The projects are managed by study groups which consist of at least a junior fellow and a seniorfellow and which perform research on a specific field of medicine (e.g., Hungarian Pancreatic StudyGroup). A research fellow can participate in multiple study groups with different projects, as indicatedin the blue badge.

In September 2019, 20 research fellows started their scientific carrier in the Centre forTranslational Medicine, University of Pécs in clinical and basic research PhD programs (with 17 and 3research fellows, respectively). This brochure aims to describe the research projects in which thefellows are working. SUPPORT AND SUPERVISION Clinical scientific supervisor: a senior clinician (expert) who raises relevant clinical questions, determinesthe direction of the research and bridges the gap between the theoretical and clinical work in theclinical PhD program. Basic research supervisor: a senior research fellow (expert) who raises relevantbasic research questions, determines the direction of the research and ensures that the methodologyused in the basic research PhD program meets the international standards. Educational supervisor: anexpert in pathophysiology who provides support in academic education. Scientific methodologysupervisor: a methodologist who has experience in designing and carrying out translational researchprojects and provides methodological support in various aspects of science including meta-analyses,patient registries, and clinical trials.

Researchfellows are offered to participate in 6-, 12-, and 24-month scientific education, as indicated inthe orange badge.

Klementina is participating in a 24-month clinical PhD fellowship programme. She iscurrently working on network meta-analysis, evaluating the benefits of liver support inpatients who had sustained acute-on-chronic liver failure. She is responsible for planning arandomized clinical trial which aims to reduce the recurrence rate of alcoholic acutepancreatitis by complex lifestyle intervention. Her third project includes a multicentrepatient registry which records the clinical cases of decompensated cirrhosis (DELIVER-IT).As her fourth area of research, a cohort analysis is currently being performed using thedata available from the Acute Pancreatitis Registry of the Hungarian Pancreatic Study Group.The aim of this project is to analyse the effect of hypoalbuminemia on disease severity anddisease course. Furthermore, she plans to carry out a retrospective study of patients whounderwent liver transplant and received artificial liver support therapy. This project will beconducted in cooperation with the Department of Transplantation and Surgery of theSemmelweis University.

Nóra is participating in a 24-month PhD fellowship programme. Upper gastrointestinalbleeding is a common condition which still has high mortality rate despite all modern therapeuticapproaches. Her main goal is to plan and operate the gastrointestinal bleeding registry. Inaddition, she runs a diagnostic test accuracy meta-analysis which aims to investigatedifferent testing methods of Helicobacter pylori in patients with peptic ulcer bleeding. She isinvolved in a second project, an interventional meta-analysis studying the side effects ofbisphosphonate therapy in osteoporotic patients with special emphasis on severe adverseevents. As for her secondary objective, she is involved in the operation of the EsophagealCancer Registry, as well.

Bernadett is participating in a 24-month PhD fellowship programme. Bernadett’sprimary aim is to set up a nationwide patient registry in order to prospectively collect andanalyse data on clinical characteristics, diagnostics, therapeutic interventions and mortality ofchronic myeloid leukemia (CML) patients who had been admitted to the hospital with arterial andvenous complications. In addition, a prognostic meta-analysis is currently in progress toinvestigate prognostic factors contributing to the development of arterial and venous thromboticevents in CML patients and other chronic myeloproliferative neoplasms. As regard to thesecondary objective, she is contributing to the development of another patient registry recordingthe data of patients with multiple myeloma.

Félix is participating in a 24-month PhD fellowship programme. Since cystic fibrosis is oftenassociated with vitamin deficiency and its consequences, Félix is currently working on aninterventional meta-analysis assessing the harms and benefits of vitamin D supplementationin patients with cystic fibrosis. His other primary objective is to set up a multicentre randomizedcontrolled trial which aims to investigate the effects of dietary interventions on the recurrencerate of idiopathic acute pancreatitis. In this study, he plans to compare the effects of a standardhealthy diet to that of a low-fat (<15%) diet and to that of a low-carb high-fat (>70%) diet. Hissecondary aim is to plan and set up a pediatric patient registry to follow-up the studypopulation of the APPLE-trial which investigated the etiology, genetic background and therapy ofpediatric pancreatitis.

Her first priority includes thesetup and operation of the TRANS-IBD, which is a pragmatic, multicentre randomized controltrial investigating transitional care in adolescents’ suffering from IBD. In addition, she is currentlyworking on a meta-analysis, which aims to examine the relapse rate and survey any possibleprognostic factors after de-escalating drugs (immunosuppressants and/or biological agents) in IBDpatients. She is operating an ongoing patient registry collecting data of IBD patient in multiplecentres, while another patient registry on polyposis syndromes in children and adults is underpreparation.

Fanni is participating in a 24-month PhD fellowship programme. Her plan is to compose anetwork meta-analysis about the different therapeutic options including antimicrobial agents andfecal transplantation of recurrent Clostridium difficile infection. In addition, she is involved in theorganising of two randomized controlled clinical trials: one will examine the efficacy and safetyof fecal microbiota transplantation in patients with newly diagnosed non-alcoholic fatty liver disease(NAFLD), while the other one will compare the efficacy and safety of fecal microbiota transplantationand fecal filtrate transplantation in recurrent Clostridium difficile infection. She is responsible forplanning and setting up a nationwide patient registry to monitor Clostridium difficile infection, asthe most frequent nosocomial infection with a high recurrence rate.

Her basic research focuses on the complex central effects of urocortin 2 (Ucn2) on metabolicparameters, e.g. oxygen consumption, heart rate, body temperature, food intake and body weightin male Wistar rats. Furthermore, she is planning to investigate the changes in the effects of thispeptide on animals of different training levels. The third area of her research focuses on thecomparison of Ucn2 effects in male and female rats.Dóra is currently working on three meta-analysis projects. With regard to the interventionalanalysis of the effects of ketogenic diets on cognitive functions, data analysis is still in progress,since the variable scores and units of various cognitive tests present a challenge for ourstatisticians. The second project on the effects of body weight reduction (WL) combined withpositive pressure ventilation (CPAP) on cardiovascular risk factors in obstructive sleep apnea(OSAS) has reached the stage of completed statistical analysis and quality assessment. Thepreparation of the manuscript is under progress. The most recent meta-analysis of Dóra on thebeneficial cardiovascular effects of Ucn2 in human heart failure has progressed well, dataextraction is completed.

He is involved in theoperation of the Acute Pancreatitis Registry and is doing a cohort analysis of the clinicalcharacteristics of acute necrotising pancreatitis and its predictive factors. His meta-analysisinvestigates the safety and efficacy of granulocyte and monocyte apheresis (GMA) in ulcerouscolitis. He will perform a network meta-analysis assessing all therapeutic options in newly-diagnosed patients with multiple myeloma. In addition, he will set up a patient registry onmultiple myeloma to monitor the epidemiology and therapy of this rapidly evolving area.

Lajos is participating in a 24-month PhD fellowship programme. One of his main goals is tooperate the Esophageal Cancer Registry and prepare a cohort analysis of the patients included.In addition, he conducts a meta-analysis concerning the treatment of T1B-T2 esophageal cancerwhich compares minimal invasive surgery and open surgery. His secondary objective is toparticipate in the work of the operation of the ERCP Registry. Concerning the high number of casesenrolled, he will make an analysis of the currently available data. As an ancillary project, he isinvolved in a methodological review concerning the quality of meta-analyses of post-ERCPpancreatitis.

Marcell is participating in a 24-month PhD fellowship programme. He is involved in thedevelopment and maintenance of the Hungarian Celiac Disease Registry, launched to recorddata on celiac patients and to compile data on epidemiology, clinical symptoms, complications,inheritance pattern, and dietary questions related to celiac disease. In addition, he is currentlysetting up a case-control study to assess the prevalence of celiac disease among cystic fibrosisand to compare the clinical phenotype of patients with cystic fibrosis and celiac disease to that ofthose with cystic fibrosis and without celiac disease. He investigates the prevalence of differentautoimmune diseases among patients with cystic fibrosis by performing an epidemiologicalmeta-analysis.

Since obesity is a major concern of modern societies, she will set up a patientregistry to improve the understanding of the disease course and to identify potential risk factorsfor common comorbidities and complications. She is performing an interventional meta-analysis to test the efficacy and safety of transverse abdominis plane (TAP) blockade afterbariatric surgery, which procedure has often been used recently.

Leonardo is currently working on an extensive translational research project, which consists of 3main pillars: 1) engineering a novel experimental setup; 2) applying of the new setup in basicresearch experiments; and 3) validating of the basic research topic in humans with meta-analysis. Based on literature data he will evaluate and analyze the impacts of transient receptorpotential (TRP) V1 channel agonists on blood cholesterol levels. While the statistical analysis is inprocess, he is improving his technical skills, i.e., working with wire myograph system DMT 620 andisolating carotid and tail-skin arteries from mice and rats. He will use those skills in experimentswith knockout (TRPV1 and TRPA1) mouse models and study the role of TRP channels in a variety ofthermophysiological processes. He is also collaborating with the 3D-printing team on the technicalperfection of a rapid temperature-exchanging device to be used in investigating the roles of TRPA1and TRPV1 receptors in pathological conditions such as systemic inflammation.

Szimonetta investigates the long-term effects of overnutrition on the regulation of the metabolicbalance and aging. She is working on multiple prognostic meta-analyses. Three of them focuson the effect of maternal overnutrition on offspring’s insulin sensitivity (submitted manuscript,under review), blood pressure (in preparation) and obesity in adulthood (in progress). Her newestmeta-analysis aims to clarify the impact of obesity on mortality, quality of life and frailty insarcopenic adults. Szimonetta’s basic animal research aims to clarify the regulatory mechanismsin the background of long-term effects: how does the perinatal overnutrition change the functionof appetite-regulating centers involving neuropeptides of hypothalamus-adipose tissue axis andthe gut–brain communication during the course of life? She is currently working on in vivo and invitro animal experiments in perinatally overfed rats to explore the long-term aging-related biphasicchanges of neuropeptidergic body weight regulation, which may promote an earlier onset of bothmiddle-aged obesity and aging anorexia leading to sarcopenia. Her project involves investigation ofthe age-related changes in hypothalamic neuropeptide systems complemented with a novelexperimental approach: RNA Scope method in collaboration with the Department of Anatomy,University of Pécs.

Anna Kanjo

Anna is participating in a 12-month resident fellowship programme. There has been aconsiderable improvement in liver support system in the past years. Anna is currentlyperforming a network meta-analysis on the safety and efficacy of bio-artificial and artificialliver support systems in acute/hyperacute/subacute liver failure. A similar improvement hasbeen observed regarding the therapeutic approach to septic shock: she is planning arandomized controlled clinical trial (DECRISS), studying the effect of Cytosorb therapy inpatients with septic shock. In addition, she is a co-investigator in the retrospective cohortstudy describing the course and prognosis of patients with severe acute pancreatitis requiring(sub)intensive care. Piroska is participating in a 12-month resident fellowship programme. The acuteexacerbation of chronic obstructive pulmonary disease (AE-COPD) is one of the most commonreasons for hospitalization of pulmonary patients. Elevation of inflammatory biomarkers,including C-reactive protein, neutrophil-lymphocyte ratio and interleukins are often observed inAE-COPD. To test the diagnostic and predictive ability of these biomarkers in hospitalizedpatients with infective and non-infective AE-COPD, she is currently working on a meta-analysis. Secondly, she initiated a retrospective cohort study to describe the course andprognosis of patients with severe acute pancreatitis requiring (sub)intensive care, whileorganizing a patient registry for the surveillance of these patients. In addition, she isresponsible for planning and initiating a randomized controlled clinical trial studying theeffect of Cytosorb therapy in acute pancreatitis.

Ágnes is participating in a 12-month resident fellowship programme. She plans to set up apatient registry to develop a national registry for patients living with percutaneous endoscopicgastrostomy (PEG) both in adults and in children. She is currently investigating the diagnosticmetrics of different screening modalities, including elastography for cystic fibrosis-related liverdisease by performing a diagnostic accuracy-test meta-analysis. She is involved in basicresearch, as well as in conducting a study comparing the genetic samples of patients with biliarypancreatitis, non-biliary pancreatitis (control group I), gall-stones without pancreatitis (control groupII) and patients without gallstones or pancreatitis (control group III - healthy control group). Aftermultiplying the DNA samples with PCR, gel-electrophoresis will be used to detect two exons of theCFTR gene (dF508 and R117H).

Orsolya is participating in a 12-month resident fellowship programme. She is now performing anetwork meta-analysis of intervention, used to eradicate Pseudomonas aeruginosa in cysticfibrosis patients. The meta-analysis aims to involve all therapeutic agents separated by therapeuticregime and to investigate the recurrence rate of the infection, as the primary outcome. In addition,she is performing a retrospective cohort study on children with Pseudomonas infection in cysticfibrosis to assess antimicrobial resistance, clinical symptoms and the effect of infection on lungfunctions. Meanwhile, she will set up a patient registry of these patients.
