoseontheintegratedpre-reginNorthWaleshavebenefittedfromgaininganunderstandingofhowGPpracticeswork,anelementwhichcommunitypre-regstatedtheywouldliketogainfurtherunderstandingofthroughvisitstoGPsurgeries.InScotland,pre-regrecruitedthroughthePre-registrationPharmacistScheme(PRPS)followacoretrainingprogrammewheretheytooareprovidedtheopportunityforcross-sectorplacements(NHSEducationforScotland,2017).Furtherresearchintotheeffectivenessofsuchprogrammesisneeded.Participantsinthisstudyhadvariedexperiencesdependingontheirsectorofpractice,consistentwiththefindingsofa2016study;whichshowedthatexperiencesdifferedbetweensettingswhere“servicesprovidedandpatientmixvaried”(Jeeetal.2016).Arecommendationwasmadethatstandardisationacrosssitesmayreducevariationinexperienceandpre-regprofessionalsocialisation(Jeeetal.2016).Inthisstudyitwasperceivedthattheintegratedprogrammemaybeawayinwhichpre-regcouldobtainthe“sameexperience”byseeingthingsinothersectorswhichtheyotherwisewouldn’thavetheopportunitytosee,thuspotentiallyachievinggreaterstandardisation.

Limitationsofthestudyincludetherelativelysmallsamplesize,astherearecurrentlyonlysixpre-regundertakingtheintegratedprogramme.Theeffectofself-reporterbiascannotberuledoutaspre-regweremakingcommentsrelatingtotheirownpreparedness(Gordon,1999;Daviesetal2006).Triangulationofintegratedpre-regviewswiththoseoftheirtutorsaimedtominimisethiseffectaspreviousstudieshaveshownthatemployerperceptionsofgraduatepreparednessinpharmacyhavebeenlessfavourablethanthoseofgraduatesthemselves(Kairuzetal,2010).Thisstudydidnotobservethiseffectandagreementwasseenonmanyofthekeyareasexplored.Theopinionsofhospitalandcommunitypre-regtutorswerehowevernotincludedinthisstudy.Twofurtherthemes(‘tasksundertakenbypre-registrationpharmacists’and‘desiredqualitiesofintegratedpre-registrationpharmacists’)wereidentifiedfromthedatabutarenotdiscussedhereastheyarelessrelevanttotheresearchaims.ConclusionThisstudyprovidesaninsightintostakeholderperceptionsoftheintegratedpre-registrationtrainingprogrammeinNorthWales.Ithasidentifiedthatbothintegratedpre-regandtheirtutorsalikehavepositiveperceptionsoftheprogramme.Itsbenefitsinpreparingpre-regforpracticeandwiderbenefitswithinthepharmacycommunityhavebeensuggested,aswellaswaysinwhichtheprogrammemayberefinedforfuturecohorts.Thestudyalsoidentifiedthatthoseconsideringimplementingorreviewinganintegratedpre-registrationprogrammemaywishtoconsider:

- Thebalanceofprovidingbothstructureandflexibilitywithinthetimetable

- Carefulmanagementofstakeholderexpectationssuchthatindividualsinvolvedintheintegratedprogrammearewellinformedoftheroles,responsibilitiesanduniqueneedsofintegratedpre-regpharmacists

- Theimportanceofcommunicationinintegratedprogrammes(particularlybetweentutorsindifferentsectorsandbetweentutorsandpre-regpharmaciststhemselves)

- Geographicallocationoftrainingsites,suchthatpre-regareabletotravelwithrelativeconveniencebetweensectors

FutureWorkEthicalapprovalhasbeenobtainedinordertoconducttelephoneinterviewswiththeintegratedpre-regcohortuptosixmonthspostcoursecompletion,inordertoexploretheiropinionspostregistration.
