The Golden Rule for Energy Isolation

• Any isolation of energy systems; mechanical, electrical, process, hydraulic and others, can not proceed unless:

the method of isolation and discharge of stored energy are agreed and executed by a competent person(s)

any stored energy is discharged a system of locks and tags is utilised at isolation points a test is conducted to ensure the isolation is effective isolation effectiveness is periodically monitored

• There have been Energy isolations immersion and improvement program over the past several years but unsafe energy isolations still occur:• BP E&P Group: 9 HiPOs in 2006. • BP Indonesia: 4 Recordable Incidents in 2006 (3 Tangguh, 1 VICO).

• The Objective:Continue building awareness in order to improve hazardassessment to create a safe energy isolation which aligned withControl Of Work Standard requirement.

Isolate Energy – important notes

• Do Risk Assessment• Energy Isolation have to be approved (by Isolating Authority),

analysed, discussed and understood by workers involved.• Energy Isolation have to be monitored and conducted by a competent

person. • Energy Isolation has not been accomplished if the following tools have not

been installed (avoiding another person to open the isolation without permission):− Lock out – to secure the isolation will not be opened until the work

finished− Tag out – to inform the purpose of the isolation.

EI Incident Lesson Learned:Pile Driving Barge Flash Fire IncidentTangguh LNG, Jan 06, 2006Actual Outcome: 1 Restricted WorkThe injured received 1st Degree burns to his faceCaused by a flash fire erupted from the

exhaustport and minor injuries to his eyes while

performingroutine troubleshooting inspection of a poorlyoperating diesel powered pile driving hammer.

EI Incident Lesson Learned:Vibro Hammer Incident

• Tangguh LNG, Feb 12, 2006• Actual Outcome: 1 Medical Treatment

Injured employee was part of a 4 man crew which maintained the vibro hammer. There are 3 vibro hammers on site; 2 of them powered by 220V generators, the rest is powered by 380V generator. The employee terminated the 380V generator, and hooked it up into one of the 220V system.When one of the employee attempted to switch on the pump that controlled the chuck of the vibro hammer, it blew the cover off the switch and caused a flash fire. The worker received 2nd degree burns to his right hand.

EI Incident Lesson Learned:Hook Release Incident

• Tangguh LNG, May 01, 2006• Actual Outcome: 1 Medical Treatment

The victim was assigned to assist in the transferring of materials from Crane Barge HI 32 to LCT Clever. The hook on the 6 ton sliding choker became stuck between the c-channel and the metal deck on the LCT. The rigging team could not release it. Both the victim and his co-worker each used a piece of timber pole in an attempt to raise the c-channel bundles to release the hook. Immediately after the hook was released, the victim's co-worker released his grip on his timber pole. This caused the victim's timber pole to fall back against the metal deck of the LCT Clever and caught his middle finger between the pole and the deck.

HIPO Incident – Wytch Farm, UK

• September 12, 2006• An operation was being planned to install

electrical cables into an SCR ( Silicon Controlled Rectifier ) electrical switchroom which supplies electric to the drilling mud pumps. The electrical distribution system is 600 volts and 1600 Amp rated. A worker lifted and shifted a roof panel (wood/metal/plastic sandwich). When this was done the panel contacted a live termination which was out of sight above the roof panel. This created a short circuit to earth, tripping the circuit breakers and shutting down the generator. The worker was checked by the paramedics and the BP doctor who confirmed that he did not receive an electric shock.

HIPO Incident – GUPCO, Egypt

• September 09, 2006• Over pressurization of a Glycol/HC Condensate Vessel occurred as a

result of gas blowby from a Low Temperature Separator (LTS) when a shut down valve failed to close on low-low level in the separator. Gas entered the liquid side of the process over-pressuring the system to approx. 1.5 times normal working pressure. Contributing to the over pressure incident was the malfunction of a safety relief valve (PSV) on the vessel, and failure to operate of a second PSV on the inlet to the condensate pre-heater. Operators quickly spotted the malfunctions and relieved pressure by opening a manual by-pass valve.

HIPO Incident – Shah Deniz, Azerbaijan

• September 11, 2006• A flame arrestor cap approximately five inches in diameter and

weighing approximately 1.5 kg came off a local vent as a projectile following a valve opening for depressurisation. The flame arrestor cap traveled approximately 45-50 meters before landing on a drilling rig walkway 33 meters above the rig floor. A nitrogen leak test was ongoing on the export gas pig launcher following the removal of de-watering pipework and re-instatement of blinds. The test pressure was 175 barg. The top of the local vent with the flame arrestor was one level above the pig launcher, and the operator could not see that an incident occurred. One of drilling deck crew heard the impact and reported it to the night shift Deck Foreman.
