    ENLISTED ASSOCIATION NATIONAL GUARD OF ARIZONA

    Effective 1 October 2012, Southwest Truck Driver Training (SWTDT) has established ascholarship program for members of the Enlisted Association National Guard of Arizona(EANGA). The purpose of this scholarship is to provide a full scholarship from SWTDT to anEnlisted National Guard member who is a member of EANGA. The scholarship provides forattendance at the SWTDT four week training program to earn a Class A Commercial DriversLicense. A Scholarship Committee appointed by the EANGA President will select a primaryand alternate candidate.

    The applicant must meet the following criteria to apply for the scholarship:

    1. All applicants must be a current Annual or Life member of the Enlisted Association(EANGA) at the time of application.

    2. All applicants must be Enlisted members of the Arizona National Guard assigned in anactive DSG or M-DAY status, Technician, or AGR Title 32 status, or must be a formerEnlisted member or Retiree of the Arizona National Guard. Applicant must provide proof ofcurrent Enlisted AZNG service; retirees or former members must provide an NGB 22,DD214 or equivalent documentation.

    The applicant must provide the following documents for consideration by the EANGAScholarship Committee:

    1. EANGA/SWTDT Application Form (attached)2. A 250-500 word letter from applicant with personal, specific facts as to his/her desire toattend the SWTDT course and why financial assistance is required.2. Three (3) letters of recommendation giving general moral and personal traits (these lettersshould be from your Unit Commander/First Sergeant, community leaders, college instructors,ministers, elected officials, etc.).3. The Southwest Truck Driver Training Pre-Application Questionnaire (attached)4. A military service record (proof of current AZNG membership; Retirees/former members:NGB 22, DD-214 or equivalent).5. A personal ADOT Motor Vehicle Record (MVR).

    Upon notification of selection, the scholarship recipient must take and pass the ADOT MotorVehicle Division Medical Exam. The ADOT Medical Examination form may be viewed at:http://mvd.azdot.gov/mvd/formsandpub/mvd.asp?txtNumber=40-1501 The recipient will alsopay the nonrefundable $30 application fee ($25 medical screening/$5 processing fee), however,this cost may also be waived/sponsored by EANGA/SWTDT upon proof of financial hardship.

    The scholarship will be awarded by SWTDT when the medical exam and all pre-screeningprocesses are completed successfully by the selectee, and upon enrollment into Southwest TruckDriver Training program.

    There shall be no consideration whatsoever given because of friendship, rank or grade ofapplicant. Neither shall the applicant's geographical place of residence be considered. Awardwill be made on the basis of qualifications, character, leadership and need.

    Former recipients of the SWTDT Scholarship Award are not eligible to reapply. Those personswho have applied for scholarship award in the past and were not selected may reapply.

    If the recipient does not complete the course term for which the scholarship is awarded due toany cause other than sickness or physical injury, the recipient will refund the award to SWTDTthrough the Enlisted Association National Guard of Arizona within sixty (60) days from the dateenrollment has terminated.

    Each application will be reviewed for completeness. Applications must include all of therequired materials. Deadline date for all applications for scholarship will be observed in allinstances.

    The composition of the EANGA / SWTDT Scholarship Committee shall be made up of at leastthree (3) members, as follows:

    1. The EANGA Scholarship Committee Chairman.2. An appointed representative from SWTDT.3. At least one, and no more than three, additional persons appointed by the EANGAPresident and/or Scholarship Chair.

    There will be a specific number of scholarships awarded within a time frame deemed bySWTDT. The application for scholarship must be postmarked no later than the date establishedby the EANGA President. The suspense date for this year is NLT November 15, 2012.
