ENTRANCE EXIT SURVEY SYSTEM Entrance/exit survey system is an evaluation form which has to be done by the instructors for every semester and it is actually related to every course that is offered to the students. Entrance survey has to be done at the beginning of the semester which is from the first week until the fourth week to measure the level of background knowledge of the students that is regarding the courses that they have registered. As for the exit survey, it has to be done at the end of the semester (week 13 and week 14) to re-evaluate the level of knowledge of the students after they have finished studying about the courses, that is whether there are any improvement or otherwise. This evaluation is one of the requirements in the “Outcome Base Education”. This system has been used since May 2011 until October 2011. Requirements as a Resource Person (RP):- 1) Appointed by the faculty/campus. 2) Has to register as an i-Learn user. Then, person-in-charge on i-Learn will register the lecturer as a Resource Person. 3) The lecturer has to be prepared, that is regarding the material which actually refers to the Course Outcome (CO). Guidelines for the Lecturers 1) Login to i-Learn Portal. 2) Enrol the subject if the subject is not listed in the existing list. Then, Click at the subject.

ENTRANCE EXIT SURVEY SYSTEM

Entrance/exit survey system is an evaluation form which has to be done by the instructors for every semester and it is actually related to every course that is offered to the students. 1) Login to i-Learn Portal. 2) Enrol the subject if the subject is not listed in the existing list. Then, Click at the subject.

1) Subjects’ list page. Click one of the subjects to enter the entrance/exit survey.

2) Click at the Entrance/ Exit Survey link. Then, choose the desired/ related semester.

3) Click at the Create Survey link to enter the material. The Create Survey link will only appear if the lecturer is a Resource Person for the chosen subject.

The page below will also show the name of the Resource Person for the chosen subject. Users have to use Mozilla Firefox browser for the purpose of entering the survey.

4) The screen below will appear after the Create Survey link is clicked. Please fill-in the necessary information. Example on how to fill-in the Description:- Please complete this inventory by circling the appropriate ratings.

5) To enter the information for the Course Outcome (CO), please Click +Question button.

If you want to remove the Question, please Click the –Question button.

6) To add the number of the Course Outcome, please Click “add new course outcome” button, as shown below.

7) After you have confirmed with the Course Outcome that you have entered, please Click

“Submit” button to save the information.

8) I-Learn centre will activate the survey automatically starting from the first day of lecture. The survey will be de-activated on the first day of Week 5. Resource Person (RP) has to prepare the survey earlier that is one week before the survey is activated to make sure that the survey is being prepared and there are no changes in the questions.

9) There are other additional menus such as Preview, Result, Edit and Delete. Only the Resource Person is been permitted to Edit and Delete. The following page shows each of the menus stated before.

Please Click the following menu to see the students’ page.

The following shows the screen page that will be seen by the students.

b) View Result

Click the following menu to see the analysis of the survey.

The analysis of the survey can also be seen by clicking the Survey link, as follows:

The following shows the analysis of the survey that has been answered by the students. Lecturer/ Resource Person can also view the analysis by filtering the information into Program and Group, as shown below.

c) Edit Survey Click Edit menu to update the Course Outcome (CO) that has been entered before. Course Outcome that has been entered before can only be updated when there are no respondents/ students online. The Edit menu will only appear when the number of the respondents is zero.

d) Delete Survey Resource Person can delete/ remove the survey that is not needed anymore.

Pop-up message will appear when the Delete button is clicked.
