MISSION STATEMENT Southwest Acupuncture College is committed to the prevention of incidents that result in injury and/or illness and will comply with all applicable federal and state health and safety rules. Therefore, we require that:

1. Management spares no effort in providing a safe healthful work environment for all employees, instructors and students.

2. All levels of supervision are accountable for the health and safety of those employees, instructors and students under their direction.

3. This written Emergency Action Plan assigns responsibility to ensure performance under that responsibility.

All clinic and classroom divisions are included in this program. The written plan will be available in the Southwest Acupuncture College Administrative Director’s office at each campus for review by any interested employee. We, the Southwest Acupuncture College staff, will meet the requirements of these operating procedures as follows:

FIRE PROTECTION EQUIPMENT AND SYSTEMS PORTABLE FIRE EXTINGUISHERS

Fire extinguishers are mounted in readily accessible locations. All are recharged regularly and it will then be noted on the inspection tag. All employees are trained in the use of the extinguishers and fire protection procedures. The Clinic Directors in Santa Fe and Boulder will be responsible for ensuring this process. Santa Fe: 1622 Galisteo Street This location has a total of eight extinguishers. Five are located on the first floor of the building. Two are located on the left wing of the first floor. Extinguisher #1 is in the Administration Lobby. Extinguisher #2 is in the stairwell at the end of the hallway. Extinguisher #3 is located in the Foyer. Two are located in the right wing of the first floor. Extinguisher #4 in located in the student lounge next to the refrigerator. Extinguisher #5 is located at end of the classroom hallway left exit. Three are located on the second on the second floor. Extinguisher #6 is located in the Foyer across from the Staff Lounge. Extinguisher #7 is located next to the Pharmacy. Extinguisher# 8 is outside Exam Room #13. Boulder: 6620 Gunpark This location has four fire extinguishers. The first is located in the hallway at the front door, the second is near the restrooms on the first floor, the third is near the restrooms on the second floor and the fourth is in the upstairs foyer between the Lotus and Jade classrooms. 6630 Gunpark - Clinic There are four fire extinguishers in the clinic. One is located in the student lounge. One is located in the hall across from the workroom. One is located in the east hallway between treatment rooms 6 & 7. The fourth is located in the north part of the building, in the hall between consultation rooms 3 and 4.

6630 Gunpark – Administrative Offices There are two fire extinguishers in the Administrative offices. One is located at the front door to the main office and the second one is located outside the lounge area in the main hallway. Fire extinguishers are mounted in readily accessible locations. All are recharged regularly and it will then be noted on the inspection tag. All employees are trained in the use of the extinguishers and fire protection procedures. The Administrative Director in Boulder will be responsible for ensuring this process. EMERGENCY FIRE ALARMS

Santa Fe: 1622 Galisteo Street There are ten Emergency fire alarms located in the building. Seven are located on the first floor. Two are located in the foyer. Fire alarm # 1 is located in the hall to the left of the front entrance to the building, before the Administration Lobby. Fire alarm # 2 is located next to the exterior door. Two alarms are located on the left wing of the building. Fire alarm # 3 is located at the end of the hallway next to the right exit door. Fire alarm #4 is located on the right outside the left interior exit door. Two alarms are located in the right wing of the first floor. Fire alarm # 5 is located next to the exterior doors of the student lounge. Fire alarm # 6 is located at the end of the hallway next to the left exit door. Fire alarm #7 is located next to the right exit door near the student mailboxes. Three fire alarms are located on the second floor. fire alarm # 8 is located in the foyer to the left of the Staff Lounge door. Fire alarm # 9 is located on the right next to the door to the clinic hallway and the Pharmacy. Fire alarm # 10 is located at the end of the clinic hallway across from Consultation Room # 4.

Boulder: 6620 and 6630 Gunpark Dr. The fire alarms in all Boulder campus locations are automatic integrated systems including installed smoke detectors.

Fire alarm systems at all campuses will be tested annually. SPRINKLER SYSTEMS

Southwest Acupuncture College buildings are built to comply with fire codes. Sprinkler systems are maintained in the buildings where they are required by code. All sprinkler heads are protected by metal guards to prevent physical damage. Proper clearance is maintained below the sprinkler heads at all times.

FIRE HAZARDS, IGNITION SOURCES & HAZARDOUS CHEMICALS

The following is a list of all known Hazardous Chemicals used by our employees. More information on each chemical is listed on their respective Safety Data Sheets (SDS), (formerly MSDS) in the SDS Binders that are available in the Campus Director’s office and the clinic. In the Santa Fe clinic the SDS binder is available in the Herb Pharmacy, and in Boulder in the Clinic Reception office. In 2015 OSHA implemented the Globally Harmonized System which is a common and coherent approach to classifying chemicals and communicating hazard information,

appropriate handling and safe use of chemicals. It requires a written program, SDS sheets, proper labeling and training (OSHA). Labeling for chemicals have all been updated to contain 6 pieces of information to communicate the hazards of the chemicals: 1. Name of chemical, 2. Signal Word (Danger/Warning) 3. Hazard Statement 4. Pictograms (see chart below) 5. Precautionary Statement (including 1st Aid) 6. Supplier Information. All chemicals are currently kept in a storage room. There are no ignition sources in the area and the present fire protection equipment and system is able to control these chemicals if ignited. Employees are aware of the fire hazards of the materials. The Clinic Manager in Santa Fe and in Boulder will be responsible for the identification of SDSs and obtaining them for all chemicals brought into Southwest Acupuncture College. SDSs are maintained in the Hazard Communication Plan.

EVACUATION ROUTES AND PROCEDURES

Evacuation routes are posted in every room of Southwest Acupuncture College. All signs have a primary and secondary route listed. All employees, faculty and students are to become familiar with each route so they may safely direct individuals from the building if a fire occurs.

The Clinic Manager in Santa Fe, Campus Director in Boulder will be responsible for ensuring that everyone is trained in this process, and that evacuation drills take place on a random basis once per semester. PROCEDURES In the event of fire: Santa Fe: 1622 Galisteo Street

1. Any fire alarm signal is to be treated as real. 2. No one is to enter the elevator for any reason. 3. Interns are to go to their assigned treatment rooms to ensure that their patients

are assisted out of the building via established evacuation routes. 4. Supervisors are to follow behind their observer and intern groups to ensure that

all persons (students and patients) are safely out of their sector of the building. Supervisors should take their attendance books with them out of the building to ensure that all students are accounted for once evacuated.

5. The Clinic Receptionist will ensure that all persons in the reception area and restrooms are safely evacuated from the building using established evacuation routes. The Receptionist will take the appointment book to use as a checklist to ensure that all patients, students and staff are accounted for once evacuated.

6. The Administrative Assistant will check the faculty lounge, student lounge and restrooms to ensure that all persons are safely evacuated from the building using established evacuation routes.

7. The Administrative Director will ensure that all office personnel are safely evacuated from the building using established evacuation routes.

8. Classroom instructors will ensure the evacuation of their classrooms using established evacuation routes. All instructors should take their attendance books with them out of the building to ensure that all students are accounted for once evacuated.

All personnel from 1622 Galisteo Street will meet at the Casa Real service road to the west of our building to assess the whereabouts of all individuals under their supervision. Any missing people will be reported immediately to fire fighting personnel.

Boulder: 6620 Gunpark

1. Any fire alarm signal is to be treated as real. 2. The instructor in the Dragon Room will check the second floor including the

student lounge, Jade & Lotus classrooms, restrooms and their classroom to ensure that all individuals are evacuated via established routes.

3. The instructor in the Jade Room will evacuate their classroom to ensure that all individuals are evacuated via established routes.

4. The Librarian or Library Assistant will check the first floor, including the restrooms and the library, to ensure that all individuals are evacuated via established routes.

5. Classroom instructors will ensure the evacuation of their classrooms using established evacuation routes. All instructors should take their attendance books/computers with them out of the building to ensure that all students are accounted for once evacuated.

It is a deliberate release of viruses, bacteria, or other germ agents used to cause illness or death to people, animals, and plants. These biological agents can be spread through the air, through water or food, and person to person. Terrorist may use biological agents because they can be extremely difficult to detect, and may cause widespread illness, death, and panic before these agents can be identified and isolated. Which biological agents are most threatening?

• Anthrax � Smallpox • Botulism � Tularaemia • Plague � Ebola, Junin, Marburg, and other hemorrhagic fevers.

What can you do to prepare for a bioterrorism attack? • A scenario of this magnitude is for the most part out of our control.

However, we are responsible for maintaining a healthy state of mind. • Local hospitals, the Red Cross, CDC, and The Department of Homeland Security

are the agencies that would be overseeing this type of disaster. • We might be required to shelter-in-place, or face a period of isolation or

quarantine. • You may be required to vaccinate or take a course of antibiotics. • It is advisable to have a supply of clean drinking water and food that can carry

you through a period of at least one week. • Close and seal windows and doorways with towels and sheets to prevent

possible further exposure. • Wear personal protective equipment. ( masks, gloves, gowns, goggles. ) • Have a battery powered radio to keep informed with what is happening in your

area.

EMERGENCY PREPAREDNESS & REPONSE TO VIOLENCE ON CAMPUS

Definition

• Verbal abuse. • Any physical or sexual assault. • Harassment, intimidation, or coercion. • Threats or actual attempts to cause injury to another student, faculty, staff or co-

worker. • Attempted or successful suicide.

Basically, campus/workplace violence is any act that results in threatened or actual harm to people or property. All such acts leave trauma and fear in their wake. Warning Signs

• Direct or veiled threats of harm. • Intimidating, belligerent, harassing, or other inappropriate or aggressive

behavior. • A person involved in numerous conflicts with fellow students, or staff. • Making inappropriate references to or a fascination with weapons. • Bringing or brandishing a weapon on campus. • Making statements indicating a fascination with or approval of violence. • Extreme changes in behavior such as increased absenteeism, mood swings,

deterioration of academic performance, deterioration of personal hygiene. What to do if faced with a violent or potentially violent situation If you notice a fellow student or co-worker exhibiting some or all of the above behaviors, report it to a supervisor, or administrative personnel immediately. It is not your responsibility to determine if there is a real threat, or to provide counseling to the individual. It is your responsibility to alert others and have someone call 911. If someone is angry, hostile or threatening:

• Stay calm and listen attentively. Be courteous and patient. • Maintain eye contact. • Try to keep the situation in your control. • Discreetly signal a fellow student or supervisor that you need help. • If you are engaged with them, have someone else call 911.

If someone is threatening you with a weapon:

• Stay calm, quietly signal for help and maintain eye contact. • Stall for time. • Keep talking, but follow their instruction. • Do not try to grab the weapon. • Watch for a safe chance to escape. • Call 911, or have another person call, as soon as possible.

EMERGENCY PREPAREDNESS & RESPONSE IN CASE OF A TORNADO

Signs of an Approaching Storm Sometimes tornadoes strike rapidly, without time for a tornado warning, and sometimes without a thunderstorm in the vicinity. When you are watching for rapidly emerging tornados, it is important to know that you cannot depend on seeing a funnel: clouds or rain may block your view. The following weather signs may mean that a tornado is approaching:

• A dark or green-colored sky. • A large, dark, low-lying cloud. • Large hail. • A loud roar that sounds like a freight train.

If you notice any of these weather conditions, take cover immediately, and keep tuned to a local radio station. The key to surviving a tornado and reducing the risk of injury lies in planning and preparing. Flying debris causes most injuries and death during a tornado.

What to do in the Event of a Tornado

• Move away from windows and glass doorways. • Go to the innermost part of the building on the lowest possible floor. • Do not use elevators because the power may fail, leaving you trapped. • Protect your head and make yourself as small a target as possible by crouching

down. • Take shelter under a sturdy table or desk. • If you are with a patient during a treatment. Remove the needles, crouch under a

heavy wooden treatment table and cover yourselves with pillows and blankets to avoid falling objects if there is not time to get to the ground floor hallway.

• Do cover your head with anything available, even your hands. What to Do after a Tornado

• Be aware of hazards from exposed nails and broken glass. • Check yourself and others for injuries. Do not attempt to move injured people

unless they are in immediate danger of further injury. • Get medical assistance immediately if needed. • If someone has stopped breathing, begin CPR if you are trained to do so. • Stop a bleeding injury by applying direct pressure to the wound. Have any

puncture wound evaluated by a physician. • If you are trapped, try to attract attention to your location.

LOCKDOWN PROTOCOL 1. The following protocol shall be followed to ensure a unified response from law

enforcement and emergency personnel. 2. Each classroom should have an Emergency Folder by the exit door and should

contain a Standard Response Protocol (SRP). 3. Exterior doors to rooms that are occupied are promptly closed and locked. See

Lockdown Steps below. 4. Lockdown drills should be performed once a year.

LOCKDOWN STEPS

1. Bullhorn will sound off. 2. IF YOU ARE IN A CLASSROOM, FOLLOW THESE INSTRUCTIONS:

• Teacher gather attendance book – close and lock classroom door and shut off lights. – Take SRP from Emergency folder.

• Teacher and students quickly move out of sight. Please remain Quiet! • Wait for First Responders Only to open the door. • Take roll to account for all students. IF YOU ARE IN THE CLINIC, FOLLOW THESE INSTRUCTIONS • Supervisor close blinds in occupied rooms, interns stay with their patient, all

others stay in Clinic treatment room hallway. Please remain Quiet! • Clinic Manager close window blinds in office, usher waiting room patients

into the clinic hallway, lock main door to lobby area and lock hallway door. - CALL 911

• Waiting room patients move to interior hallway of clinic. Please remain Quiet!

Bathrooms/Lounges: • Return to your classroom as quickly as possible

Library: • Call 911 - Close and lock door to lobby area, keep lights on. • Move yourself (staff) & any students to Administrative workroom. Bullhorn will sound to indicate Lockdown is over.
